Nirmal, Priyanka Gupta, Nadeem Ahmad, Smita Nath, Narendra Pal Singh, Shukla Das","doi":"10.32677/ejms.v9i1.4530","DOIUrl":"https://doi.org/10.32677/ejms.v9i1.4530","url":null,"abstract":"Objectives: The objective of this study is to determine the bacteriological profile and their antimicrobial susceptibility pattern among clinically suspected adult septicemia admitted patients and to study the correlation between the clinical presentation and inflammatory biomarkers (C-reactive protein [CRP] and Procalcitonin [PCT]) in the final outcomes of nosocomial sepsis in adult septicemia patients. Material and Methods: This is a descriptive cross-sectional study of 1-year duration from November 2022 to November 2023. The study has been approved from the Institutional Ethical Committee. Adult patients with a clinical suspected diagnosis of sepsis were admitted to the medicine ward of the tertiary hospital. Blood samples of adult sepsis suspected patients samples were taken for as a part of septicemia screening. Blood culture and sensitivity were performed according to standard guidelines. The sepsis inflammatory prognostic markers CRP and PCT test were evaluated according to manufactures instruction. Results: Out of 300 clinically suspected adult septicemia patients, 93 (31%) patients were isolated culture-positive sepsis while 207 (69%) patients were culture-negative sepsis. Gram-positive organisms were more (69%) as compared to Gram-negative organisms (31%). Staphylococcus aureus was predominant among Gram-positive organisms while Acinetobacter baumanii complex (9%) followed by Enterobacter spp. (8%) isolated in Gram-negative bacilli. Culture-positive sepsis patients have CRP level ≥3.2 mg/dL (80%) and the majority (80%) of these patients have reactive PCT. Conclusions: Successful treatment of bloodstream infection depends on early diagnosis and appropriate use of antimicrobial agents. Materials and Methods: The study involved a total of 35 HD patients identified through clinical evaluations. CAG repeats expansion analysis was conducted using polymerase chain reaction (PCR) to confirm positive cases. Results: Employing CAG flacking PCR in 35 cases, 19 tested positive, predominantly displaying CAG repeats in the range of 40–50. Noteworthy cases include a 5-month-old patient with 44 CAG repeats and an 80-year-old patient with 46 CAG repeats. It might involve any organ in the body with a special predilection for lymph nodes, salivary glands, the pancreas, and the retroperitoneum. Due to the non-specific features of the disease and the capability of mimicking many other medical conditions, the diagnosis of IgG4-RD could be delayed. Herin, we describe a case of a middle-aged man who presented with obstructive jaundice due to a pancreatic mass. Further evaluation revealed multiple cervical and inguinal lymphadenopathy, bilateral wedge-shaped lesions in the kidneys and substantial thickening of the aortic wall that was suggestive of aortitis/periaortitis. A histopathological examination of an inguinal lymph node showed increased infiltration of nodal immunoglobulin G4 (IgG4) plasma cells (>30/high-power field) with a high serum IgG4 immunoglobulin level. Based on that, the patient was diagnosed with IgG4-RD, and he was successfully treated with pulse steroid therapy with complete resolution of his symptoms and the radiological and laboratory findings. IgG4-RD is a protean condition with a large spectrum of clinical manifestations that could mimic many other serious medical conditions. Symptomatic GCT of the neurohypophysis is exceedingly rare, being <70 cases described so far. GCTs predominantly exhibit benign behavior, while the malignancy rate remains at 2%. Imaging is quite unspecific and diagnosis is difficult to establish preoperatively. Histopathology and immunohistochemistry serve as the definitive diagnostic approach for GCTs and help in distinguishing GCT from other pituitary tumors, including pituitary adenoma, pituicytoma, and spindle cell oncocytoma. These tumors are resistant to radiotherapy and chemotherapy, necessitating surgical resection as the primary treatment modality. Due to the potential absence of distinct tumor masses and local tissue infiltration by tumor cells, complete excision is crucial, with resection extent extending beyond areas of infiltration. Here, we present a rare case of GCT originating from the posterior pituitary in the suprasellar region. On immunohistochemistry, tumor cells expressed diffusely thyroid transcription factor-1, S-100 protein, and vimentin confirming the diagnosis of supra-sellar GCT. Hypnotherapy is often described as a tool that helps individuals shift their self-image from that of a smoker to a non-smoker. The purpose of this systematic review was to examine the status of existing research on the impact of hypnotherapy on smoking cessation in terms of type of hypnotherapy, type of application, relapse rate, duration of intervention, extent, and safety of smoking cessation, along the effectiveness in comparison with other pharmacological and non-pharmacological interventions. For this study, the authors used databases such as APA, Scopus, PubMed, JSTOR, and Frontiers for studies that used hypnotic interventions for smoking cessation. The Preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines were adhered to for screening of the research papers and finalizing of data, from which the PRISMA flowchart was constructed. The authors extracted data on the nature and characteristics of the studies, the types of interventions used [hypnotherapy as a sole intervention or paired with other interventions], characteristics of the sample population [sample size, control groups, and demographic details of the sample], duration of the interventions, and the method of assessment of smoking abstinence. The results show that there is inconclusive effect of hypnotherapy on smoking cessation in the target population. However, it is also an important vehicle of entry of infections. Bacterial, viral and parasitic agents, causing these infections can all spread by food. Bacteria can produce many toxins and also be invasive sometimes, which can lead to diarrhoea and dysentery, respectively. This contamination takes place by risks like cooking food at improper temperature and keeping food open after cooking. These risks lead to various hazards. Also, microbes can help prepare different foods like fermented foods and kombucha tea. Modern society relies heavily on processed and ready-to eat foods, both of which can cause foodborne infections. Keeping all these things in mind, the science of food microbiology becomes very important in modern times. Aim: All these aspects of food microbiology and food safety have been discussed in this chapter. Objectives: Many points like food safety, cleanliness and other aspects like chilling of cooked food minimize risks of microbial food contamination and resultant hazards, have been elaborated upon. Methods: Scientific literature search was carried out to study the risk factors and related reports with respect to food microbiology, by food scientists and others. Results: Food contamination can be of microbial origin and a multitude of factors may lead to microbial contamination of food. These factors could be improper cooking, leaving cooked food uncovered, and other things. Conclusion: Food microbiology is a very important aspect of public health and quite neglected too. These cysts often have a predilection toward the male gender with the third and fourth decades of age group. They generally arise from the follicular infundibulum, primarily due to plugging of the follicle. Often a part of genetic syndromes, they can also be post-traumatic where the ectodermal cells get stuck or reimplanted. Epidermoid cysts are often a slow-growing and painless lesion with few exceptions. The proper neurological examination, clinical examination, and investigations such as magnetic resonance imaging help in confirming the diagnosis and proceeding with the management. Clinically, epidermoid cysts may or may not present with the neurological impairment. The preferred intervention is total or subtotal resection with minimal chances of complications such as recurrence, swelling, and infection. However, these can very well be avoided by regular follow-up assessments. The clinical presentation in the lower back with massive dimensions is relatively uncommon, and therefore, authors have presented here an unusual case from the outpatient department of neurosurgery, where the patient presented with a large, soft, exophytic lesion at the level of (S4/S5), which gave rise to the suspicion of meningocele and meningomyelocele, but after complete local assessment and radiological investigations, it was diagnosed finally as an epidermoid cyst. In Miller’s Classes I and II mandibular gingival recession, the goal of this study was to evaluate the predictability of the free gingival graft (FGG), as a single-step operation in terms of root coverage and esthetics. Materials and Methods: 10 patients between the ages of 25 and 50 who were sent to the Department of Periodontics (3 men and 7 women) made up the study group. There were a total of 14 mandibular recession sites that were chosen. Recession depth (RD), recession width, probing depth (PD), clinical attachment level (CAL), width of attached gingiva (WAG), and visual analog scale (VAS) were assessed at baseline, 3 months, and 6 months post-operatively. Results: The mean RD was 3.13±2.0 mm at the baseline and decreased to 0.80±0.78 mm after 6 months. At baseline, the VAS was 3.52±0.81, and at 6 months after surgery, it had improved to a mean of 6±2.14. From a baseline WAG of 0.35±0.38 to 2.83±0.68 mm, there wasa significant increase with p<0.0001. Six months after surgery, there was an increase in CAL, from 5.63±1.12 to 1.28±0.76 mm. In addition, PD improved from 1.98±0.54 to 0.97±0.36 mm. Conclusion: FGG showed statistically significant gain in recession coverage and had good esthetic score.
