The incidence of ischemic stroke increases with age and is particularly high in people aged ≥80 years. This study aimed to evaluate the efficacy and safety of reperfusion therapies (intravenous thrombolysis, endovascular thrombectomy, intravenous thrombolysis+ endovascular thrombectomy) in acute ischemic stroke patients aged over 80 years in one of the largest comprehensive stroke centers in Armenia. The study was based on retrospective analyses of prospectively gathered data from all stroke patients of Astghik medical center benefiting reperfusion therapies from 1 February 2021 to 30 April 2022. The safety was evaluated by assessing in-hospital complications and in-hospital and 3-months mortality rates. The efficacy was defined as neurological improvement at discharge and a modified Rankin scale 0-2 (or a return to at least the pre-stroke modified Rankin scale) at 3 months. A total of 216 patients with acute ischemic stroke were treated with reperfusion therapies. The number of patients aged 80 years or over was 41(18,9%). The study group had a mortality rate of 9,7% in an in-hospital setting and 22% at 3 months. Symptomatic intracranial hemorrhage was detected in 3 cases (7.3%). None of the patients developed aspiration pneumonia. 32 patients (78%) had neurological improvement at discharge and 21 patients (51%) were independent or regained pre-stroke activities at 3 months. One of the causes of maternal death is postpartum hemorrhage. Previous studies have found that postpartum hemorrhage is closely associated with uterine atony. This study aims to explore risk factors for postpartum hemorrhage caused by uterine atony. A case-control study design in Dr. Soetomo General-Academic Hospital. Patient medical records for a period of 2 years at the Dr. Academic General Hospital Soetomo Indonesia used and divided into case and control groups with a ratio of 1:2. Chi-square analyses odds ratios were calculated. A total of 32 patients with uterine atony and 64 medical records without uterine atony were studied. The results of statistical tests on several factors related to postpartum hemorrhage due to uterine atony showed that advanced maternal age (>35 years) (p= 0.763; 95% CI 0.474 – 2.768), grand multipara parity (p = 0.238; 95% CI 0.583-8.185), anemia with Hb <11 (p=0.027; OR=5.000. 95% CI 1.073–23.303), uterine overdistention including macrosomia, polyhydramnios, or gemelli (p=0.031; OR=2.647, 95% CI 1.081-6.482), accelerated labor (p=0.884, 95% CI 0.396-2.221, and previous history of postpartum hemorrhage (p=0.047; OR= 2.435, 95% CI 1.003 – 5.933). This case control study found that anemia, uterine overdistention, and a previous history of postpartum hemorrhage may increase the risk of postpartum hemorrhage due to uterine atony. {"title":"Prevalence of weight status and associated factors of underweight among the medical students in Iran","authors":"F. Karimpour, S. Published studies most researched overweight/obesity and its contributing variables in this population. However, underweight can threat health and wellbeing of the students during academic period. The Objective of the present study was to investigate the prevalence of weight status and to explore associated factors of underweight among the students from Yasuj University of Medical Sciences.\u0000Methods: In a cross-sectional study, a random sample including 455 students were recruited from Yasuj University of Medical Sciences, Iran, during 2018-2019. Academic, socio-demographic and anthropometric characteristics of the students were gathered through a structured questionnaire. Body mass index was determined and classified according to the World Health Organization reference. Descriptive statistics were used to summarize data in numbers and percentages. The relationships of students’ characteristics with their weight status were checked using Chi-square test. Factors associated with underweight occurrence in students were detected by analyzing multiple binary logistic regression model and adjusted odds ratio.\u0000Result: Of the total participated students 87.5% were 19-24 years old and 66.2% were female. The mean ± standard deviation of students’ weight and BMI were 61.42±11.51and 22.14±3.36, respectively. The prevalence of underweight, normal weight and overweight/obesity among students were 15%, 66.15 and 18.9%, respectively. Among the considered variables, age (OR: 0.78 95% CI: 0.63,0.97), gender (OR:2.o8 95% CI: 1.o8, 3.99), number of educational term (OR:1.25, 95% CI: 1.06, 1.47) and father’s occupation (Or:2.13 95% CI:1.14, 3.99) were significantly associated with the underweight occurrence in students.\u0000Conclusion. The prevalence of underweight and overweight/obesity among studied university students were high. The younger students, female students, students at higher number of educational term and students with fathers not an employee were at higher risk of being underweight. As time passes and our understanding of this novel respiratory virus deepens, it is increasingly clear that its effects extend beyond that of the respiratory system. The link with endocrine disorders was noticed quite early when it was found that patients with diabetes and uncontrolled hyperglycemia were at an increased risk of severe disease as well as mortality from COVID-19. However, the other endocrine manifestations of COVID-19 probably were more subtle and information about them emerged more gradually over a period of time. Thyroid diseases are common endocrine disorders, and accordingly, a lot of attention has gone into the study of how COVID-19 affects the thyroid. Although the majority of mild to moderate COVID-19 patients remain euthyroid, a significant proportion of those with severe disease manifest with abnormalities in thyroid function. These manifestations include low thyroid stimulating hormone and low T3 levels, whereas low T4 levels were observed less commonly. Apart from this, several reports of subacute thyroiditis following COVID-19 have also been published. Similarly, Graves’ disease has been reported to occur in patients who had recovered from COVID-19. Under physiological conditions, perivascular adipose tissue maintains normal vascular function by releasing anti-atherogenic, anti-inflammatory, and vasodilating biologically active substances. Until recently, adipose tissue was considered a morphological unit with only a thermoregulatory and shock-absorbing function; however, with the development and improvement of modern medical science, we can confidently assert that adipose tissue is a factory for the production of biologically active substances with a broad spectrum of action. The released biologically active substances have both autocrine and paracrine effects, thus playing a pivotal role in maintaining the morphophysiological balance of the whole organism.\u0000Various theories are put forward about the possible key role of perivascular adipose tissue in the pathogenesis of various diseases. It has been shown that perivascular tissue is an independent cardiovascular risk factor, even without visceral obesity. When exposed to specific pathogens, the regulation of adipocytes is disrupted, and a subsequent rearrangement of the adipocyte production profile occurs. The secretion of damaged adipocytes exhibits the following properties: anti-atherogenic, anti-inflammatory, and vasodilating, with subsequent initiation or progression of cardiovascular disease.\u0000Within the framework of this review article, the following are raised and comprehensively discussed: the function of perivascular adipose tissue in health and disease and its contribution to the pathogenesis of cardiovascular disease. Endothelial dysfunction as a cornerstone of cardiovascular diseases’ pathophysiological mechanisms, causes overactivation and over aggregation of platelets resulting in the development of ischemic complications.Though we have huge arsenal of antiplatelet and anticoagulant drugs used for prevention and treatment of ischemic consequences, there is an increase of annual cases of side effects associated with pharmacotherapy.The “gold standard” of antiplatelet therapy - Aspirin gradually loses its role and importance in the primary and secondary prevention, as well as at the treatment of cardiovascular diseases due to the haemorrhage mostly on those patients who are under 70, have diabetes or are in the low and mild risk group of cardiovascular disease.That is why the development of newer and safer substances with possible less hemorrhagic complications has become an important task of modern medicine.In this point of view the plant based agents, containing rich composition of polyphenolic substances, particularly anthocyanins and flavonoids, could serve as potent and promising sources due to their antiplatelet effects, which is conditioned with several action mechanisms.Each action mechanism of polyphenolic substances is directed to the different phases of platelet aggregation. A study of calcium-regulating hormones is a crucial element to understand underlying pathophysiological mechanisms of heart failure. Pro-inflammatory factors, released during mechanical, hypoxic or bacterial damage of myocardial cells, lead to an imbalance of calcium and disrupt to heart function. The investigation of mentioned factors influence mechanism on the heart, is an urgent solution for preventing the development of heart failure.\u0000Present study aimed to reveal the role of calcium-regulating hormones in heart functional activity and their possible involvement in the development of heart failure. The pharmacological analysis of the action mechanism of bacterial lipopolysaccharides on heart functional activity was carried out using a calcium channel blocker. \u0000The concentrations of calcium-regulating hormones in blood serum in patients suffering from heart failure was determined by immunoassay enzyme method, and ionized calcium and inorganic phosphate concentrations - by spectrophotometric method. The photoelectrical method was used to determine the direct effect of calcium-regulating hormones and possible calcium-dependent action mechanism of bacterial lipopolysaccharides on the isolated frog’s heart. \u0000Clinical findings show that chronic heart failure is accompanied by shifts in the calcium-regulating hormonal system and blood electrolyte balance. In vitro experiments on isolated frog hearts have shown the potentiating effect of parathyroid hormone, its related protein, calcitonin, and vitamin D3 on myocardial contractility. It has been shown, that bacterial lipopolysaccharides suppress the contractile and rhythmogenic functions of the myocardium, and their action can be mediated through a calcium-dependent mechanism. \u0000The increase of parathyroid hormone in chronic heart failure has a protective significance aimed at maintaining the contractile ability of a weakened myocardium and preserving cardiac output. Pharyngooesophageal perforation, aorto-oesophageal fistula, and deep neck infection are all serious consequences, albeit uncommon. Several risk factors for complications that aggravate this condition include time of therapy (> 24 hours after the onset of symptoms), sharp foreign objects such as fish bones, mental illness, and diabetes mellitus are risk factors for complications. Although deep neck abscesses have decreased due to the availability of antibiotics, these infections still occur with high morbidity and mortality rates if the patient is not managed properly.\u0000Aim: to report case of fatal deep neck abscess and aerodigestive complications of foreign agency in Dr. Soetomo Teaching Hospital, Surabaya Indonesia.\u0000Method: A retrospective report of 4 cases of ingestion of foreign bodies with complications of deep neck abscess resulted in death in Dr. Soetomo Academic Hospital, Surabaya between the year 2018 and 2020. This case report describes clinical symptoms, result of physical and diagnostic examinations, and therapy of each subject until death.\u0000Results: Between 2018 and 2020, there were four cases at Dr. Soetomo Teaching Hospital, Surabaya with a history of foreign body ingestion that escalated to severe neck abscesses. In all four cases, the subjects died as a result of deep neck abscess complications.\u0000Conclusion: Four cases were recorded, each of which had a history of ingestion of foreign bodies causing complications of deep neck abscess. Abovyan","doi":"10.56936/18290825-2022.16.4-14","DOIUrl":"https://doi.org/10.56936/18290825-2022.16.4-14","url":null,"abstract":"Calcium is involved in all vital processes of the body: synaptic transmission, memory development, immunity, blood clotting, heart contractions, etc. In this regard, it is important to understand the involvement of calcium in the development of SARS-CoV-2 virus infection and COVID-19 disease. \u0000We have studied the existing scientific literature, looking for the involvement of both calcium and calcium-regulating hormones (parathyroid hormone, calcitonin, vitamin D) in COVID contagiousness and severity of disease. \u0000Separately, both hypocalcemia and vitamin D deficiency have been identified in a number of large clinical trials as a predictor of mortality in patients hospitalized with COVID-19. Angiotensin-converting enzyme-2 plays a key role in contagiousness with the SARS-CoV-2 virus, and its formation is a calcium-calmodulin-dependent process. In our opinion, in order to prevent the penetration and spreading of the virus in the body, this link should be targeted with the usage of drugs that activate the calcium calmodulin system. Calcium blockers used parallel with hypocalcemia, to some extent, reduce the spreading of the virus in the body in the acute phase of the disease, but it is possible that in the future they lead to deeper and long-term complications - cognitive dysfunction.\u0000However, we did not find any study in the literature that aimed to identify calcium-dependent mechanisms in a complex, dynamic study of the same patients. Therefore, this question is still open in science, because without understanding how the amount of calcium-regulating hormones changes during the disease, without understanding how much phosphorus changes in parallel with calcium, it is impossible to get a complete picture of the disease on the role of calcium-dependent mechanisms in development. Therefore, nurses are required to be knowledgeable and positively oriented towards patients’ rights. \u0000Objective: The purpose of the study is to investigate the attitudes of psychiatric nurses at Eradah mental health complex in Jeddah city towards patients’ rights. \u0000Material and Method: A descriptive cross-sectional research design was adopted in this study. The study used the convenient sampling method to recruit a sample of 171 psychiatric nurses. To collect data, the study used the questionnaire that consisted of two parts: the socio-demographic part and the attitudes towards patient’s rights part (28 statements). \u0000Result: The results of the study showed that psychiatric nurses at Eradah mental health complex had positive attitudes towards patients’ rights (4.06±0.76). The results showed that the highest positive attitudes were related to those patients should receive healthcare services in safe environment, having the right to choose the healthcare providing institution or change it if needed, and the right to benefit from any provided healthcare services that comply with their medical condition.
