CVD morbidity and mortality rates are rising steadily worldwide, by about 2% each year.\u0000The main task of primary care physicians is to identify patients at risk of cardiovascular disease and early prevention of such diseases by eliminating the factors of their development, especially in young people in the preclinical stage.\u0000Traditional risk factors for CVD are hypertension, smoking, hypodynamics, obesity, dyslipidemia and more. With the development and improvement of clinical observations, the number of risk factors that can cause cardiovascular disease increase. At the end of the 20th century, the scientific community increasingly began to focus on individual nosological units, which are independent predictors of CVD and associated with them pathogenetically. These include nonalcoholic fatty liver disease (NAFLD), hereditary hyper- and dyslipidemias, such as hetero- and homozygous hypercholesterolemia, familial combined hyperlipidemia, and others. With a latent course, these pathologies are not diagnosed or treated for a long time, which leads to a reduction in the duration of cardiovascular catastrophe in patients with these diseases by 20 years compared to the general population. Even when detected, these diseases are often underestimated as risk factors for future CVD by both physician and patient.\u0000That why for the effective prevention of cardiovascular diseases and their complications, the primary care physician must fully have information about the presence of possible risk factors, be able to detect, diagnose and prevent them in a timely manner. The family physician should also inform the patient of the need for lifestyle modifications to effectively address the risks involved and prevent CVD in the future.
Family medicine. Study the state and morphostructure of adipose tissue. To analyze the relationship between lipid metabolism disorders and changes in reproductive health of female combatants.\u0000Materials and methods. The research was conducted at the Department of Obstetrics, Gynecology and Reproductology of the Ukrainian State Institute of Reproductology of the Shupyk National Healthcare University. 118 women participated in the study. Group I consisted of examination data of women who suffered concussion during combat operations with PTSD, group II – control, it consisted of healthy women. Average age of women who participated in hostilities and suffered contusion 27.08±4.23 years.\u0000Results. The influence of stress factors associated with military service in women servicemen on the development of lipid metabolism disorders is obvious. In the main group (118 women), blood cholesterol levels were significantly higher than in the control group. In parallel, a very low level of anti-atherogenic lipoprotein fractions was detected. Lipid metabolism in women who experienced concussion during combat with PTS was characterized by a high frequency of hyperlipidemia of types IIa and IIb, higher serum concentrations of atherogenic lipids fractions (CL, CL-LDL) and low anti-atherogenic fractions (CL-HDL), which were observed in all age subgroups and progressed with age.\u0000Conclusions. This genetic «defect» ensures the uncontrolled proliferation of myeloid cells.\u0000The objective: to analyse the content of medium-mass molecules (MMM) in peripheral venous blood plasma of patients with CML at different stages of the disease and theoretical justification of approaches to the correction of detected changes.\u0000Materials and methods. The study enrolled 77 patients with CML, who were divided into three groups according to the stage of disease. The control group included 20 people who were primary donors of the Kyiv City Blood Center. The main parameters characterizing the state of protein and carbohydrate metabolism were analyzed.\u0000All patients groups were similar in terms of age, sex, anthropometric parameters, and duration of the disease. All patients during hospitalization were examined by clinical, laboratory, instrumental and special research methods, and if necessary, consulted by specialists.\u0000Statistical processing of the results was done by methods of variational statistics, using the Microsoft Excel XP computer program.\u0000Results. Today, there are known factors that determine the development of CML, namely: chromosomal abnormalities, ionizing radiation, hematopoietic system diseases, toxic effects of drugs and polluted environment, etc.\u0000Changes in the content of MMM in plasma of patients with CML compared to the control group were determined. As the disease progresses, the imbalance of MMM metabolism deepens.\u0000The severity of plasma MMM content violations of CML patients is accompanied by endogenous body intoxication, since MMM are universal markers of endogenous metabolic intoxication.\u0000Conclusions. Changes in the content of medium-mass molecules in the plasma of patients with chronic myeloid leukemia dictate the need for further in-depth research to determine role in the pathophysiology of the disease for further treatment and reduction of the manifestations of endogenous intoxication.\u0000Effective treatment of patients with CML is based on modern diagnostics, timely prescription of drugs and subsequent monitoring of the course of the disease.
Family medicine. These are chronic polyetiological, recurrent diseases with disruption of the neuro-endocrine and humoral mechanisms that regulate the secretory-trophic process in the gastroduodenal zone with the development of ulcers. In this disease, ulcers have different localization and clinical course, significantly reduce the quality of life, affect work capacity and personal life. Lack of timely diagnosis and treatment can have adverse consequences.\u0000The objective: to analyse the characteristics of clinical and metabolic disorders in iron deficiency anemia (IDA) and its combination with EULGZ in order to develop the methods for assessing the disease severity, optimizing diagnostics and treatment tactics.\u0000Materials and methods. The observation group included 26 patients (15 men and 11 women) with iron-deficiency anemia (IDA) on background of EULGZ with localization of ulcerative defects of the mucous membrane in the stomach or duodenal bulb. The diagnosis of IDA was verified on the basis of changes in the basic characteristics of clinical manifestations (signs of anemic hypoxia and sideropenic syndrome), specific changes in peripheral blood and indicators of iron metabolism. Patients with IDA and EULGZ, were examined by endoscopic methods, underwent ultrasound and, if necessary, radiological examination.\u0000The control group included 35 primary donors (16 women and 19 men), who underwent two laboratory tests for hepatitis B and C, as well as HIV infection/AIDS. All examined donors underwent a comprehensive examination of peripheral blood and determined biochemical indicators.\u0000The indicators of iron metabolism were determined in all participants: iron content in the blood serum (IS), total iron-binding capacity of the blood serum (TIBS). The index of unsaturated iron-binding capacity of blood serum was calculated as the difference between TIBS and IS. The coefficient of transferrin saturation with iron was determined as the ratio of the content of IS to TIBS. The content of transferrin was determined according to the TIBS indicator, ferritin – by the radiometric method.\u0000Results. Peripheral blood indicators in primary donors and morphometric changes in peripheral blood in patients with IDA were analyzed. The results of investigations of the indicators of iron exchange in blood serum are presented: iron concentration, total iron-binding capacity of serum and the degree of saturation of serum with iron. It was found that male donors have significantly higher levels of iron and ferritin in blood serum compared to female donors.\u0000The dynamic of changes in peripheral blood parameters in patients with IDA caused by long-term blood loss due to EULGZ during the treatment process is shown. The causes of hearing loss are very diverse. Hearing loss can be caused by hereditary and non-hereditary genetic factors, certain complications during pregnancy and delivery, infectious and chronic diseases, head or ear trauma, etc.\u0000A common problem today is hearing impairment. Development of diagnostic algorithm for hearing disorders in primary care will improve the quality of diagnosis, optimize the patient’s management and ensure the earliest possible treatment and prevention measures, which will positively effect the results of treatment and patients quality of life. Primary care physicians use the ICPC-2 classification in their practice. They collect complaints, anamnesis, and test hearing ability. There are various methods to examine auditory function, most of which are based on the patient’s subjective data about perception of the heard sounds. These include perception of whispered and spoken speech, tuning fork experiments (accumetry), audiometry.\u0000Hearing is considered normal if a whispered speech is perceived at a distance of 6 m. The whispered speech degree of perception is the greatest distance from which the patient correctly repeats the words spoken by the doctor fro three times.\u0000Primary care physicians conduct a tuning fork examination of the patient using the Weber test (W), Rinne test (R), Schwabach test (Sch).\u0000Timely hearing examination with the help of modern diagnostic methods in primary care and the correct assessment of the potential capabilities of the auditory system of patients allows correct evaluation of its condition.
