Hence targeted management using effective interactive communication system becomes imperative to combat the phenomenal increase of glucose intolerance in gestation. Methods: The study was conducted at Dr. Seshiah Diabetes Specialties Centre, Chennai and gestational follow-up until delivery was carried out for 900 pregnant women. A diabetes protocol was developed to maintain a complete record of their clinical assessment every month. The clinical management of GDM was done in 3 stages namely during gestation, immediate postpartum and after delivery through effective communication techniques like lecture method, computer-aided counselling, power point presentations, brochures, individual and family counselling, postpartum follow-up through postpartum follow-up module and counselling prospective mothers through audio-visual aids. The impact of dietary intervention given to the pregnant women under follow-up was studied in two folds: Knowledge, Attitude and Practice (KAP) study and estimation of their glycemic level before and after intervention. The glycemic levels before and after intervention was tested for significance level using t test and effect of risk factors on gestational glycemia was tested using multiple regression analysis. Results:  The targeted management through interactive counselling and education communication proved effective as maternal glucose was well controlled during gestation and encouraged postpartum screening. Nutrition intervention proved effective in bringing down their energy intake and improved their glycemic level. The post counselling scores in KAP showed significant improvement. Conclusion: GDM may well be incurred in future as an open window of opportunity for prevention, rather than a closed door of bleak outcomes. The present study was planned with an objective to study the complications, safety and efficacy of PICCs in haemopoietic stem cell transplantation for haematological diseases. Methods: A prospective cohort study was conducted in stem cell transplantation department of our tertiary care cancer hospital for a period of two and half years as per the proforma. All patients with hematological cancer who were undergoing stem cell transplantation were enrolled and were followed up until catheter removal or patient death. The basic information was recorded at the time of PICC insertion, weekly care, and removal after the transplantation. The data were analyzed to study the aims and objectives of the study. Results: Seventy two PICCs were inserted over a period of two and half years for a total of 8048 catheter-days (mean of 111.77 +/_ 66.55 days i.e 3.7 months, range: 9 to 269 days). Out of these 72 PICCs, 11 (15.27%) PICCs had complications and all of them were removed at the rate of 1.35/1000 PICC-days. Catheter related blood stream infection (CRBSI) was higher in allogenic transplant group (16%) than in autologous group (2.5%) while thrombosis was present in allogenic group only. Mortality due to non-PICC complications was higher in allogenic transplant (40%) than in the autologous group (7.5%). Methods: A descriptive study was conducted on pregnant women who have applied to the Maternity Hospital Non-stress Test unit in the city of Erzurum from 1 August 2014 to 31 December 2014. A total of 301 pregnant women who met the inclusion criteria participated in the study. Data were collected using the Tilburg Pregnancy Distress Scale (TPDS) and Humor Styles Questionnaire (HSQ). For statistical analysis, percentages, means, and Pearson, r correlation were used. Results: There was a negative (r= -0.149, p=0.01) relationship between a self-enhancing humor style and distress level. A negative significant correlation was found between the partner involvement sub-dimension and participatory (r= -0.270, p=0.000), self-enhancing (r= -0.289, p=0.000) and self-destructive humor styles (r= -0.156, p=0.007). Furthermore, there was a significant positive correlation between an aggressive humor style and the negative emotion sub-dimension (r=0.136, p=0.018). Conclusion: As a result of the study, it was determined that the use of positive humor styles by pregnant women was higher and their distress levels were low. Identification of these risk factors is necessary in terms of creating a management plan of the cases, and for planning protective and preventive activities. Objective: To discuss the potential of forensic evaluation in improving the health of the children, and practical difficulties and to evaluate the risk factors and protection needs of children involved in crime. Method: Data of the children between the age group of 12-18 years, who attended the Forensic Medicine Policlinic during the time period of 08th October 2015 – 31st January 2016, were taken for evaluation, retrospectively. Age, gender, socio-demographic data and reasons for evaluation were investigated. Results: Out of the 89 cases included in the study, 12 were girls (13%), 77 were boys (87%), and the mean age was 15.14. Custody examination was the most common reason of the application with 76 cases (85%). In the studied group, 60 children had the habit of smoking, and 24 were using alcohol/substance, while 36 of them were working. The mean period of education was 7.18 years; in this study, 3 children were living on the streets, 4 were staying at a children’s dormitory (society for protection of children), 5 were living with their friends. New or recent blunt traumatic lesions were detected on the examination of 20 children. Conclusion: In our retrospective study, risk factors of juvenile delinquency were evaluated regarding gender, educational status, family status, working status, smoking and use of alcohol/substance, and these were found to be compatible to the literature. Whether low testosterone is associated with diverticular disease of the colon (DDC) risk independent of metabolic co-factors is not clear. Aim: This study was conducted this study to reveal the possible link between serum testosterone levels and colonic diverticula. Methods: This study was conducted between May 2018 and December 2018, total 208 subjects were enrolled for the study at a university hospital, Turkey. At total, 85 men with DDC were enrolled for the study. DDC was defined by non-contrast CT scan with colonoscopic examination. Control group (123 subjects without DDC; the mean age was 54.5 ±14.5) were selected from otherwise healthy men. Results: Of the 85 men with DDC, the mean age was 60.8 ± 13.5 years. Lower levels of total testosterone were associated with a 25% increased odds of prevalent DDC on adjusted analyses. Importantly, lower levels of total testosterone remained strongly associated with prevalent DDC, even in men with normal glucose levels (p=0.01). Conclusion: Decreasing levels of total testosterone, even within normal range, are independently associated with DDC in elderly age. Methods: A total of 922 nursing students participated in the study, which was designed as cross-sectional research. The data were collected using the Sociodemographic Characteristics Data Form, the Communication Skills Scale (CSS) and the Stress-Coping Styles Scale (SCSS) in 2017. Higher scores from the CSS and the SCSS indicated higher levels of communication skills and stress-coping methods. The data were analysed using SPSS for Windows (version 18.0). Results: The total score from the CSS was 124.09 ± 14.94 (min. 45, max. 225). The average of the total CSS scores of the nursing students was found to be low (124.09 ± 14.94). Out of the sub-scales of the SCSS, the highest score was for the Self-Confident sub-scale (19.87 ± 3.18) and the lowest score average was for the Seeking Social Support sub-scale (7.81 ± 2.25). The relationship between inflammation and OSA has been described and inflammation was found to have a role in the development of the disease. Hematologic parameters have been found as inflammatory biomarkers in various diseases. Aim: To evaluated the association of OSA and red blood cell distribution width (RDW) value, and the association of RDW value in OSA patients with hypertension (HT) and without HT. Method: A retrospective and cross-sectional evaluation of the complete records of 412 patients, who were presented to our sleep center and underwent polysomnography. Polysomnographic parameters and RDW-CV values for these patients were evaluated. Result: Total 372 patients with obstructive sleep apnea syndrome and 40 people as a control group were included in the study. There was no significant difference between the OSA and control group regarding the mean RDW value (p>0.05). No statistically significant association was detected between RDW and apnea hypopnea index (AHI) in the evaluation of the OSA group. There was a negatively significant correlation between RDW and minimum oxygen saturation and mean oxygen saturation values in the OSA patients (p=0.002; r=-0.159,p=0.004, r=-0.148;respectively). A statistically significant difference was found in the mean RDW value based on the presence of HT in all OSA groups (p=0.031). The mean RDW of the group with HT was higher than group without HT. Thrombocytopenia (low platelets) is one of the very common manifestations in case of dengue infection. Platelet transfusion in dengue always remains controversial. Objective: Present study was conducted to study precise criteria and transfusion trigger for platelet transfusion in our set-up. Materials and Methods: This study was conducted on 112 serologically antibody positive confirmed dengue patients admitted in a Hospital, Powai, Mumbai, between 1st of August to 30th October 2017. The age groups of the included patients were within 20-50 years and did not have any co morbidity and were diagnosed at OPD. Indication for hospitalization includes high grade fever (>101° F) and platelet counts less than 100,000 along with other viral prodromal symptoms. Clinical data, reports of laboratory investigations, platelets requirements and data obtained from daily follow-up and record sheets were analyzed. Results: In the serologically positive confirmed cases, the prevalence of thrombocytopenia (count <100,000/mm3) was 97% on admission and there was no bleeding recorded in any patient. Out of 112 serologically confirmed dengue cases, only 1 (<1%) patient of dengue received platelet transfusion whose platelet counts were <7000/mm3 and he had petechiae all over the body. Total 9 patients had platelet count <10000/mm3 (8.03%), 28 (25%) patients had platelet count <20,000/mm3, 52 (46%) had a platelet counts <40,000/mm3 while total 79 (70.53%) patients had platelet counts <50,000/mm3. In the study population, 28 (25%) patients had platelet count <20,000/mm3 and 9 (8.03%) patients had platelets count <10000/mm3, only 1 patient had petechiae; hence platelet transfusion was given. For all other patients who had platelet counts <20,000/mm3, their clinical parameters were monitored strictly and frequent manual platelet counts were done. These patients were not given any transfusion. Conclusion: This study suggests that bleeding due to thrombocytopenia in case of dengue fever is not a common occurrence especially, in otherwise healthy adult or middle aged patients. Those with no or minimum co morbidities and strict monitoring of symptoms are required to preserve the resources and to avoid unwanted platelet transfusions. Platelet count should not be a bench mark for someone to transfuse the platelet. Uzel","doi":"10.32677/EJMS.2019.V04.I01.010","DOIUrl":"https://doi.org/10.32677/EJMS.2019.V04.I01.010","url":null,"abstract":"The most common adverse event associated with oral anticoagulation (OAC) is bleeding in general that is seen in 5 to 48% of patients and gastrointestinal bleeding (GIB) in particular which occurs in 2–4% of patients. Intramural hematoma (IMH) is a rare variant of GIB, occurring in 1 in every 2500 patients and is treated with OACs. IMHs are rarely fatal and the current data suggest conservative medical therapy and clinical follow-up for management of IMHs. GIB and intestinal IMH is becoming a more common clinical problem because of increasing number of patients taking OACs. This report describes a 91-year-old woman who was presented with abdominal discomfort for 2 days. The patient had been under anticoagulant therapy with warfarin because of atrial fibrillation. She was found to have an inappropriately prolonged international normalized ratio level of 8.1. IMH was confirmed with abdominal ultrasound and computed tomography (CT) scan. The patient was treated conservatively with fluid resuscitation, nasogastric decompression, bowel rest and reversal of anticoagulation. Follow-up CT scans, approximately one month after medical treatment, demonstrated complete resolution of the IMH.
