{"title":"Acute Renal Failure and Thrombocytopenia To a 36-Year -Old Woman: Case Report","authors":"M. Stoian","doi":"10.31579/2690-4861/068","DOIUrl":"https://doi.org/10.31579/2690-4861/068","url":null,"abstract":"Background: Antiphospholipid syndrome (APS) is an acquired, immune-mediated thrombophilia occurring alone (primary APS, PAPS) or in association with other autoimmune diseases, mainly systemic lupus erythematous (SLE), (secondary APS), characterized by recurrent venous or arterial thrombosis and /or pregnancy morbidity in association with antiphospholipid antibodies (aPL) and/or lupus anticoagulant (LA). Case report: A 36-year-old woman was admitted to the hospital because of acute renal failure and thrombocytopenia. This woman presented with an acute illness characterized by a prodrome of respiratory symptoms and fever that were unresponsive to antibiotic therapy, followed by the progressive involvement of multiple organs. There was enzymatic and functional evidence of myocardial necrosis leading to intermittent, severe heart failure, as well as acute renal failure requiring dialysis, laboratory evidence of pancreatic injury, pulmonary infiltrates with respiratory failure, and central nervous system involvement, with confusion. After a renal biopsy was detected a perinephric hematoma and thrombocytopenia. The laboratory criteria for the diagnosis of the antiphospholipid-antibody syndrome are an elevated value for IgG anticardiolipin antibody, a positive test for lupus anticoagulant, and an elevated value for IgM anticardiolipin antibody. Conclusions: This patient has the antiphospholipid antibody syndrome, with an acute thrombotic angiopathy that caused ischemic damage in the myocardium, pancreas, kidneys and lungs. {"title":"Hydroxychloroquine and risk of cardiac decompensation","authors":"L. HCQ has also been suggested as a treatment in patients suffering from severe acute respiratory syndrome–coronavirus 2 (SARS–CoV-2). One of the cardiac complications of SARS-CoV-2 is myocarditis and ventricular dysfunction. Case summary: We present the case of a 52 year old lady presenting with 2 months history of breathlessness, found to have severely impaired left ventricular function. She had been taking HCQ for 28 months for seronegative inflammatory arthritis. No cause was identified on initial investigation. She was treated with optimal medical therapy, and HCQ was stopped. After 5 months a cardiac MRI scan revealed full remodelling of the ventricle. Discussion: This case highlights one of the lesser recognized side effects of HCQ, and the potential for severe cardiac dysfunction. The patient’s stomatognathic system should be examined preventatively prior to the initiation of antiresorptive drugs in order to avoid pathological buccal manifestations, following the same healthcare clinical protocols used for patients receiving head and neck radiotherapy. Additionally, patients should be informed of the precautions to be taken, including regular dental appointments for oral health assessment. The risk of developing MRONJ should be evaluated according to the type of antiresorptives or antiangiogenic drugs administered and treatment duration. In the case of MRONJ, its fundamental characteristic is positioned in the biochemical particularity of the pharmacokinetic expression of antiresorptive drugs, reversibly (DS) or irreversibly (BPs) inhibiting the functionality of the osteoclast. Therefore, the consideration of invading bone tissue as little as possible and performing resective therapies in cases of systemic infectious spread follows, since its long-term resolution would not be effective because the drug (BPs) has frank accumulation at a distance, a characteristic used by treating doctors and it would not have clinical relevance to suggest its suspension. According to the recommendations of AAOMS; Task Force and AOCMF coincide with the sharing of consensus on minimally invasive manipulations once the necrotic foci have been installed and the preventive attitude prevails of eliminating all septic foci prophylactically before starting therapy with antiresorptive drugs. Three most common and obvious symptoms in patients with PD are tremor, rigidity, and bradykinesia. A multidisciplinary team involving neurologists, primary care practitioners, nurses, physical therapists, social workers is used to diagnose PD. Nonpharmacological and pharmacological treatment is given to the patient. {"title":"Acute Obstructive Abdomen Secondary to Intestinal Lipoma Intussusception","authors":"I. Although rare in adults, it corresponds to 1% of cases involving intestinal obstruction, being more common in the small intestine. The objective of this study is to report a case of intestinal intussusception secondary to lipoma. Case Report: female patient 51 years was admitted to the emergency room with a colicky abdominal pain for 1 month, no irradiation, and with worsening for 2 days, associated with nausea, vomiting and hematochezia. On physical examination, distended and hypertympanic abdomen with pain to palpation and rectal touch with melena was observed. Acute abdominal X-ray demonstrated evidence of colonic loop dilation and hydro-air level. Computed tomography examination of abdomen and pelvis demonstrated a target lesion suggesting intussusception, confirmed in the intraoperative by invagination from the terminal ileum to the proximal descending colon with subtotal colectomy and terminal ileostomy. Discussion: Intussusception in adults usually occurs secondary to a point of advance, either by benign tumors, malignant or motility disorders. In the large intestine, lipoma is considered a rare cause of invagination, and malignant tumors are more common, and its symptomatology and risks of complications are mainly related to the size of the lesion.
