Inflammation caused by helicobacter pylori differs depending on the virulence factors, density and host tissue response of the bacteria. This study is intended to investigate the relation between density of helicobacter pylori colonization in gastric mucosa in biopsy specimens and gastric mucosal inflammation severity. Materials and methods: Our study included 685 patients who applied to the hospital with dyspeptic complaints and had gastroscopy between the dates of January the 1st 2017 and January the 1st 2018. Histopathologic results of endoscopic biopsy specimens of these patients were retrospectively screened. Inflammatory severity and helicobacter pylori intensity were assessed by Sydney scoring. Results: 68.6% of the patients were infected with Helicobacter pylori. As the intensity of inflammation increased, the intensity of Helicobacter pylori increased as well. The relation between the intensity of Helicobacter pylori and the severity of inflammation was statistically significant (p < 0.001). Skin diseases can significantly impact not only a patient’s physical appearance, but also their social and emotional well-being. Even the smallest skin lesions have been shown to disrupt a person’s level of overall wellness. Here, we aim to address the potential psychological and emotional impact of the most common and debilitating dermatologic conditions. We describe how skin disorders affect individuals and subsequently, how they are perceived by society. In addition, we review several dermatologic conditions that are not only skin-related, but are also linked to disorders of internal organs. In many cases, the diagnosis can be made promptly, but patients with rare, complex, or unusual conditions often embark on odysseys spanning years to sometimes even decades in search of a diagnosis. This prolonged process can often result in excessive costs, preventable medical errors and iatrogenesis, as well as feelings of frustration, abandonment and isolation. To address these issues and facilitate discussion, a shared conceptual framework and vocabulary are needed. Here we propose a framework that partitions a patient’s diagnostic journey into three distinct phases related to navigation within the healthcare system: The primary encounter, an initial round of referrals, and subsequent evaluations and second opinions. Most of the times it is diagnosed incidentally. The majority of patients present flank pain, right and/or left iliac fossa pain with few other symptoms. These findings lead usually the physician toward a surgical disease. Imaging is very important to confirm the diagnosis of appendagitis and to avoid unnecessary hospitalizations and surgeries. In this article, we present five cases of acute appendagitis that have had nearly similar symptoms at presentation and while we were suspecting more severe diseases on their admission at emergency department (ER). {"title":"Clinical Outcomes of Parathyroidectomy in Resistant Renal Hyperparathyroidism","authors":"Pekkolay Zafer, A. However, some patients have medical treatment resistance. The definition of resistant renal hyperparathyroidism despite intake of calcimimetic, parathormon binding and vitamin D analogs persistent parathormone elevation and related symptoms. The only treatment for resistant renal hyperparathyroidism is surgery. This study was designed to investigate the indications for operation of patients with parathyroidectomy due to resistant renal hyperparathyroidism. Materials and methods: Nineteen patients over 18 years of age who underwent hemodialysis due to end stage renal failure and who underwent parathyroidectomy due to secondary and tertiary hyperparathyroidism were included in the study. Patients preand post-operative and most recent calcium, phosphorus, parathormon values, operative patterns, pathology reports were recorded. Clinical results of parathyroidectomy were discussed. Results: The study was designed with 19 patients. Eleven of the patients were female (57.9%), 8 were male (42.1%) and the mean age was 44 ± 19 (18-91) years. Preoperative parathormon: 1811 ± 901 (436-3513) pg/ml, 3 patients (15.8%) tertiary hyperparathyroidism; Sixteen patients (84.2%) were diagnosed with secondary hyperparathyroidism. Subtotal parathyroidectomy in 11 patients (57.9%), and recurrent disease in 3 patients (15.8%). Minimally invasive surgery was performed in 5 patients (26.3%). Three patients had persistent hypoparathyroidism (15.8%). The most frequent operation indication was a very high parathormon value (18/19: 94.7%). Surgical success rate was 68.4%. In patients, the most common symptom was bone and muscle pain (84.2%). It is the 4th most common sarcoma of the skin and typically occurs on the scalp of white men over 60 years of age. Its prognosis is traditionally very poor, with 5-year survival rates reported as low as 11%. Risk factors for cutaneous angiosarcoma have not been clearly established. Herein, we describe a 64-yearold male with a chronic history of psoriasis maintained on methotrexate and infliximab for over 10 years, who presented with a 7-week history of a cluster of reddish-blue, violaceous nodules on his occipital scalp. Biopsy showed classic features of angiosarcoma. He was treated with multimodal therapy.
