Objective: To identify the clinical predictors of hospital outcomes in COVID-19 patients and to evaluate the effect of therapeutic’s interventions on length of stay, ICU admission, need for mechanical ventilation (MV) and mortality. Materials and methods: The primary outcome was to assess whether any treatment alone or in combination with standard care, was able to reduce ICU admission, need for MV or mortality. The secondary end points were 1) To know if any single or combined treatment was able to reduce the length of stay in the hospital, MV and ICU, 2) Whether some form of oxygen support prevented MV and 3) Which clinical predictors are relevant for worse outcome. Results: AZM-Corticosteroid and therapeutic anticoagulation when indicated reduced the ICU stay in 1.5 day and MV in 4 days, but when the same combination included HCQ this time increased. For the AZM-Corticosteroid and therapeutic anticoagulation combination the mean ICU length stay was 15.9 days, however when HCQ was present, again the mean ICU time was 40.3 days. The clinical predictors at admission for death were: Age > 65 years, presence of up one comorbidity, pulmonary involvement more than 50%, saturation < 93%, lymphocytes < 900 mm3, D-dimers > 1.250 ng/mL and C-reactive protein (CRP) > 8.0 mg/dL. Conclusion: AZM-Corticosteroids and therapeutic anticoagulation, when indicated, represented a favorable combination for inpatients with COVID-19.Some clinical predictors at admission may help to estimate a higher risk of poor evolution.","PeriodicalId":346614,"journal":{"name":"International Archives of Internal Medicine","volume":"107 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129734304","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, perianal fistulas and fissure can occur with or without active inflammatory bowel disease, occurrence of which prior to the development of ulcerative colitis is a rare occasion. Case presentation: Here we report a case of ulcerative colitis, which was misdiagnosed, treated as a perianal fissure and anal pressure ulcer without recognizing inflammatory bowel disease. Conclusion: In case of anal fistulas and fissures a multidisciplinary approach should be considered, including both surgical and internal medicine professionals.","PeriodicalId":346614,"journal":{"name":"International Archives of Internal Medicine","volume":"2014 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-09-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128586985","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The virus can cause systematic inflammation that can lead to severe cardiac complications like uncontrolled atrial fibrillation (AF), which carries high mortality among patients. We present a case of an 82-year-old female from nursing home with advanced dementia and multiple comorbidities. She initially screened negative for COVID-19 in the nursing home, however her repeat test was positive. Soon after admission she developed progressive, hypoxic respiratory failure and was subsequently diagnosed with ARDS and required endotracheal intubation. The patient was found to have uncontrolled AF on telemetry monitoring. Initially, intravenous calcium channel blockers were unsuccessful in controlling her heart rate, thus this was later changed to intravenous amiodarone. The patient was successfully extubated on day 15 and transferred to the general medical floor on antiarrhythmic medication for AF.","PeriodicalId":346614,"journal":{"name":"International Archives of Internal Medicine","volume":"5 1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-05-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127831327","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We report a case of vulvar sebaceous carcinoma in a 64-year-old woman that presented as an asymptomatic nodule and was successfully treated by wide local excision. It involves granulomatous deposits in multiple organs, leading to a wide variety of manifestations such as bone lesions, pulmonary nodules, pituitary lesions, and skin lesions. With mucocutaneous manifestation, it is often diagnosed in childhood, typically making LCH a childhood disease. However, due to the involvement of multiple organ systems and variable clinical courses, if not diagnosed during childhood, the patients with LCH often suffer from delayed diagnosis and treatments as adults. choroid glioma, and metastatic lesion.
