Healthcare professionals at the University Hospital of Rouen have been trained in developmental care based on André Bullinger's approach, using sensorimotor behavior observation grids to assess preterm infants. While compelling, this approach have been studied less extensively than the NIDCAP program. Being part of a larger effort to promote Bullinger's approach, this first study aims to evaluate the feasibility of using these grids.</div></div><div><h3>Materials and methods</h3><div>This is a monocentric, observational cohort study conducted at the University Hospital of Rouen. All preterm neonates born before 33 weeks of gestational age (GA) and admitted between January 1, 2019, and December 31, 2023, were included. Bullinger's observation grid was proposed in the newborn's digital health record and should be filled weekly.</div></div><div><h3>Results</h3><div>A total of 979 preterm infants under 33 weeks of GA were included. Sixty-one percent of the included preterm infants were observed during their hospitalization. Preterm infants between 28 and 30 weeks of GA received significantly more observations per week, with 0.44 observations per week on average. Mean observations recorded significantly increased, reaching a peak in 2021.</div></div><div><h3>Conclusion</h3><div>The use of sensorimotor observation is feasible. Nineteen professionals participated in the development of the “Internet Plus Newborn Care” model, while seventy newborn caregivers in Lincang from January to December 2023 participated in an evaluation of the model's efficacy. The intervention group was provided with ‘Internet Plus Newborn Care’ model, whereas the control group received standard care. Newborn outcomes and family satisfaction were assessed. The data were analyzed by using descriptive statistics and Chi-square test.</div></div><div><h3>Results</h3><div>The “Internet Plus Newborn Care” model was developed based on the concepts of “Internet Plus Nursing Services” and “Home Care or Newborns”. Comparing with the control group, the intervention group had a significantly higher exclusive breastfeeding rate (77.1 % vs. 31.4 %, p = 0.0001) and lower rate of umbilical cord stump infection (2.8 % vs. 14.3 %, p = 0.027 at one month after birth. Although family satisfaction was higher in the intervention group, the difference between the two groups was not statistically significant (p &gt; 0.05).</div></div><div><h3>Implications for practice and research</h3><div>These results suggest that this developed “Internet Plus Newborn Care” model should be encourage in remote mountainous areas. Participants included nine caregivers.</div></div><div><h3>Result</h3><div>Six themes were identified: types of education provided at the clinic, the clinic's supportive and non-judgmental environment, awareness of infant development, increased confidence or affirmation, reliance on personal experiences in decision-making, and caregiver-identified needs.</div></div><div><h3>Conclusion</h3><div>Healthcare providers should continue to ensure that education is provided in a supportive and non-judgmental environment. Caregivers need additional education to understand developmental milestones and educational resources regarding NAS, acid reflux, and specialized medical conditions. Notably, it escalates the suffering and mortality of low birth weight neonates in sub-Saharan frica. However, there are limited studies regarding sepsis and its predictors among these neonates in Ethiopia. Therefore, we aimed to assess the incidence of early neonatal sepsis and predictors among low birth weight neonates.</div></div><div><h3>Methods</h3><div>A retrospective follow-up study was conducted among 407 low birth weight neonates from September 1, 2021, to October 31, 2023, in Northwest Ethiopia. A data abstraction checklist was used for data collection, and a simple random sampling technique was employed. EPI-DATA™ 4.6.06, and STATA™ 14 were used for data entry and analysis respectively. Kaplan Meir cure and log-rank test were computed. Both bi-variable and multivariable Gompertz regression analyses were applied to identify significant predictors.</div></div><div><h3>Results</h3><div>The incidence of early-onset neonatal sepsis was found to be 15.6 (95% CI: 13.85–17.56) per 100 neonate's day observation. Spontaneous vaginal delivery (AHR = 2.42; 95%CI: 1.66–3.51), maternal history of UTI/STI (AHR:1.40; 95% CI; 1.05–1.86), resuscitation (AHR = 1.46; 95% CI; 1.12–1.89), perinatal asphyxia (AHR = 1.47; 95% CI:1.01–2.11) prematurity (AHR = 1.36; 95% CI: 1.02–1.80), and birth weight&lt;1000 gm. (AHR = 2.15; 95% CI:1.12–4.14) were predictors.</div></div><div><h3>Conclusions</h3><div>The incidence of early-onset neonatal sepsis among low birth weight neonates was significantly high. Spontaneous vaginal delivery, maternal UTI/STI, resuscitation, perinatal asphyxia, prematurity, and birth weight &lt;1000 were predictors. Hence, the stakeholders need to strictly implement the national infection prevention strategies and critically follow those high-risk neonates. The use of preoperative enteral nutrition aims to address these deficits, improving growth and recovery. This review assesses preoperative enteral nutrition's efficacy and safety in neonates with congenital heart disease, whilst also identifying potential barriers to implementation of preoperative enteral nutrition, focusing on growth, recovery, and complications like necrotizing enterocolitis. While preoperative enteral nutrition can improve outcomes, concerns about necrotizing enterocolitis particularly in high-risk neonates, limit its use. Recent studies suggest management with human milk, can mitigate necrotizing enterocolitis risk as well as support intestinal maturation, improve immunity and improve growth. However, inconsistent feeding protocols across institutions create variability. Standardized protocols are needed to optimize nutritional outcomes. The participants’ assessment of the utility of the tool, particularly regarding language and content, in everyday clinical practice was also explored.</div></div><div><h3>The principal results</h3><div>The CO-PARTNER tool was translated following Wild's 10-step process. Cognitive debriefing was conducted through three focus group interviews with parents and nurses. These interviews revealed how to adapt the Norwegian version in terms of item order and wording and emphasised the importance of timing when introducing the tool to parents.</div></div><div><h3>Conclusion</h3><div>The CO-PARTNER tool was positively perceived as a valuable contribution to fostering parental involvement in infant care. A multi-disciplinary team supports the families to coordinate therapies and follow-up care with the goal to shorten hospital stay and ensure success of treatment amongst others. Video consultations (VC) could help overcome challenges in structurally weak regions, complex illnesses and language barriers.</div></div><div><h3>Objective</h3><div>To assess feasibility of this method and satisfaction of families, standardized interviews were conducted after VC and home visits.</div></div><div><h3>Methods</h3><div>A standardized interview with 21 questions using a 7-point-Likert-scale was applied to evaluate feasibility and parent satisfaction of SMN via VC.</div></div><div><h3>Results</h3><div>A total of 138 interviews were conducted and compared (99 after VC and 39 after in-person consultations). There were no significant differences found between the VC and in-person home visit groups in terms of confidentiality, feasibility, and discussion of private and sensitive information.</div></div><div><h3>Conclusions</h3><div>Parents' acceptance and positive evaluations of the confidentiality of VC offers the possibility of using them beyond the pandemic. However, home visits were preferred. The Difficult Intravenous Access (DIVA) score is a predictive tool designed to assess the level of difficulty prior to IV access attempts. This cross-sectional study aimed to evaluate the success rate of peripheral IV access in neonates performed by clinical nurses, considering the DIVA score and other influencing factors.</div></div><div><h3>Methods</h3><div>A random sampling method was used to recruit participants. The DIVA score was assessed before each IV placement. Variables recorded included the clinical competence of nurses, DIVA score, training history, years of neonatal experience, needle insertion experience, and gestational age. Multivariate analyses were conducted with a significance level of 5 %.</div></div><div><h3>Results</h3><div>A total of 60 clinical nurses and 60 neonates were included in the study. Multivariate analysis revealed that the DIVA score (OR = 0.525, p = 0.028) was the only statistically significant factor associated with IV access success. Needle insertion experience showed a strong positive influence but was borderline significant (OR = 36.863, p = 0.060). Other variables, including clinical competence (OR = 0.064, p = 0.106), training history (OR = 4.133, p = 0.333), years of neonatal experience (OR = 0.972, p = 0.732), and gestational age (OR = 1.020, p = 0.873) did not show significant associations with success.</div></div><div><h3>Conclusion</h3><div>The DIVA score was the only variable significantly associated with the success of peripheral IV access in neonates, while needle insertion experience showed a borderline positive effect. Other factors, such as clinical competence, training history, years of neonatal experience, and gestational age, were not significantly related to success in this study. However, various aspects of the NICU environment—such as physical design, workflow, organizational culture, and staff experience—can pose challenges to its effective implementation. Understanding parents' lived experiences is essential for identifying barriers to FCC and uncovering opportunities for improvement. This study aims to explore parents' perceptions and perceived needs regarding their involvement in their infant's care in NICUs.</div></div><div><h3>Methods</h3><div>The authors conducted a comprehensive systematic literature review using five electronic databases. We identified primary studies using CINAHL, MEDLINE, Embase, Scopus, and MIDIRS. The team assessed the full-text papers included in the review for quality using the Joanna Briggs Institute checklist (JBI). We completed the literature search on the February 6, 2024.</div></div><div><h3>Result</h3><div>We included 17 relevant studies in this review. Our analysis identified four main themes related to family-centred care challenges and/or facilitators: Parental Mental Health, Staff-Parent Partnership, Informational Needs, and the NICU Environment. These themes highlight critical factors that impact the implementation and effectiveness of family-centred care in neonatal intensive care units (NICUs).</div></div><div><h3>Conclusion</h3><div>This review identified key challenges and facilitators in delivering family-centred care in NICUs and provided initial recommendations for improving practice. While FCC challenges and facilitators exist worldwide, their impact on practice may vary depending on cultural beliefs and healthcare systems.
