Hong","doi":"10.51407/mjpch.v29i2.250","DOIUrl":"https://doi.org/10.51407/mjpch.v29i2.250","url":null,"abstract":"Introduction: Psychological distress was experienced by millions of people affected by the coronavirus (COVID-19) pandemic, particularly adolescents with diabetes. Method: A cross-sectional study was conducted among adolescents with type 1 (T1DM) and type 2 diabetes mellitus (T2DM) aged 10-18 years during the nationwide lockdown from June-December 2020 in Malaysia to assess the impact of COVID-19 towards their mental health. Eighty-seven participants were recruited and interviewed regarding their lifestyle changes, followed by the administration of the Depression, Anxiety and Stress Scale (DASS-21). Results: Fifty-eight patients with T1DM (30 males, 51.7%) and 29 patients with T2DM (9 males, 31%) participated in the study. The incidence of depressive, anxiety and stress symptoms were detected in 34%, 41% and 26% respectively of diabetic adolescents with no significant difference between T1DM and T2DM patients. Male gender, T2DM, puberty and the presence of anxiety symptoms were associated with deterioration in glycaemic control post-lockdown. There was a deterioration in HbA1c among male and T2DM patients by 0.76% and 0.94% respectively (p-value 0.013, 0.004 respectively). An increment of HbA1c pre- and post-lockdown was observed in patients with anxiety symptoms (9.39 ± 0.49 vs 10.16 ± 0.54%, p-value 0.028). Patients who displayed stress symptoms showed improvement in their HbA1c (10.00 ± 0.57% vs 9.50 ± 0.063%, p-value 0.036). Conclusion: There is a high prevalence of psychological disturbance among diabetic adolescents during the COVID-19 pandemic, particularly the anxiety component related to poor glycaemic control. Case report: We described a 14-year-old girl who presented with thyrotoxicosis and a large goitre associated with compressive symptoms. Thyroid scintigraphy demonstrated a solitary autonomous hyperfunctioning thyroid nodule. FNAC examination was not suggestive of malignancy. Thyrotoxicosis was controlled with carbimazole and a hemithyroidectomy was done. Histopathological examination revealed minimally invasive follicular thyroid carcinoma, following then, complete thyroidectomy was done. Discussion: This patient’s risk factors of malignancy were her age (young), gender (female), large nodule size, progressive growth, and compressive symptoms. This case illustrates the limitation of FNAC to ascertain malignancy in patients with large AFTN as the initial FNAC report was consistent with a benign lesion. Surgical resection without prior FNAC should be considered for cases with hyperfunctioning thyroid nodule which then would allow timely histologic analysis and accurate diagnosis. Conclusion: Autonomous functioning nodules in children and adolescents warrant careful evaluation to exclude malignancy. Neonatal respiratory distress is the most common clinical presentation of diaphragmatic eventration. Direct abdominal organ compression on the lung or lung hypoplasia is the cause of this symptom. Preoperative radiological imaging is important to diagnose diaphragmatic eventration. Chest radiographs, ultrasound, CT scans and MRI can be used to evaluate the anatomy of the diaphragm. In complicated condition, video-assisted thoracoscopy (VATS) play a role as a diagnostic and therapeutic tool to diagnose and manage diaphragmatic eventration. Case presentation: We presented a case of a full-term unfortunate baby delivered via emergency lower segment caesarean section due to bleeding of the placenta previa type III. Immediately post-delivery, the patient had respiratory distress with an episode of desaturation. CT scan showed a focal right hemi-diaphragmatic bulge with liver tissue within. Video-assisted thoracoscopy (VATS) was performed and the diagnosis of right diaphragmatic eventration had been established. Diaphragmatic plication was done via a VATS-assisted thoracotomy approach. Conclusion: Video-assisted thoracoscopy (VATS) plays a role as a diagnostic tool and even as a therapeutic tool to diagnose and manage diaphragmatic eventration. In this case report, we describe two siblings who presented with multiple fractures since childhood. A low 1,25-dihydroxyvitamin D (1,25(OH)2D) despite a sufficient 25-hydroxyvitamin D (25OHD) level was suggestive of 1-alpha-hydroxylase deficiency. Genetic testing confirmed a mutation in the CYP27B1 gene on chromosome 12q13.3, which produces 1-alpha-hydroxylase. The inability to convert 25OHD to 1,25 (OH)2D leads to hypocalcemia and severe rickets. Treatment requires a pharmacologic dose of activated hydroxylated vitamin D and adequate calcium intake. Rare forms of hereditary rickets should be suspected in severe rickets. Early recognition of VDDR type 1A is imperative to ensure adequate healing of rickets during childhood. This study aims to determine the failure rate and to identify factors associated with nonresponders to acetaminophen to close hsPDA. Methods: This retrospective, observational cohort study involved all inborn delivered at ≤ 32 weeks and birth weight of < 1.5 kg who had hsPDA. A multivariable logistic regression model was used to identify factors associated with nonresponders to paracetamol. Results: There were 137 hsPDA patients with a mean birth weight, gestational age, and age of diagnosis of 1027.9 ± 209.2 g, 28.1 ± 2.1 weeks, and 12.0 ± 7.2 days. The mean dimension of hsPDA, left atrium to the aorta, and PDA to left pulmonary artery ratio was 3.03 ± 0.64 mm, 1.94 ± 0.41, and 1.02 ± 0.17, respectively. The nonresponders to paracetamol was 18.2% (95% confidence interval [CI]: 12.4-25.9) and was significantly associated with bronchopulmonary dysplasia (BPD) (15/25 vs. 16/112, p<0.001) and interventricular hemorrhage (IVH) (13/25 vs. 31/112, p=0.02). Factors associated with nonresponders were a PDA to left pulmonary artery ratio (LPA) > 1.0 and ventilation at diagnosis with adjusted odds ratios of 6.69 and 4.17, respectively. Conclusions: One in five infants with hsPDA did not respond to treatment and were significantly associated with BPD and IVH. Invasive ventilation at diagnosis and a PDA to LPA ratio of > 1 were associated with treatment failure. It is the most sensitive and specific of the cardiac markers. Studies have shown its elevation among Human Immunodeficiency Virus (HIV) infected subjects. Most of these were in adults hence we determined the use of cTnI as a marker of myocardial injury among HIV subjects. Methods: The study was a comparative cross-sectional study among 100 HIV - infected children aged 2 – 14 years and equal age-matched HIV-uninfected controls. Recruitment was through the Paediatric HIV and outpatient clinics of the hospital respectively. Both groups had serum cTnI assay (normal is <0.1μ/L) over four months period. Data analysis was done using the SPSS® v 20. Normally distributed data had mean standard deviation while not normally distributed data had a median with an interquartile range determined. The diagnostic performance of cTnI was determined using the Receiver Operating Characteristics curve. Significant p is < 0.05. Results: A total of 200 (subjects and controls) were studied. The mean age of participants was 9.42 years. There was a significant difference in the median (IQR) cTnI of HIV-infected subjects [73.8 (51.8 – 149.1)] compared with HIV uninfected controls [25.0 (13.3 – 51.4)] p < 0.001. The prevalence of myocardial injury among subjects was 48% compared with 8% among controls, also statistically significant (p < 0.001). Serum cTnI has 91.3% and 88.9% sensitivity and specificity respectively. Conclusions: The prevalence of myocardial injury was high in HIV - infected children. Aim: We developed a brief education program on learning disability (LD) in children and validated its corresponding assessment tool (Knowledge and Attitude on Learning Disability Questionnaire (KALDQ) for pre-school teachers in Malaysia. Methods: Based on an extensive review, the essential educational components of a program for LD in children and relevant items for KALDQ were identified, from which a brief health educational program was designed. The face and content validity of the tools were established via expert evaluation. A subsequent pilot study was held on 57 pre-school teachers (N=57) to evaluate the feasibility of the program and to establish the construct validity and reliability of the newly developed KALDQ. Results: A 10-hour interactive health education program comprises of 4 sessions to orientate pre-school teachers on LD in children and direct them towards medical referral and rehabilitations services, as appropriate, was developed. The face and content validity for both the program and KALDQ were satisfactory. Exploratory factor analysis performed on the original 65-items questionnaire. The final KALDQ consisted of 34 knowledge-domain and 14 attitude-domain items. The reliability of the two domains was satisfactory (Knowledge-domain α=0.83, Attitude-domain α=0.60). Discussion: This study developed a brief education program on LD in children and its corresponding assessment tool. However, the studies included were dated more than three decades ago. Objectives: To determine the safety (short-term complications) of UAC in the newborn at two different positions. Methods: A randomized control trial was performed from March 2012 to August 2013, among the newborns requiring UAC insertion in Neonatal Intensive Care, at Hospital Universiti Sains Malaysia. Patients were randomly assigned to either high or low UAC positions using block randomization. Outcomes: The primary outcomes were the incidence of aortic thrombosis, intraventricular haemorrhage (IVH), necrotising enterocolitis, hypertension, haematuria and feeding intolerance. The secondary outcome was to compare the incidence of short-term outcomes of UAC position to gestational age and birth weight. Results: One hundred and seventy-eight patients were included (89 per study arm). The mean gestational age was 34 weeks with a mean birth weight of 2.31 kg. There were no significant differences between the two groups in terms of baseline data. The overall incidence of complications was 20 (23%) in the low position and 18 (20%) in the high position group UAC (p=0.76). The observed complications between the low and high position UAC group: IVH 6.7% vs. 9%, p=0.58; feeding intolerance 6.7% vs. 4%, p=0.51; hypertension 4% vs. 6%, p=0.51; peripheral ischemic 2% vs. 0%, p=0.25.
