Basisdiagnostik ist nach wie vor die fraktionierte motorische, aber auch sensible Elektroneurografie (ENG) des N. ulnaris. Demgegenüber spielt das EMG eine eher untergeordnete Rolle. In den letzten Jahren gewinnt die Nervensonografie zunehmend an Bedeutung, nicht nur zur Bestätigung der klinischen Diagnose insbesondere bei negativer ENG, sondern auch zur ätiologischen Abklärung der Störung. Es ist zu erwarten, dass sie wahrscheinlich ebenso wichtig wie die ENG werden wird, insbesondere auch, da die Kombination beider Methoden die Sensitivität in der Diagnostik erhöht.</p></div><div><p>Diagnosis in ulnar neuropathy at the elbow (UNE) is much more demanding than in carpal tunnel syndrome (CTS). Basic diagnostic procedure consists of motor as well as sensory electroneurography (ENG) in different sections of the ulnar nerve. In contrast electromyography plays a minor role in diagnosis of UNE. In recent years nerve ultrasound became increasingly important, not only in confirming clinical diagnosis in case of negative ENG, but also for etiological clarification. Die klinische Relevanz ist dabei nicht auf den Nachweis gesteigerter hirnelektrischer Erregbarkeit beschränkt. In der neurobiologischen Forschung kommen neurophysiologische Verfahren breit zum Einsatz, Neurofeedback-Techniken werden zukünftig therapeutische Relevanz erlangen.</p></div><div><p>Electroencephalography (EEG) is a tool for the investigation of brain function and remains important in the workup of psychiatric disorders. For brain research a wide spectrum of neurophysiological methods are available. Daraus resultiert eine Veränderung des Berufsbildes und dem erhöhten Bedarf an qualifiziertem Personal. Das alleinige Anlernen in diesem spezifischen Arbeitsfeld wird den Qualitätsanforderungen an die Untersuchungen nicht gerecht und deckt schon jetzt nicht den vorhandenen Personalmangel. Neben einer grundständigen Qualifizierung bedarf es lebenslangen Lernens in Form von professionellen Fort- und Weiterbildungen. Die Durchführung der Untersuchungen nach einheitlichen Qualitätsstandards (SOPs) ist erforderlich und dient der Sicherung der Patientenversorgung.</p></div><div><p>The field of neurophysiological diagnostics has expanded in recent years many times over with new examination methods. This results in a change of qualified personnel. The sole training in this to the investigations and not fair and professional image and the increased need for specific field of work is the quality requirements already does not cover the existing staff shortage. In addition to basic professional training and further education, lifelong learning is required in the form of continuing education. Kommende Entwicklungen, auch durch Einsatz moderner Methoden der Signalanalyse, könnten dazu führen, demyelinisierende Neuropathien besser zu erkennen und zu beschreiben.</p></div><div><p>Both motor and sensory Neurography will remain and serve their purpose, namely to localize dysfunction, to delineate demyelination, to determine the time course of peripheral nerve lesions, and to evaluate the neuromuscular transmission.
