In this study we retrospectively analyze the long-term therapeutic effects of endovascular treatment of traumatic Type B aortic dissection to get some inspiration for the next step treatment and to provide a useful guidance. Method: 21 patients with Stanford type B aortic dissection caused by trauma endovascular treatment after strict condition assessment in our vascular center from December 1999 to May 2012 was recruited into this retrospective study. The overall diameter of the aorta, the number of tears and the extent of the dissection were observed based on three-dimensional reconstruction of the CTA during the follow-up. The recurrence of the operation and the 2-year and 5-year survival rates were recorded. Conclusion: Endovascular treatment of traumatic aortic dissection increases the survival rate of these patients who cannot tolerate traditional open surgery. This survival benefit sustained throughout the long-term follow-up.","PeriodicalId":34849,"journal":{"name":"Vascular Investigation and Therapy","volume":"65 1","pages":"15 - 20"},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82164168","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods: A total of 61 patients with uterine fibroid accept LUAO from January 2013 to December 2015 were selected as the research objects. The changes of ovarian hormone secretion volume, internal blood supply of uterine fibroid, uterine volume, fibroid volume, and the occurrence of complications were observed before and 3, 6, 12, and 24 months after surgery. These results were compared with the outcomes of 63 patients with uterine fibroid accept myomectomy. Results: Compared with myomectomy treatment, the length of intraoperative blood loss, operation time, and hospitalization time of patients in the LUAO group were shorter (P < 0.05). Menstruation characteristics, uterine volume, and uterine fibroid volume postoperation were better than the prior ones in the LUAO group (P < 0.05). The ovarian secretion of luteinizing hormone, estradiol, and follicle-stimulating hormone levels in patients in the following 24 months after surgery were not statistically different in two groups (P > 0.05), and the pelvic floor function had similar results. Conclusion: Uterine fibroid treated by LUAO is effective, it can decrease the internal blood supply of uterine fibroid, narrow the volume of uterine fibroid, and little impaction on ovarian function. Hence, this kind of treatment is worthy of promotion.","PeriodicalId":34849,"journal":{"name":"Vascular Investigation and Therapy","volume":"137 1","pages":"1 - 5"},"PeriodicalIF":0.0,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76745304","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Most of its epidemiologic data have come from the Western hemisphere suggesting female predominance and a high risk of venous thromboembolism. Initial diagnosis is still based on clinical suspicion and physical findings, but duplex ultrasound is often required to rule out concomitant deep venous thrombosis. Management options, including surgical and medical means, have been attempted with inconsistent result. In the last decade, clinical trials using anticoagulants such as low molecular weight heparin and new oral anti-Xa anticoagulants have demonstrated beneficial effects. Currently, oral nonsteroidal anti-inflammatories with or without compression are still the predominant initial treatment despite the proven clinical efficacy of oral anticoagulation in lowering thrombotic risk and symptom relief. In this review, most of the studies analyzed are after the year 2010 through PubMed index search and thus making it a contemporary update.","PeriodicalId":34849,"journal":{"name":"Vascular Investigation and Therapy","volume":"168 1","pages":"95 - 97"},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"88457325","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Noninvasive measurement of lower limb outflow resistance andimplications for stenting","authors":"A. Nicolaides, O. Maleti, M. Lugli, S. Guerzoni","doi":"10.4103/VIT.VIT_3_20","DOIUrl":"https://doi.org/10.4103/VIT.VIT_3_20","url":null,"abstract":"AIMS: Stenting to relieve iliac vein obstruction is now practical and safe. However, the rates of ulcer healing, pain, and edema relief are 76%, 52%, and 42% respectively. In addition, a high ulcer recurrence (up to 86%) has been reported. Currently, patient selection depends on symptoms, imaging methods, and intravascular ultrasound (IVUS) for the assessment of iliac vein stenosis without consideration of the collateral circulation. The aim of this article is to present the results of a noninvasive method of measuring lower limb outflow resistance (LOR) and also to test the hypothesis that LOR is extremely variable in limbs with iliac obstruction and that some patients with iliac stenosis may have a LOR close to that of normal limbs as a result of a well-developed collateral circulation.MATERIALS AND METHODS: LOR was measured at different venous pressures from 60 to 25 mmHg using air-plethysmography in 15 limbs without and 15 limbs with iliac vein obstruction. Reflux in ml/sec (venous filling index [VFI]) and venous clinical severity score (VCSS) were also measured in all limbs. RESULTS: The LOR at 25 mmHg (LOR25) was found to be the most discriminating measurement between the two groups. The area under the receiver operating characteristic curve was 0.973 (95% confidence interval [CI] 0.923–1.000). The range of LOR25in limbs without obstruction was 0.0043–0.038 mmHg/ml/min and 0.0170–0.330 mmHg/ml/min in limbs with obstruction. By plotting VFI against LOR25, a subgroup of limbs was identified with iliac obstruction that had a high VCSS (5–12) and a near-normal LOR25(0.050 mmHg/ml/min) presumably as a result of a well-developed collateral circulation but a high VFI in the range of 5–14 ml/s. Another subgroup of limbs with iliac obstruction and a high VCSS (5–18) had a high LOR25 (0.100–0.330 mmHg/ml/min) presumably from a poorly developed collateral circulation. CONCLUSION: The noninvasive measurement of LOR25provides a quantitative estimation of overall lower LOR. It can indicate which limbs are compensated by the development of a good collateral circulation and which are not. The combination of LOR25with VFI enables the clinician to determine the relative contribution of reflux and obstruction in individual limbs. A low LOR25in the presence of severe iliac stenosis or occlusion is an indication of a well-developed collateral circulation and suggests that stenting would provide little benefit if any. However, this hypothesis needs to be verified by future prospective studies.","PeriodicalId":34849,"journal":{"name":"Vascular Investigation and Therapy","volume":"5 1","pages":"88 - 94"},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86482393","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

AMI accounts for the highest mortality rate of approximately 15% worldwide. The acute complications of AMI, including cardiac arrhythmias, tolls almost 50% deaths, which may occur on the way to the hospital. The effective treatment modalities for AMI include antianginal medications and reperfusion with thrombolytics (tissue plasminogen activator) or percutaneous coronary intervention (PCI). PCI is the established and widely used reperfusion technique in the management of AMI as thrombolytics are contraindicated in many patients. PCI is superior to thrombolytics with a higher reperfusion rate of approximately 90%, shorter hospital stay, and lowering the mortality rate. The intervention performed within 90 min of hospital arrival improves the clinical outcomes significantly. In this article, major updates in diagnosis and treatment modalities of AMI have been summarized, mainly focusing on the efficacy of PCI in improving the clinical outcomes.","PeriodicalId":34849,"journal":{"name":"Vascular Investigation and Therapy","volume":"50 1","pages":"98 - 104"},"PeriodicalIF":0.0,"publicationDate":"2019-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78381707","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Although clinical useful, the use of these drugs has been associated with bleeding complications of varying magnitude. Until recently, there was no antidote available for the control of bleeding associated with DOAC's. Such traditional approaches as the use of blood products and mechanical methods have been used to control bleeding in past. Molecular approaches have resulted in the development of specific antidotes for the oral anti-Xa and anti-IIa drugs. Andexanet alfa is a molecularly modified recombinant human factor Xa which is developed as an antidote for rivaroxaban and apixaban. Praxbind is an antibody which neutralizes dabigatran. Both agents have approved by the FDA and the European Medicine Agency, while these two agents are valuable in the management of bleeding with DOAC's, their use is associated with various adverse responses. Since both agents are proteins, antibody generation and interactions with endogenous proteins may also contribute to some of the observed adverse effects. The clinical data on their use is rather limited and additional studies are warranted to optimize their use. A global antidote, Ciraparantag, (PER977) is also developed for the neutralization of DOAC's. Both the activated and non-activated forms of prothrombin complexes are reportedly effective as an antidote for DOAC's. Additional studies are needed to develop guidelines for the safe use of antidotes to minimize the observed complications with the use of antidotes.","PeriodicalId":34849,"journal":{"name":"Vascular Investigation and Therapy","volume":"22 1","pages":"78 - 81"},"PeriodicalIF":0.0,"publicationDate":"2019-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81984406","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
