The physicians need to be supplemented with new information using various unbiased and reliable drug information (DI) sources which will promote rationale use of medicines. This study aims to understand the commonly used sources of DI by prescribers at our institute, their usefulness and the need for an independent drug information unit at the institute.\u0000Methods: A cross-sectional descriptive study that included all prescribers presently working at this institute and actively involved in patient care was conducted. Consenting participants were requested to fill in the self-administered questionnaire. Data thus collected were entered using EpiData version 3.1 and were analysed using SPSS version 18.\u0000Results: Filled-in questionnaires were obtained from 147 prescribers. Almost all of the participants (95.80%) used textbooks and Online Medical Sites (OMS) as sources of DI. Among participants using OMS for DI, 80 (58.39%) classified them as sometimes biased. Less than half (41.25%) agreed that they will absolutely be benefitted from having unbiased DI services at the hospital. The participants most commonly (136, 92.50%) had queries related to dosage / administration on a daily basis.\u0000Conclusions: Most prescribers relied on textbooks and OMS for DI which in their opinion had some level of biasness associated with them. Women’s autonomy is seen as an important factor in utilization of maternal health services. Therefore, the objective of study was to find the level of autonomy and utilization of antenatal services among the women of reproductive age group.\u0000Methods: A descriptive cross-sectional study design was used. A total of 151 women of reproductive age group having at least one child in the age group of less than one year were selected using non probability purposive sampling technique from four randomly selected wards of Godawari Municipality. A validated and pretested Nepali version interview schedule was used to collect data from the respondents through door-to-door visit. Descriptive statistics such as frequency, percentage, mean, standard deviation and inferential statistics i.e. Fisher’s exact test was used to analyze data.\u0000Results: The study findings revealed that the majority (58.3%) of respondents had a lower level of overall autonomy. The highest autonomy was seen in decision making and lowest in financial related issues. Almost all (96%) of the respondents had fully utilized antenatal services. There was significant association between antenatal services utilization with women’s autonomy (p-value 0.041). Education of women and their husbands (p-value 0.009 and 0.013 respectively) as well as easy access to health facilities and availability of private transportation (p-value 0.005 and 0.039 respectively) were significantly associated with utilization of antenatal services.\u0000Conclusions: This study concludes that women’ autonomy tends to influence their antenatal services utilization. Likewise, education of women as well as their husbands tends to facilitate utilization of antenatal services. There are many factors like temperature, pH, lifespan of protein, substrate concentrations and hemoglobin levels that influence HbA1c estimation by any methods. The aim of the study is to estimate HbA1c by High Performance Liquid Chromatography (HPLC) and immunochromatography analyzer and find the influence of hemoglobin in HbA1c by both the methods.\u0000Methods: This was a descriptive, cross-sectional study carried out in the department of Biochemistry of Shree Birendra Hospital, Chhauni, Kathmandu over a period of two months from May 2021 to June 2021. We selected patients with type 2 diabetes mellitus. HbA1c was measured in EDTA blood samples by Bio-Rad D10 (HPLC method) and Nycocard (Immunochromatography method) whereas hemoglobin was estimated using Horiba Penta XLR fully automated hematology analyzer.\u0000Results: In our study of 100 known cases with type 2 diabetes mellitus, the mean hemoglobin level was 14 ± 2.09 g/dL. The mean HbA1c value obtained by Nycocard and HPLC analyzer were 7 ± 1.95 % and 7.39 ± 2.21% respectively. We found that there was a strong correlation of HbA1c values with hemoglobin levels in HPLC method p = 0.001, r = 0.6. However, we found no such correlation by Nycocard method p = 0.6, r = 0.08.\u0000Conclusion: There was a positive correlation of hemoglobin with HbA1c values obtained by HPLC method however no such variation was seen with immunochromatography method. There is limited data on normal dimensions of adrenal gland in Nepalese population. Our study aimed to find out the normal adrenal gland thickness measurement.\u0000Methods: It is a descriptive cross-sectional study conducted in a tertiary referral hospital over a period of one year. Ethical approval was taken from the institutional review committee. All the patients referred to Radiology Department for abdominal Computed Tomography evaluation unrelated to adrenal pathology were included. We measured maximum thickness of adrenal limb and body along with diaphragmatic crus on either side.\u0000Results: Mean adrenal gland thickness of right medial limb, right lateral limb and body was 3.57 ± 0.87 mm, 3.33 ± 0.79 mm, 5.69 ± 1.19 mm respectively. Similarly, mean adrenal gland thickness of left medial limb, left lateral limb and body was 4.16 ± 0.9 mm, 3.84 ± 0.85 mm, 6.07 ± 1.35 mm respectively. On comparison of medial and lateral limbs to ipsilateral diaphragmatic crus thickness, 298 (99.3%) and 296 (98.6 %) cases had ratio less than one on right side and 280 (93.3%) and 268 (89.3 %) cases had ratio less than one on left side.\u0000Conclusions: Our study has established the reference measurement of normal adrenal gland thickness in Nepalese adult population. At least two Humphrey Visual Field(HVF) assessment two weeks apart was performed. Glaucomatous visual field defect and its severity was diagnosed on the basis of Hodapp-Parrish-Anderson criteria. The relationship between visual field defect and IOP were analyzed.\u0000Results: Superior arcuate scotoma was most common visual field defect seen in 18(19.1%) eyes.  56 eyes (59.6%) had mild POAG (MD of <-6dB) with no cases having double arcuate scotoma whereas 1 eye (16.0%) with moderate POAG (MD of -6 to -12dB) had 1 double arcuate scotoma and 23 eyes (24.5%) with severe POAG (MD of >-12dB) had 14 double arcuate scotoma. The mean IOP in mild POAG was 24.29 mmHg (range 21-30); 26.40mmHg (range 22-31) in moderate POAG and 29.70 mmHg (range 22-36) in severe POAG. Significant relation was demonstrated between elevated IOP at presentation and higher negative values of MD (p<0.001).\u0000Conclusions: Higher IOP at presentation is associated with severity of POAG. The main objective of the study was to find out the health-related quality of life of patients with rheumatoid arthritis in a selected rheumatic Centre, Lalitpur.\u0000Method: A Descriptive cross-sectional design was adopted among 115 rheumatoid arthritis patients through non-probability purposive sampling. Data was collected through face to face interview taking written informed consent. Data was collected from 2nd September to 28th September 2018. Data were analyzed through descriptive and inferential statistics using SPSS version 16.\u0000Result: In this study, the mean score of the overall health-related quality of life was 55.84±13.43. The mean physical component score was 55.96±13.61 and the mental component score was 45.42±13.84. Regarding association with the physical quality of life, age (p=0.008) and type of family (p=0.04) which were significantly associated. Regarding association with mental quality of life, age (p=0.02), marital status (p=0.03) and BMI (p= 0.001) which were significantly associated with mental quality of life.\u0000Conclusion: The overall health-related quality of life of patients with Rheumatoid Arthritis was average whereas mental health domain score was below average. Various factors can lead to poor sleep quality in a patient during hospitalization. Therefore, the purpose of this study was to assess the prevalence of sleep deprivation and its associated factors among the hospitalized patients.\u0000Methods: This study was done using descriptive survey design. A total of 65 hospitalized patients were selected for the study using non-probability purposive sampling technique. Informed written consent was taken from each respondent beforehand.  Data was collected by interviewing the patients meeting the criteria with pretested semi-structured interview schedule at their bedside by maintaining privacy through bedside curtains. Collected data were analyzed by using descriptive statistics and inferential statistics i.e., chi-square test and Fisher’s exact test.\u0000Results: Almost all (98.8%) of respondents had sleep deprivation on the first night of hospitalization which decreased in frequency with increased length of hospital stay. Majority (73.8%) of respondents expressed that hospital environment was disturbing to sleep. Hospital noise (95.8%) and light (66.6%) were reported as the most disturbing factors. Sleep deprivation was associated with hospital environment (p=0.036, p=0.002, p=0.002) on first, second, and third day of hospitalization. This study revealed no significant association of sleep deprivation with age and pain or discomfort of respondents.\u0000Conclusions: Hospital environment is the most important factor of sleep deprivation among hospitalized patients, particularly noise and light. Unhealthy diet, low intake of fruits and vegetables, physical inactivity, harmful use of alcohol and tobacco, overweight or obesity, family history of hypertension are the risk factors for hypertension. This study aims to identify the prevalence and risk factors of hypertension among Military personnel of Nepal Army.\u0000Methods: Cross-sectional study was conducted among 416 serving army personnel posted at four selected military barracks; Dhading, Birgunj, Lamjung and Tanahun with the data collection period from January to June 2020. The civilian population working in Army units, retired army personnel and dependents were excluded from the study. Systematic random sampling was used for the selection of participants. Pre-tested questionnaires, weighing machine, stadiometer, measuring tape, stethoscope and BP set were used as a tool for data collection. Data was entered and analyzed using SPSS 20.0.\u0000Results: Participants had a mean age of 34.46 ± 5.25 years. Prevalence of hypertension and pre-hypertension was 13.4% and 37.3% respectively. Among the risk factors studied for hypertension; spouse’s education, tobacco chewing, history of hypertension in father and being overweight, were found to be statistically significant with p value of 0.047, 0.000, 0.021, and 0.002 respectively.\u0000Conclusions: Prevalence of hypertension was less in military personnel compared to other occupational groups. Unlike in other malignancies in which cancer tends to be aggressive in young individuals, the probability of developing muscle invasive bladder cancer increases with age. The standard treatment for non-metastatic muscle invasive bladder cancer is radical cystectomy. However, there is controversy regarding the outcome of radical cystectomy in elderly patients, especially more than 80 years.\u0000Methods: This is a retrospective study of prospectively collected data. All patients > 80 years of age with bladder carcinoma were included in the study. A patient with good functional status was offered Radical Cystectomy. Patients not fit for surgery or refusing surgery were managed with TURBT.\u0000Results: A total of 23 patients were included in the study. Out of which, 13 patients underwent radical cystectomy (RC) with ileal conduit and while ten patients who refused surgery were managed with TURBT and chemotherapy. The prognosis of patients with MIBC managed with TURBT was within 1 year. While patients who have undergone RC had survived more than 1 year.\u0000Conclusions: Radical Cystectomy is a safe, feasible, and effective treatment option for the carefully selected octogenarian. A  very few studies have compared the effects of various types of laryngoscope blades on hemodynamic response to laryngoscopy and intubation.\u0000Methodology: A prospective randomized study was done to compare the hemodynamic response to using McCoy and Macintosh laryngoscope. A hundred patients, belonging to ASA grade I and II, between 15-65 years, requiring general anesthesia with intubation were included. A standard anesthesia technique was used. Both groups(n=50) were matched demographically. Mallampati grading, laryngoscopy and intubation time, laryngeal visualization grades, and hemodynamic variables at baseline were comparable.\u0000Result: Following laryngoscopy there was significant rise in heart rate, systolic, diastolic and mean arterial pressure. The maximum change in HR compared to baseline was 20.45±9.29 vs 12.36± 7.28 (p<0.001) in Macintosh and McCoy groups. As compared to baseline maximum change in SBP (mean) was 7.92±10.53 vs 2.80± 6.73 (p= 0.005), the maximum  change in DBP (mean) was 9.28±14.74 vs 5.72±7.37 (p= 0.130), the maximum percentage change in MAP (mean) was 8.62±12.07 vs 4.36±7.83 (p=0.039) in Macintosh and McCoy group respectively. Compared to variables just before insertion of the laryngoscope, maximum percentage rise in mean HR was 22.74±10.88 vs 16.40±7.43 (p=0.001), maximum percentage rise in SBP (mean) was 28.31± 13.22 vs 19.41±6.82 (p<0.001), maximum percentage rise in DBP (mean) was 30.00±15.25 vs 24.64±12.21 (p=0.003), maximum percentage rise in MAP (mean) was 28.89±11.55 vs 22.31±11.34 (p=0.05) in Macintosh and McCoy group respectively.\u0000Conclusion: The hemodynamic response to laryngoscopy and intubation with McCoy laryngoscope was significantly less than with Macintosh laryngoscope.","PeriodicalId":33963,"journal":{"name":"Medical Journal of Shree Birendra Hospital","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-07-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44760839","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
