This study included 20 preterm babies with different gestational ages and birth weights who were admitted to the NICU during that period. The babies’ response to noise, in terms of changes in heart rate, respiratory rate, blood pressure, oxygen saturation and discomfort level, were assessed with ear plugs and without ear plugs on alternate days and the results were recorded. Descriptive statistics were used for statistical analysis. Results − On days 1 and 3, mean heart rate and systolic blood pressure were lower in babies wearing ear plugs (141.55±2.42 SD and 60.1±11.58, respectively) than in the same babies without ear plugs (157.55±2.52 SD and 61.85±1.6 SD, respectively) on days 2 and 4. The mean oxygen saturation was higher in babies wearing ear plugs (96.7%±0.80 SD) than in the same babies without the intervention (93.75%±1.29 SD). Both brothers presented with growth impairment - the older one at the age of 2.5 years, and the younger one at the age of 3.3 years. Central hypothyroidism and GH deficiency were established in both of them. Replacement therapy was started using levothyroxine and recombinant growth hormone. Pituitary magnetic resonance imaging was normal. Genetic analysis identified a pathogenic homozygous frameshift variant c.301_302delAG in the PROP1 gene.Conclusion − Patients with PROP1 mutation must be closely followed up to reveal other possible hormonal defects and to avoid possible life-threatening adrenal crisis in the future. Subjects andMethods − Term infants aged up to 3 months, admitted to Ljubljana University Children’s Hospital due to acute bronchiolitis in the period from January to April 2020, were studied prospectively. LUS score and clinical assessment score (CAS) were determined upon admission. The patients were divided into groups according to LUS score and CAS. Clinical and laboratory characteristics were compared between the groups. Correlations between LUS score, duration of clinical signs prior to admission, CAS and partial carbon dioxide pressure in the capillary blood (PCO2) upon admission, length of hospitalization and duration of supplemental oxygen (O2) therapy were analysed. Additionally, the predictive value of LUS and CAS for the need of non-invasive respiratory support (NRS) was calculated.Results − The LUS score correlated with the clinical severity of acute bronchiolitis. Patients with higher LUS score had higher CAS (P<0.001) and PCO2 upon admission (P=0.014), and needed O2 therapy for a longer time (P=0.023). These patients also required NRS (P=0.024) more often, were positive for respiratory syncytial virus (P=0.008), and had a chest X-ray performed (P<0.001). The LUS score correlated well with CAS (P<0.001, r=0.762). On the basis of the laboratory and clinical parameters, the PRISM III score was calculated at admission. Lactate levels were measured at admission and 24 hours after admission.Results − The study included 107 patients of whom the majority (54.2%) were female. The median patient age was 3 years and the median duration of PICU stay was 4 days. Among the hospitalized patients, 90.7% were survivors, with female patients having an 8.8-fold higher risk of mortality compared to males (P=0.05). The serum lactate value in survivors (2.95 mmol/L) was much lower in comparison to non-survivors (3.11 mmol/L). The PRISM III value measured at the time of admission in survivors (8) was low when compared to non-survivors (16.50). The area under the curve (AUC) for PRISM III was 0.884 compared to lactate levels (0.831).Conclusion − A significant positive correlation was seen between the PRISM III score and lactate levels. In 2022, now being 17-year-olds, 3rd-grade high-school students filled out the YP-CORE questionnaire once again during the COVID-19 pandemic. Socio-demographic data were collected from the participants’ health charts.Results − The median YP-CORE score significantly increased from 7 in 2020 to 11 in 2022. A significant increase in psychological distress was determined, from 11% of adolescents in the pre-pandemic period to 23% of adolescents in the pandemic period. Significantly higher rates were reported by girls in both periods. Students from the School of Applied Arts and Design reported psychological distress significantly more often than students from the Electrical Engineering and Traffic School and the Grammar School before and during the pandemic. Lymphadenopathy is a common finding in the paediatric population. Due to the many pathologies that can present in a similar fashion, imaging is a valuable method used in diagnostic process. Ultrasound is the modality of choice for lymph node detection in children, with a high safety profile as it does not use radiation. In this review the US appearance and characteristics of normal lymph nodes, and of some more common lymphadenopathies, are presented. Special attention is paid to distinguishing between benign and malignant lymph nodes on the basis of US characteristics. All findings must always be correlated with clinical and laboratory findings, and frequently followed by fine-needle aspiration or excisional biopsy as some US morphologies of benign and malignant nodes may overlap. {"title":"The World of Sound and Speech in Pediatric Cochlear Implant Patients: An Experience from a Tertiary Care Centre in Central India","authors":"Minhajuddin Ahmed, Mohammed Iqbal Ansari, Sachin Gupta, Nitesh Upadhyay","doi":"10.5457/p2005-114.334","DOIUrl":"https://doi.org/10.5457/p2005-114.334","url":null,"abstract":"Objectives − To assess the effect of auditory and speech rehabilitation according to the revised Category of Auditory Performance (CAP) and Speech Intelligibility Rating (SIR) score in patients who underwent Cochlear Implant (CI) surgery at a tertiary care centre in Central India.Patients and Methods − This was a retrospective interventional study conducted at the Department of Pediatrics and Otorhinolaryngology of Chirayu Medical College and Hospital, Bhopal, Madhya Pradesh, during the period from 2014 to 2018, to assess the hearing and speech of the patients who underwent CI, and was approved by the Institutional Ethical Committee.Results − A total of 114 patients underwent surgery using the mastoidectomy-posterior tympanotomy (MPTA) approach at our centre. Of these, 61 (54%) were males and 53 (47%) were females, with a mean average age of 24.66 months. The number of patients with a right ear defect was 107 (93%) whereas with left ear defect there were only 7 (6%). Six patients were lost to follow-up. The majority of children fell into the 4th category followed by the 2nd category of CAP scoring, and in SIR scoring the majority of children fell into the 4th category, followed by the 3rd and 2ndcategories, which is statistically significant.Conclusion − CI surgery is the gold standard for prelingual deafness. Early detection and early implantation of a CI is highly recommended for patients to enable them to live a healthier life. The study also indicated that early CI reduces the discrepancies between physical and expressive age. Clinical records revealed that he had been found to have left hand lesions, global developmental delay, and height below the 3th percentile in a previous assessment. The combination of severe hypocalcemia, hyperphosphatemia, increased PTH levels, and identification of a mutation on a GNAS genetic test, confirmed the diagnosis of type Ia pseudohypoparathyroidism (PHP Ia).Conclusion − In this case report, the combination of clinical and biochemical findings led to the diagnosis. Elementary students in the Indonesian context have many problems in daily breakfast and vegetable consumption. Therefore, health education programs to improve their nutritional status should be conducted in context. Further, this study sought to examine a health education program using a SIMON Comic with an emic Papua approach for improving food intake of breakfast and vegetables among elementary school students in Indonesia.Method - A quasi-experimental study design with a pre-test-post-test control group was performed on 60 elementary school students (30 in the intervention and 30 in the control group). The intervention group received the comic for one month. Meanwhile, the control group received a leaflet. Food intake of breakfast and vegetable were measured using a self-administred questionnaire to measure knowledge, attitudes, and practice. Data were analysed using the independent T-Test.Results - The SIMON comic with an emic Papua approach significantly increased students’ knowledge about improving breakfast and vegetable consumption, P-value = 0.001). However, there was no significant attitude and practice change between comic and leaflet groups in breakfast and vegetables consumption (P-value>0.05).Conclusion - The SIMON Comic with an emic Papua approach improved knowledge of students about breakfast and vegetable consumption.
