The development of resistance to antibiotics in bacteria often develop as a result of unnecessary and inappropriate use of antibiotics. Through the intense use of antibiotics, resistant microorganisms have emerged over the years, and problems were started to be experienced for the treatrment of these infections emerged with these resistant microorganisms. Today, on the one hand trying to develop new drugs, on the other hand, there are difficulties in treatment as a result of development of resistance to these drugs rapidly. The development of resistance to antibiotics is a major public health problem in all over the world (1-3). The main four types of resistance to antibiotics develops; 1. Natural (Intrensic) resistance 2. Acquired resistance 3. Cross-resistance 4. Multi-drug resistance and pan-resistance 1. Natural (Intrensic, Structural) resistance: This kind of resistance is caused by the structural characteristics of bacteria and it is not associated with the use of antibiotics. It has no hereditary property. It develops as result of the natural resistance, or the microorganisms not including the structure of the target antibiotic, or antibiotics not reaching to its target due to its characteristics. For example, Gramnegative bacteria vancomycin does not pass in the outer membrane so Gram-negative bacteria is naturally resistant to vancomycin. Similarly, L-form shape of bacteria which are wall-less forms of the bacteria, and the bacteria such as cell wall-less cell Mycoplasma and Ureaplasma are naturally resistant to beta-lactam antibiotics that inhibit the cell wall synthesis (1,4-6). 2. Acquired resistance: As result of the changes in the genetic characteristics of bacteria, an acquired resistance occurs due to its not being affected from the antibiotics it has been responsive before. This kind of resistance occurs due to mainly structures of chromosome or extrachromosomal (plasmid, transposon, etc.). a. Chromosomal resistance arise from mutations in developing in spontaneous bacterial chromosome (spontaneous). Such mutations may occur according to some physical (ultraviolet, etc.) and chemical factors. This can be a result of structural changes in bacterial cells. The result may be reduced permeability of bacterial drug or changes of the target of the drug may be in the cell. To improve survival in arrest cases, recommendations have been revealed especially for data collection, education, and public participation. The report stresses that it is necessary to centralize data collection and distribution, monitor and improve care services, increase the impact of research and treatments, and strengthen stakeholder communication. The study by Van Diepen et al. published in the Journal of American Heart Association discussed the first steps to fulfill most of the recommendations of The Institute of Medicine report. The Heart Rescue Project, implemented by Van Diepen et al., created a population-based record bank that covered 90% of the US population. Of prehospital arrest cases, bystander CPR was performed in 42.8% and automated external defibrillator was used in 4.6%. In 21.9% of these cases, CPR was performed by police officers or first aiders before the ambulance team arrived. In this context, CPR and first-aid defibrillation rate increased from 14.1% to 23.1% and, with positive neurological outcomes, the survival rate increased from 7.1% to 9.7% [2]. According to a study published in 2015 on survival rates in arrest cases, special importance was attached to traumatic arrest cases [1]. Staphylococcus aureus that was isolated from skin lesions of psoriatic patients was subjected to gel filtration chromatography for extraction and purification of their exoproteins. The skin of BALB/C mice was injected by 0.2 ml of the extracted purified exotoxin.. The induced lesions were markedly similar to that of human psoriatic lesions, although the histopathological changes were not completely mirrored to that of human psoriatic skin lesions. Thalassemias are monogenetic hereditary disorders related to decrease in the synthesis of globin chains (alpha, beta, delta, and gamma). These are the most prevalent monogenic disorders in the world though mostly seen on the hemoglobinopathy belt. Patients with homozygous, double heterozygous condition present as microcytic hypochromic hemolytic anemia, but minimal hematologic findings might also be seen with heterozygous inheritance. Öztürk, Abdulkadir But","doi":"10.5505/ias.2022.58672","DOIUrl":"https://doi.org/10.5505/ias.2022.58672","url":null,"abstract":"This study aimed to compare Baska Mask laryngeal mask airway (LMA) with classic LMA in terms of clinical performance, ease of use, and patient comfort. The duration of placement was compared between the groups as the primary outcome. Comparisons were also made for the number of LMA entry attempts, hemodynamic parameters. and postoperative complications. The study included 66 patients. The patients were randomly separated into two groups using the sealed-envelope method. After general anesthesia induction, a new LMA (Baska Mask) or a classic LMA was placed in the mouth of all the patients. The heart rate, blood pressure, mean and peak airway pressure, and amount of ventilation leakage were recorded throughout the surgery. The patients were examined for the presence of blood smears after removing the LMA. One hour after the surgery, an anesthetist blinded to the groups asked all the patients whether they had throat pain. The duration of LMA placement was determined as 14.5 s for classic LMA and 16.0 s for new LMA, with no significant difference between the groups ( P = 0.117). The number of attempts at entry was similar in both groups ( P = 0.741). The mean and peak airway pressure measurements 5 min after LMA placement were statistically significantly different between the groups ( P < 0.05), with higher values in the new LMA group. No blood smears were observed in the patients using new LMA, while blood smears were determined in seven patients in the classic LMA group ( P = 0.012). The results of this study showed that an adequate, safe open airway was provided throughout the surgery in both groups.
