As COVID-19 transitions to an endemic phase, this study explores how consistently its ongoing risk is communicated during the surgical consent process. We prospectively reviewed 147 surgical consent forms collected over 2 months in 2024 at a UK tertiary centre. The primary aim was to assess how often COVID-19 risk was documented; secondary outcomes included postoperative infection rates and 30-day mortality. COVID-19 was explicitly mentioned in only 5.4% of consent forms. No significant variation was found across clinician grades (<i>p</i> = 0.840). Three patients developed postoperative COVID-19, and one died within 30 days. This mortality association was statistically significant (<i>p</i> = 0.020). Despite existing guidance, COVID-19 risk is rarely discussed when obtaining consent, raising ethical and legal concerns. A hand saw as a means of suicide is most unusual and the resulting irregular and serrated wound margins, depth of injury. Associated anatomical damage must be analysed to distinguish a suicide from a homicide or accident. The psychological factors contributing to such an extreme choice of weapon should also be explored. Same-sex couples remain excluded by restrictive surrogacy laws, developed in 2015, which only allow altruistic arrangements for married heterosexual couples. Proposed legal amendments aim to expand access to the healthcare system and surrogacy while hospital-based maternity and neonatal care remain critically unprepared. This paper discusses the gap between legal progress and clinical readiness, arguing that without national standardised surrogacy birth care guidelines, hospitals risk perpetuating inequality and confusion during one of life's most sensitive moments. Drawing on international experience, the discussion highlights the need for inclusive protocols, staff training and ethical guidance. It was superseded by guns and nowadays maybe used for sport and recreation. They can cause fatal injuries though such incidents are fortunately rare. Accordingly, forensic pathologists may face challenges when they are asked to determine the manner of death when a crossbow is involved. We present the case of a young man who was found dead in a fenced field. It was summertime and he was in an advanced state of putrefaction. The examiner observed a roundish cutaneous discontinuity on the forehead, from which a crossbow bolt protruded. At about 4 m from the corpse, a crossbow and further bolts were found. The cause of death was identified as an intracranial injury resulting from a single bolt shot that transfixed the head. Together with the circumstantial evidence, it was concluded that this lethal injury was self-inflicted and that subsequently the victim had been able to walk. However, examination of current practice reveals a uniformity in applying standardised timing assumptions despite well-documented individual variation in alcohol pharmacokinetics. This paper highlights factors that can influence alcohol absorption and cause the absorption of alcohol to extend beyond the conventional 2-hour timeframe since the last drink. Various physiological conditions, including the presence of food, certain medications, medical conditions and other factors, can prolong alcohol absorption beyond 2 hours.
