Exploring new medications to improve ejaculation latency offers a promising path for more effective therapies. Methods: This study was conducted to examine the effects of compound Jinji granules (CJG) on the ejaculatory behavior of a rat model. The study included a blank group, a model group, a CJG formula group, and a positive control group, with 10 rats in each group. The model rats were selected based on the continuum principle of ejaculation. The CJG group was gavaged with the CJG formula, while the positive control group was intraperitoneally injected with Cligosiban for four weeks. The sexual behavior of the rats, including indicators such as ejaculation latency (EL), was observed, and the data were statistically analyzed using SPSS 25.0 software. Results: The model group exhibited significantly reduced intromission latency (IL), ejaculation frequency (EF), and mount frequency (MF) compared to the blank group. Compared to the model group, CJG significantly prolonged IL, EL, and MF ( P < 0.01), extended IF ( P < 0.05), and reduced EF ( P < 0.05). There were no significant differences between the CJG group and the positive control group (Cligosiban) in terms of IL, EL, MF, IF, and EF. Conclusion: CJG significantly improved sexual behavior in the PE rat model, extending EL and reducing the frequency of ejaculation. These results suggest that CJG may offer a promising therapeutic option for treating PE.","PeriodicalId":349243,"journal":{"name":"Integrative Medicine in Nephrology and Andrology","volume":"12 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2025-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147895161","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Building upon this assessment, we put forward a prospect for the establishment of TCM-specific strategies for PE, including more targeted and individualized comprehensive diagnosis and treatment approaches for PE, the establishment of a TCM hierarchical management program, and the exploration of the most effective entry points for collaborative treatment of PE using both TCM and Western medicine. Indeed, integrating therapeutical strategies from the two perspectives in a multifaceted and complex symptom such as PE, would enhance the possibility of success, possibly also reducing side effects. Moreover, with the help of well-established and new scientific and technological tools, we can explore the key links and targets in treating PE by TCM. At the same time, the mechanism of TCM treatment for PE could and should be examined in depth to lay the foundation for widespread clinical application. Finally, we suggest conducting multicenter, nationwide standardized clinical studies on treating PE with TCM. These studies should employ high-quality randomized controlled trial designs to obtain robust evidence-based medical data.","PeriodicalId":349243,"journal":{"name":"Integrative Medicine in Nephrology and Andrology","volume":"108 7","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141666998","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The related pathological changes are primarily characterized by the deposition of immune complexes beneath the epithelial cells of the glomerular basement membrane, accompanied by diffuse thickening of the basement membrane. Its etiology and pathogenesis are not fully understood yet; however, they are associated with genetics, infections, tumors, drugs, heavy metals poisoning, environmental pollution, and the loss of immune tolerance. Over 10 target antigens, including the phospholipase A2 receptor (PLA2R) and thrombospondin type 1 domain containing 7A (THSD7A), associated with MN have been identified, each having different clinical implications. Clinical manifestations of MN patients mainly include proteinuria and nephrotic syndrome, with a propensity to form thrombi. Its diagnostic accuracy can be enhanced by combining clinical manifestations, serum antibody testing, and renal biopsy. Proteinuria, estimated glomerular filtration rate (eGFR), and PLA2R antibody levels are vital for the prognostic risk stratification of MN. Low-risk patients primarily undergo non-immunosuppressive treatment (angiotensin-converting enzyme inhibitor [ACEI]/angiotensin II receptor blocker [ARB], sodium-glucose cotransporter 2 inhibitor [SGLT2i], anticoagulants, and traditional Chinese medicine [TCM] treatments), while high-risk patients need to consider using steroids and immunosuppressants, such as cyclophosphamide (CTX), calcineurin inhibitors (CNIs), and anti-cluster of differentiation 20 (anti-CD20) monoclonal antibodies. TCM holds that MN is the imbalance of Fei (lung), Pi (spleen), and Shen (kidney) function, leading to the generation of Neixie such as Shi, Re, Du, and Yu. This disease is difficult to heal because of Shire and Yu. The treatment is based on the use of Jianpi Bushen, simultaneously emphasizing the use of drugs for Qingre Qushi Huoxue. Dialectical use of TCM can obviously improve the clinical response rate and alleviate symptoms such as edema and fatigue by increasing the serum albumin level more rapidly while introducing fewer adverse reactions. Combined with Western medicine, TCM can also reduce toxicity and increase efficiency. This article systematically reviews the etiology, podocyte antigens, clinical manifestations, diagnostic approaches, risk assessment, and integrative medicine therapeutic strategies for MN, aiming to deepen the understanding of MN and guide clinical practice.","PeriodicalId":349243,"journal":{"name":"Integrative Medicine in Nephrology and Andrology","volume":" 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141668497","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Signs and symptoms of uremic intoxication are often present even in presence of an adequate dialysis delivery. These along with cardiovascular and skeletal complications, have been correlated to the accumulation of inflammatory chemical mediators, beta-2 microglobulin (β2M), parathyroid hormone (PTH) and other middle to large molecular weight toxins that are insufficiently cleared by current dialysis techniques. Such condition determines a vicious loop where a subclinical status of inflammation causes a disruption of the immunological response affecting outcomes by accelerated atherosclerosis, anemia, and frequent infections. The overall picture can be described as a systemic inflammatory syndrome with simultaneous activation of the innate and the adaptive immunity. In such condition, new options and techniques are required to achieve a more effective blood purification and to correct the altered immuno-homeostasis. New efficient and biocompatible sorbents are today available (HA 130 Cartridge, Jafron Medical, Zhuhai, China) and they can be advantageously coupled in series with the hemodialyzer to perform hemoadsorption combined with hemodialysis (HA-HD). This technique has been already studied in at least two randomized trials demonstrating an effective improvement of clinical and biochemical outcomes. We have calculated the kinetics of β2M in a single session, in a series of three consecutive sessions of a week and in a period of three months using different frequencies of application (first month: Three sessions per week; second month: Two sessions per week; third month: One session per week). In the single session the reduction ratio was superior to other techniques such as hemodialysis (HD), high-flux hemodialysis (HFD) or hemodiafiltration (HDF). In the thrice weekly regime, the time average concentration (TAC) of β2M resulted inferior to HD and HDF. In the long period, a lower concentration of β2M was maintained even with a once-a-week regime. Considering the parallel reduction of inflammatory parameters, we could hypothesize that the enhanced removal of uremic toxins and chemical mediators led to a mitigation of the systemic inflammation with a progressive reduction in the generation of β2M. This “inflammation mitigation hypothesis (IMH)” supports the prescription of HA-HD once a week, possibly after a month of thrice weekly regime.","PeriodicalId":349243,"journal":{"name":"Integrative Medicine in Nephrology and Andrology","volume":"30 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140419858","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Although the TCM modernization project has made significant progress, several challenges, such as identifying modern biological indicators, remain. Recent investigations have demonstrated that the gut microbiota contributes to the metabolism and efficacy of TCMs, providing a potential indicator of TCM properties. To investigate whether the gut microbiota can partially interpret the property theory of TCM, we searched the “PubMed” and “China National Knowledge Infrastructure” databases using “gut microbiota” and “traditional Chinese medicine” as keywords. This review mainly focused on the relationship between mild-natured and sweet-flavored TCMs (MS-TCMs) and the gut microbiota. We discovered that MS-TCMs can recalibrate the gut microbiota to alleviate various diseases. At the family level, MS-TCMs upregulated Erysipelotrichaceae, Verrucomicrobiaceae, Bacteroidaceae, Christensenellaceae, Lactobacillaceae, and Prevotellaceae, while downregulating Enterobacteriaceae, Rikenellaceae4, and S24-7. At the genus level, MS-TCMs increased the abundance of Ruminococcus, Turicibacter, Parabacteroides, Dubosiella, Bifidobacterium, Bacteroides, Lactobacillus, Oscillospira, and Prevotella, and decreased the abundance of Parasutterella, Mucispirillum, Lachnoclostridium, Escherichia, Enterococcus, Butyricicoccus, Clostridiales, Blautia, Desulfovibrio, and Oscillibacter. Mechanistically, changes in gut bacteria can influence the levels of metabolites such as short-chain fatty acids, lipopolyaccharides, and neurotransmitters, which further regulate a series of physiological signaling pathways within the host. Overall, analyzing the TCM–microbiota interaction is an effective means to reveal the mechanisms of TCM, suggesting that the gut microbiota can be developed into a modern indicator of TCM properties.","PeriodicalId":349243,"journal":{"name":"Integrative Medicine in Nephrology and Andrology","volume":"91 ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139023832","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
