Melasma QoL Scale (MELASQoL) is a specific questionnaire that has been developed to assess the effect of melasma on patient’s QoL. Objective: Translation, cultural adaptation, reliability and validation of melasma specific QoL sale (MELASQoL) into Arabic. Patients and Methods: A total of 65 adult Egyptian female patients with melasma aged ≥18 years old with skin phototypes type III-V were enrolled in this study. Wood’s light was used for determination of the type of melasma either epidermal, dermal or mixed. Melasma severity was assessed by using the melasma area and severity index (MASI) score and the new modified score (mMASI). The impact of melasma on QoL was evaluated using the Arabic version of MELASQoL scale after translation and cultural adaptation according to the WHO guidelines. The questionnaire’s reliability and validity was assessed. Results: The internal consistency for the 10-item of the MELASQoL-A scale was excellent; Intra-class Correlation Coefficient (ICC) = 0.914 and Cronbach’s alpha coefficient (Cronbach α = 0.944) indicating high reliability of the developed Arabic version. There was significant positive correlation between the MELASQoL-A scale and the MASI score (r= 0.41; p= 0.0027), and the mMASI score (r = 0.36; p= 0.0032). Newer H1 blocker rupatadine alone is similarly effective with less relapse rate. Also it is convenient for the patient as similar efficacy is obtained with single dosage compared to 3 times dosing in combined therapy. This study was done to compare the traditional treatment with levocetirizine and ranitidine to a newly introduced antihistamine rupatadine for CIU. Materials and Methods: The study was a hospital based prospective randomized control trial among 40 patients with CIU in Dermatology and Venereology department of Bangabandhu Sheikh Mujib Medical University(BSMMU), Dhaka from April 2020 to September 2020.Forty patients of CIU were randomly enrolled into two equal groups (group A and B). Patients of group-A were treated with 5 mg of levocetirizine once daily plus 150 mg of ranitidine twice orally daily and group-B were treated with rupatadine 10 mg once daily for one month. The efficacy was assessed 1st and 4th week during treatment and 4 weeks after completion of treatment by observing reduction of itching, regression of the size and shape of lesions and appearance of new lesions. Adverse effects and patient satisfaction were also noted. Results: 75% patients in group A and 80% patients in group B responded to treatment (p>0.05).80% in group A and 85% in group B showed improvement in itching in the first week (p>0.05). . At the end of 4 weeks 95% showed improvement in each group. Appearance of new lesions in first week was 10% and 5% (p>0.05) and at 4th week, 5% and 0% respectively (p>0.05). 75% in group A and 80% in group B had regression in their lesions at the end of first week (p>0.05). At the end of 4th week, it was 85% and 90% (p>0.05). 40% in group A and 25% in group B had relapse of itching at follow up (p<0.05). Relapse of lesions were 35% and 20% (p<0.05). Overall occurrence of side effects (3 compared to 1) was more in group A. Conclusion: The result of the present study show that both conventional treatment with levocetirizine and ranitidine combination and newer agent rupatadine alone has similar efficacy in reducing clinical sign and symptoms of CIU. But rupatadine has significantly reduced the relapse rate and so it is a more efficacious and also safer option with less adverse effects for the treatment of CIU in comparison to conventional treatment. Distinct factors dominate the prescription of anti-depressants in the geriatric population. Accordingly, the intention of pharmacotherapy in geriatric psycho dermatoses is deciphered in the study. . Objective: To survey and analyze the selection of anti-depressants by the dermatologists in the practice of geriatric pharmacotherapy. Methods: A questionnaire was designed and circulated among ninety-three dermatologists in three sessions and the data collected was analyzed through the cross-sectional study statistics. Results: The typical senior people attending the specialist were 25-50%. A sizeable familiarity of psycho cutaneous disturbance was attributed to dermatophyte infections, lichen simplex chronic, and Prurigo in descending order. The largely established primary disorder was neurotic excoriation. The prescription pattern was classified and 51.9% dermatologists are customary with psychotropic agent Doxepin over other agents. The discipline of the multidisciplinary approach was also studied. Limitation: Small sample size, uniregional study, prevalence study. Tyrosine kinase inhibitors are a class of drugs that can be used either primarily as oral medicines or in dermatological definitions locally. In several skin problems, restraint of tyrosine kinases has been demonstrated to be advantageous. Aim of this article is to explore the role of tyrosine kinase inhibitors in various skin diseases. Tyrosine inhibitors have demonstrated a guaranteed response in psoriasis, alopecia areata, Vitiligo, while the variable response in other skin disorders, including atopic dermatitis, chronic actinic dermatitis, dermatomyositis, erythema multiforme, graft-versus-host disease, hypereosinophillic syndrome, STING vasculopathy, palmoplantar pustulosis and lupus. The approach of Tyrosine inhibitors in dermatology has been met with incredible interest. This class of prescriptions can generously propel the treatment of inflammatory and immune mediated dermatoses. The Hemangioma Severity Scale was formulated by Haggstrom et al 2012, to measure the severity of hemangiomas according to the identification of clinical variables, assigning morbidity values associated with already published variables and in the clinical experience of experts on the subject. However, it has been designed and developed in the American population. Therefore, the objective of this study was to validate and translate the Hemangioma Severity Scale for its application in Mexican pediatric patients. Materials and methods: The hemangioma severity scale was applied by 2 expert pediatric dermatology subspecialists, and then the responses were compared using correlation indices by a statistician who was biased. Results: A coefficient was performed on the first item, an index of 0.714 was obtained. The following items obtained a 0.799 for the location, a 1 for the risk of structural abnormalities, 0.516 for complications, 0.298 for the item of pain and in the general risk and probability of deformation a kappa index of 0.670 was obtained. This condition is characterized by the presence of a myopathic syndrome with bilateral and symmetric weakness predominantly of extensor, proximal muscle groups, respiratory muscles and esophageal involvement, accompanied by characteristic skin disorders. The worldwide incidence is reported at 1 to 6 cases per million inhabitants; predominates in women, with 2.5:1 ratio; and has a bimodal distribution with two peaks, one from 5 to 14 years old and another from 45 to 64 years. The amyopathic form is present in 2 to 18% of all cases of dermatomyositis. Diagnosis is supported by alteration in muscle enzymes, as well as characteristic findings in magnetic resonance imaging (MRI), electromyography (EMG) and autoantibodies. The definitive diagnosis is determined by the histopathological study of both, skin biopsy and affected muscles. The purpose of this case is to review the diagnostic approach from a dermatological point of view, where characteristic lesions gave way to a thorough physical examination looking for signs of a myopathic syndrome; and where despite the elevation of muscle enzymes, findings in electromyography and MRI compatible with myositis, the muscle biopsy was negative.
