Patients and Methods: 1,760 pupils of elementary, secondary, and high school of eight schools of Almaty (1,302 pupils of gymnasiums and 458 pupils of general education schools and sport boarding school) were examined. The 1st step was a preventive screening. The 2nd step was a distant interactive screening by teachers. The 3rd step was an eye examination of schoolers with low vision. The efficacy of the detection of visual impairments in schoolers during preventive and distant computer screening was compared. In addition, significant indicators for visual impairment monitoring in schoolers were identified. A \"Program for the Prevention of the Development and Progression of Refractive Errors in Schoolchildren\" was developed. To evaluate its efficacy, 140 pupils of general education schools with refractive errors and 1,302 pupils of gymnasiums with refractive errors or their high risk underwent eye examinations. In addition, to compare the effect of wearing glasses/contact lenses with full correction on the quality of life, 31 schoolers aged 12–17 with refractive errors were selected. Results: the rate of visual impairments was 28.4% among the pupils of general education schools and 31.3% among the pupils of gymnasiums. The most common refractive error was myopia (46.9% and 65.4%, respectively). As children moved through their school carrier, the proportion of myopia tended to increase. Accommodative dysfunction ranked second 48.5% and 29.7%, respectively). Time spent on distant screening was twice less compared to time spent on preventive screening. Visual acuity, cycloplegic refraction, reserves of relative accommodation, and axial length (measured by ultrasound) were indicators of visual impairments in children during monitoring. Poor general health was reported in 33% of children who wear glasses and 15% of children who wear contact lenses. The lack of a common standard for preventing phaco infectious complications, a growing number of resistant bacterial strains, and legal aspects of the lack of some antibiotic formulations and routes of their administration with established efficacy in Russia account for the importance of this issue. The authors provide a list of primary causative agents of endophthalmitis and specify their antimicrobial susceptibility, highlight the importance of using antiseptics to prevent endophthalmitis, and describe in detail routes of administration of antibiotics, including conventional ones (subconjunctival injections, topical use) and whose actively promoted (intracameral, transzonular). In conclusion, the authors suggested a rational antimicrobial preventive algorithm for endophthalmitis after phaco based on the analysis of recent Russian and foreign published data and adapted to the Russian health care system. Zolotarev","doi":"10.32364/2311-7729-2020-20-4-216-220","DOIUrl":"https://doi.org/10.32364/2311-7729-2020-20-4-216-220","url":null,"abstract":"The article addresses current treatment modalities for threshold retinopathy of prematurity (ROP) and aggressive posterior retinopathy of prematurity (APROP), in particular, intravitreal administration of vascular endothelial growth factor (VEGF) inhibitors. The results of multicenter clinical trials evaluating pegaptanib, bevacizumab, and ranibizumab are discussed. The clinical effect of intravitreal anti-VEGF drugs was compared to the efficacy of laser retinal photocoagulation. In children with ROP, anti-VEGF drugs are equal or even superior to the conventional laser retinal photocoagulation in terms of clinical efficacy. No negative effects of anti-VEGF drugs on ocular tissues or vasculature development in premature children were demonstrated. Moreover, several studies show the potential efficacy of intravitreal anti-VEGF drugs plus avascular zone photocoagulation for APROP. Ocular surface temperature (OST) was measured in all participants using a non-contact thermal camera (FLIRTM) integrated onto a smartphone (CAT S 60). The difference in OST between the affected eye and the healthy (fellow) eye on thermograms (Δt) was measured at each visit in both groups. Maximum (ΔtMax) and minimum (ΔtMin) Δt for each eye as well as the amplitude of fluctuations (ampΔt) referred to as the difference between ΔtMax and ΔtMin were calculated for each eye. AmpΔt was compared between the groups.Results: in a total of 6 PS patients, B-scan ultrasonography revealed the thickening of the posterior sclera. Choroidal folds (n=4), internal limiting membrane fold (n=2), subretinal fluid (n=3), and optic disc edema (n=2) were identified by eye fundus exam. At presentation, Δt was maximum in all PS patients and reduced unequally after starting treatment. Mean ampΔt was 1.21 °C (1.47–1.04 °C) in PS group and 0.44 °C (0.13–0.63 °C) in the control group. 3 out 6 PS patients were diagnosed with recurrences as demonstrated by OST spikes compared to previous measurements.Conclusion: this article describes a novel technique of the measurement and monitoring of OST. It was demonstrated that PS is associated with the increase in OST measured by thermography. Current early diagnostic and treatment strategies focus on the possibility to implement eye-preserving therapies. The 5-year overall survival for children with retinoblastoma is now 100%. First-line eye removal is recommended in extensive intraocular seeding and poor visual prognosis. The volume of adjuvant therapy is based on the histological risk factors for disease progression. This paper discusses the algorithms to determine risk factors and further systemic treatment strategy in several large study groups. Eye and even vision preservation is now possible in at least 65–75% of retinoblastoma patients. The paper also describes in detail the methods of locally administering chemotherapy which are considered eye-preserving treatment options, i.e., selective intra-arterial chemotherapy and intravitreal chemotherapy. Various physical methods of eye-preserving treatment which underwent significant development over the past decade are highlighted.
