Common methods for assessing tibial torsion include the Transmalleolar Axis (TMA) and Thigh Foot Angle (TFA). While TMA and TFA are highly reliable, their validity is limited, often overlooking a crucial variable – the foot. External rotated TFA is typically linked to a pronated foot, while a supinated foot is associated with internal rotated TFA. This association arises from foot alignment, rendering the measurement not truly reflective of tibial torsion. We propose measuring tibial torsion in subtalar neutral and in a simulated weight-bearing position to address the impact of foot alignment on TFA measurement.Conclusion - This innovative approach to TFA measurement has the potential to enhance diagnostic accuracy in clinical settings, thereby improving clinical decision making regarding treatment choices. Data regarding sociodemographic factors, habits of informing oneself regarding the health status, vaccination and vaccines and knowledge of and attitudes toward vaccination were collected.Results − The study showed that Croatian adolescents most often obtained information regarding vaccination from the internet, although they reported health professionals to be the most reliable source. Adolescents reported poor knowledge about vaccination. Better knowledge was reported by adolescents who inform themselves about vaccination through scientific literature or school teachers than those who use the internet and social contacts. Results showed that the negative attitude toward vaccination was associated with the internet as the source of information, female gender and poor knowledge about vaccination.Conclusion − Adolescent mostly get information about vaccination on the internet as the easiest accessible source. Efforts should be made to make relevant sources of information more available to adolescents since good knowledge correlates with positive attitude toward vaccination. {"title":"Assessment of Physical Condition in Cystic Fibrosis Patients during 12-month Treatment with Cystic Fibrosis Transmembrane Conductance Regulator Modulators","authors":"Lovro Hrvoić, D. Vukić Dugac","doi":"10.5457/p2005-114.354","DOIUrl":"https://doi.org/10.5457/p2005-114.354","url":null,"abstract":"Objective − To investigate the effect of cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapy on physical condition in children with cystic fibrosis (CF) during a 12month period.Materials and Methods − This is a retrospective cohort study including children aged ≥5 years treated with Elexacaftor/Tezacaftor/Ivacaftor (ELX/TEZ/IVA) or Lumacaftor/ Ivacaftor (LUM/IVA). A six-minute walk test (6MWT) was performed at baseline and at the 3-month follow-ups. Changes in spirometry and sweat chlorides, and body mass index (BMI) were also observed. Collected data were analysed for changes in 6MWT results in correlation with other parameters. The 6MWT results were compared to those predicted for healthy peers. Missing values were replaced using imputation.Results − Study includes 34 patients (median age 14 years); 28 received ELX/TEZ/ IVA, and 6 LUM/IVA. The average 6MWT walking distance (6MWD) increased from 519.3±107.7 m at baseline to 620.0±99.5 m after 12 months (P<0.0001). The 12 month 6MWD values matched those expected for healthy peers. The increase in BMI z-score (P=0.019) and ppFEV1 (P<0.0001) visible at the 3-month followup was sustained throughout the rest of the year. Sweat chloride concentration decreased (P< 0.0001); after 12 months 13/34 subjects had values below 60 mmol/L. No correlation between 6MWD and other parameters was observed.Conclusion − Significant improvement in 6MWT results was already visible at three months following the initiation of CFTR modulator treatment. Sigmoid volvulus in pediatric patients is a rare but potentially life-threatening condition. This condition occurs when a redundant sigmoid loop, with a narrow mesenteric base of attachment to the posterior abdominal wall, rotates around its mesenteric axis.Case Report − A 13-year-old boy presented as an emergency with a 3-day history of back pain and abdominal distention. Plain abdomen X-ray showed dilated large bowel loops in the upper left quadrant. An abdominal computed tomographic scan revealed a bowel obstruction with swirling of the mesentery, which suggested a sigmoid volvulus. The redundant sigmoid colon was hugely dilated, and was resected after detortion and resection. {"title":"Scabies in a Preterm Infant Younger than 2 Months: What Do We Have?","authors":"Victor Aguilar, Aniza Giacaman, Cristina Maroto, Ana Martín","doi":"10.5457/p2005-114.351","DOIUrl":"https://doi.org/10.5457/p2005-114.351","url":null,"abstract":"Objective – To present a case of scabies in a preterm neonate treated with an off-label treatment (oral ivermectin) without any adverse effects.Case Report – A preterm female neonate (29+3 weeks of gestation) was diagnosed with scabies at 35 days of life (34+3 weeks of corrected gestational age), after her parents had been diagnosed, having been admitted at birth to the neonatal unit of a tertiary hospital. Treatment was initiated with topical off-label permethrin 5%, with the informed consent of the parents. Concomitantly with the patient, the parents received treatment with permethrin 5% and oral ivermectin on days 0, 7, 14 and 21 after diagnosis. No adverse effects resulting from the use of these treatments were reported. On physical examination, active lesions due to scabies were still observed two weeks after permethrin treatment, so it was decided to re-administer the treatment with topical 5% permethrin on days 0 and 14, associated with oral Ivermectin at 200μg/kg on days 0 and 7. The treatment was administered to the patient and her entire family. Fourteen days after completion of ivermectin treatment, the patient presented with complete resolution. {"title":"Fructose Metabolism Defects in Indian Children - Uncommon or Under-reported? - a Case Series","authors":"Nida Mirza, Ankita Maheshwari","doi":"10.5457/p2005-114.348","DOIUrl":"https://doi.org/10.5457/p2005-114.348","url":null,"abstract":"Objective − The objective of this case series is to report the varied manifestations of fructose metabolic defects across various age groups in Indian children.Case Series − We report 3 cases of fructose intolerance (1 male, 2 female) presenting at 17 months, 3.3 years and 10 months, with hypoglycaemia, recurrent metabolic acidosis, and abdominal distension with hepatomegaly, respectively. Weight was more affected than height in all of them: 1 child had hypoglycaemia and 2 of them had metabolic acidosis. Genetic tests confirmed the diagnosis with 2 patients having mutations in the FBP-1 gene and 1 mutation in the ALDO-B gene. Ključevšek","doi":"10.5457/p2005-114.342","DOIUrl":"https://doi.org/10.5457/p2005-114.342","url":null,"abstract":"Objective − We present the imaging characteristics of an unusual case of Fitz-Hugh-Curtis syndrome (FHCS), a late complication of pelvic inflammatory disease in an adolescent girl with left upper abdominal pain due to perisplenitis seen on contrast-enhanced ultrasound and magnetic resonance imaging.Case Report − A 16-year-old girl presented with acute abdominal pain in the left upper quadrant, vomiting, diarrhoea, and vaginal odour with yellow discharge. She was recently diagnosed with infectious mononucleosis. Abdominal ultrasound showed perisplenic fluid collection that did not enhance post-contrast application, but it did show a mild enhancement of perisplenic tissue. MRI findings indicated perisplenitis. Bašković","doi":"10.5457/p2005-114.345","DOIUrl":"https://doi.org/10.5457/p2005-114.345","url":null,"abstract":"Objective − To determine how much children with “major” congenital abnormalities during the first year of life, compared to those without them, burden the health care system more in terms of length of stay and number of admissions and discharges.Methods − The electronic records of 57 patients with major congenital abnormalities (study group) and 57 patients who were surgically treated for another condition in the first year of life (control group) were reviewed retrospectively.Results − Children with congenital abnormalities were first hospitalized at the age of 1 (IQR=5) day, compared to 121 (IQR=278) days in children without congenital abnormalities (P<0.001). The total number of hospitalizations in the study group was 2.39±1.57, while in the control group, it was 1.14±0.35 (P<0.001). The length of stay in the hospital in children with congenital abnormalities was 48 (IQR=74) days, compared to the control group, which was 3 (IQR=15) days (P<0.001). The average length of stay in children with congenital abnormalities was 22.5 (IQR=29) days, while in the control group, it was 3 (IQR=11) days (P<0.001). The average share of the length of stay in the intensive care unit in relation to the total length of stay in the hospital for the group with congenital abnormalities was 46.6%, while for the control group, it was 23.3% (P<0.001). Less effective methods of contraception were used by 73.2% of the study participants. None of the study participants could correctly identify all named STIs. 15 (14.9%) of the study participants were diagnosed with chlamydia infection.Conclusions − The study found that many sexually active women under 18 years of age in Latvia use less effective contraception methods and have limited knowledge regarding STIs.
