Furthermore, the day-to-day operation of these services has been focused only on maintaining a set of network-related metrics at adequate levels. The end user, on the other hand, understands Quality of Service as a matter of Customer Care and Support. This difference in understanding leads to the need of a change in both the methodology used by SP's to create new services and the techniques used to manage these services once deployed. The new service development methodology, the focus of this paper, must be able to assess users' needs and translate them into meaningful parameters to be captured by newly developed services. Quality Function Deployment fits this bill perfectly. The new operation techniques must bring the Service Provider closer to the more abstract requirements of the customer (including emotional and affective requirements and perceptions). The designed system consists of two major blocks; the first one is required to be carried to the patient home, named 'Portable Telecardiology Kit'. The second one, named 'Automated Cardiac Signal Processor', a PC based system, to be permanently placed at the nearest rural health care center. The 'Portable Telecardiology Kit' contains a portable ECG machine, a dedicated microcontroller-based full duplex communication system, both interfaced, in a single enclosure. This kit will convert the ECG signal along with a tag page (containing different information regarding patient's health and history of illness) to a digitized serial electromagnetic wave. The signal may be transmitted over a distance of about 7-10 km through a cordless phone under full duplex mode. The PC-based system, after receiving the transmitted signal from patient site, extract the ECG signal after proper filtering, and then pass the signal through an in-built knowledge-base. Finally, a rule-based rough set decision system is generated for the development of an inference engine for disease identification from these time-plane features. The system will generate a report indicating the preliminary level abnormalities, and the precautions that can be adopted at an early stage. With Internet and its infrastructures playing important role in the success of this field, it is not advisable to run some of the critical applications like high quality audio and video involved in telemedicine without proper quality of service (QoS) built on to the network. This paper focuses on two telemedicine setups that have been implemented on different backbone technologies. The first case discusses a virtual critical care unit that is setup for communication on an asynchronous transfer mode (ATM) backbone and a possible model on how QoS for important traffic streams can be achieved in ATM. The European ITAIDE project (IT for adoption and intelligent design for e-government) aims to develop and implement solutions that support improved interactions between government agencies and businesses, providing single window access and creating possibilities for gaining authorized economic operator status. As an example of this, we report on the e-export solution implemented by the Danish tax and customs administration, which supports the submission of export statements in electronic format through Single Window Access. Based on this case, we argue that in order to realize massive diffusion of e-customs solutions, it is crucial to address challenges regarding for instance interoperability and IT-readiness. In particular we investigate burst loss caused by bursts overtaking their control packets, and bursts losses caused by blocking. We have made a detailed discrete event simulation model of OBS networks and evaluated the performance of hot potato deflection when bursts are routed connectionless from ingress to egress. To our knowledge such detailed investigations into the effects of increased control packet lead time and causes of burst losses have never been performed before. We conclude that increased control packet lead time gives better performance, in particular for low loads.
