{"title":"Purse String Umbilicoplasty for Repair of Omphalocele Minor with Review of Literature","authors":"S. The author below describes a simple, easy and safe method for the repair of the umbilicus in cases of omphalocele minor. Methods During the period from January 2015 to May 2017, 16 patients (11 males, 5 females) with omphalocele minor were operated by purse string technique. It is a prospective observational study with a level of evidence type 4. One case presented by intestinal obstruction due to twisted Meckel`s diverticulum. Other case presented by intestinal obstruction due to intestinal malrotation. One case the contents were the cecum and appendix. In two cases the sac contained part of the left lobe of the liver. diagnosed antenatally. Thirteen patients presented the same day of birth and 3 patients presented at the 3rd birthday. The sac was intact in 14 cases and ruptured in two. Two cases had hypospadias. One case had absent radius and ulna on the right hand and polydactyl on the left hand. Size of the fascial defects ranged from 2cm to 5cm (mean 3.5 cm). Good outcomes occurred in 14 patients. Wadhwa","doi":"10.18689/mjcrs-1000113","DOIUrl":"https://doi.org/10.18689/mjcrs-1000113","url":null,"abstract":"Methemoglobinemia is a condition in which an abnormal proportion of the iron in heme moiety of the hemoglobin is oxidized to the ferric state leading to impaired oxygen transport and anemic hypoxia and can cause tissue ischemia and death. A 21 year old male, known case of Hansen’s disease was posted for surgicalneurolysis of ulnar nerve. His surgery was already postponed once as SpO2 waslow on the operation table. He then underwent complete systemic evaluation for the cause of low saturation but was found normal. On pulse oximetry it was seen that SpO2 was 91-93% on room air without any signs and symptoms of hypoxemia. This suggestedmethemoglobinemia due to treatment with dapsone which was being taken by the patient for treatment of Hansen’s disease. The surgery was conducted uneventfully under general anesthesia. {"title":"Comparative Study between Primigravida and Multigravida Regarding Women's Self-Care Practices for Management of Selected Minor Discomforts","authors":"G. During pregnancy, the rapidly rising hormones change the maternal body and may cause some symptoms in the mother called as minor disorders. Aim of this study was to compare between primigravida and multigravida regarding women’s self-care practices for management of selected minor discomfort. Research Design for this study was correlational descriptive. Setting of this study was conducted in the keebly MCH at Menofya governorate in Egypt. Subjects and methods: convenient sample was used to collect data in a period of six months, two days / week (Monday and Wednesday) and started from March 2016, until August 2016, and the total number was 300 pregnant women for three trimesters (90 primigravida and 210 multigravida), two tools for data collection was structured first; interviewing questionnaire sheet, and second; minor discomforts assessment sheet. Results of the study revealed that (33.3%) of the primigavida had moderate education compared to (21.4%) among multigravida. Less than half of the primigravida (42.9%) attempts to manage nausea and vomiting by avoid food smelling compared to (26.3%) among multigravida. Additionally, more than half of both groups (55.3%primi, 54.1% multi) avoid fried, spicy, and fatty food as a one method to manage their heartburn. Relatively two thirds (65.5%) of the primigravida avoid standing for long time for managing back pain compared to (46.1 %) among multigravida. Conclusion: There was no significant difference between primigravida and multigravida women’s self-care practices for management of (constipation, heartburn, backache, and leucorrhea). In fact, care of such patients can sometimes require a substantial learning curve. The purpose of this short communication is to share issues that health care providers may encounter in the care of hemiplegic stroke patients and proposes strategies to minimize these challenges. Experience with care of a hemiplegic stroke patient has demonstrated that special attention should be directed towards maintaining adequate hydration and promoting good incontinence care practices. The area of mangrove was reduced by about 55%. Thus the economic, social, and environmental value of mangroves must be assessed over shortto long-term scales and employ these assessments for the awareness rising at local communities. This study provides a preliminary assessment of the risks on mangrove vegetation; it will provide database to mitigate the tremendous pressures due to coastal development and urban activities. The effects of human development on the mangrove plant cover in Eastern Region of KSA were recognised, during 2013 to 2016. The mean variations of physio-chemical characteristics in water and sediment were recorded. With regard to water analyses including; nitrogen, phosphorus, TSS, TDS, BOD, and turbidity were evaluated. On the other hand, for sediment: TDS, nitrogen, phosphorus, sulphate and total organic carbon were assessed. Moreover, the growth parameters: plant height, and size index, of Avicennia marina were recorded and estimated. The results revels that the mangrove communities subjected to severe degradation and depletion.
