The traditional safety measures used in psychiatry are still in force but challenged by a trend to avoid all involuntary action, in respect of patient autonomy. There is an urgent need to develop strategies to cope with this situation, which is especially difficult to cope with in case of first encounters with dual diagnosis patients in psychiatry. The risk for open aggressiveness is difficult to assess and to manage in first encounters, avoiding an escalation and working a deescalation strategy. The strategy described in this paper is based on a distinction of the different types of first encounter and on general rules. Cognitive flexibility, learning, memory, attention and reaction time impairment can be seen in chronic cocaine users. The first was to analyze the differences between incarcerated and non-incarcerated individuals regarding their scores on specific Rorschach variables that are associated with emotional intelligence (EI). The second was to investigate the differences in scores on Rorschach variables reflecting emotional responsiveness (CF+C, MC-PPD) and emotional management (CF+C/SumC, M-, H, and PHR) among different levels of intellectual ability for both the incarcerated and outpatient samples. The study involved an examination of archival data from incarcerated individuals receiving mental health services and individuals receiving mental health services at a community clinic. Findings indicated there was a significant difference in CF+C/SumC scores between the different levels of care and incarceration status. These results indicate those who are not incarcerated likely have more adaptive emotion management skills to manage internal and external emotionally toned stimuli in comparison to those who are incarcerated, particularly incarcerated individuals who need the highest level of mental health care. Further, the results indicated participants’ responses on the emotional management/human interaction variables (i.e., CF+C/SumC and PHR) were able to significantly classify their incarceration status, with an overall correct classification of 63.3%. Future research is needed to better understand the relationships between incarceration status and EI abilities. {"title":"Aberrant Prescription Medication Seeking Behavior Used by Prescription Drug Users to Obtain Prescription Medications from Physicians: A Qualitative Review of Webpages, Blogs and Forums","authors":"S. Individuals who suffer from substance dependence utilize a variety of aberrant behaviors to obtain prescription medications. They share their methods and experiences with others in social media and online platforms and educate each other on how to manipulate the providers, so that they can successfully obtain their medications of choice. Objectives: The main objective of this qualitative study was to review the content of webpages, blogs, and forums to explore the most common approaches shared by individuals with prescription medication dependence to obtain such medications. Methods: We used a combination of keywords to search five main search engines: Google, Yahoo, Bing, MSN and AOL. Our search strategy focused on two categories of medications: opioid painkillers and stimulants. The following keywords were used for the purpose of this search: Tylenol 3, Tylenol #3, T3, Morphine, Hydrompohone, Dilaudid, Dexedrine, Ritalin/Adderall, and Pain Medications. Results: Qualitative analysis of the verbatim revealed 5 themes and 34 sub-themes. Over-reporting the side effects of undesired alternatives, under-reporting of the therapeutic effects of the medication of choice, knowing the symptoms of the illness in advance of a visit, and threatening doctors about making complaints against them were among the most common aberrant behaviors recommended online. Conclusion: Individuals with prescription medication dependence use a variety of aberrant medication seeking behaviors to obtain their medications of choice. They use online discussion to share these approaches with other patients. {"title":"Alcohol as a Coping Mechanism for Social Anxiety","authors":"Amanda Beccaria","doi":"10.33552/oajap.2019.03.000554","DOIUrl":"https://doi.org/10.33552/oajap.2019.03.000554","url":null,"abstract":"Alcohol abuse has become a significant issue on college campuses, particularly heavy episodic drinking commonly known as binge drinking. Various factors have been studied in relation to the causal and subsequent factors of alcohol abuse on college campuses, including motives, contexts, and psychological conditions related to college students’ use of alcohol. One common impetus for college students to use alcohol is to deal with anxiety. One group of students who are particularly susceptible to experience anxiety are those who suffer from social anxiety. However, research lacks focus on the potential impact of social anxiety as a motivator for drinking behaviors. There were several significant findings related to Social Anxiety scores. A major finding was that the strongest relationship found was that students suffering from social anxiety utilize alcohol in the context of coping with their negative emotions. This is a new and important detail in understanding the ways in which those suffering from social anxiety try to cope with their negative emotions and their vulnerability to substance abuse. Significant gender differences also were obtained that indicated female students were more likely to drink to cope with negative emotions. In addition, findings indicated female students living off-campus reported higher levels of social anxiety than females living on-campus, and vice versa for males students. However, analyses do not permit tying these gender differences to differences in use of alcohol. Finally, there were differences between the women and men depending upon their employment status (i.e., full-time, part-time, unemployed). Trends in the analyses provided for implications of clinicians and offer direction in the construction of future research about social anxiety and alcohol use by college students. They occur preferably in the first minutes upon waking. They are triggered by sleep deprivation and alcohol intake. The disease follows a non-progressive course Clinical Case: A 15-year-old female patient presented to the service due to an absence crisis and an apparent insomnia problem. The semiology of absence crisis is interrogated and in effect, as well as occasional nocturnal myoclonus, electroencephalogram is performed and a 4 Hz slow wave poly tip pattern is found throughout the stroke, especially in photo stimulation (Figure 1, 2).
