This study was done to evaluate the utility of POCUS in RSI and its efficacy in confirming tube placement as compared to conventional methods. Aims & Objectives: Primary Objectives - Time taken for airway evaluation and identifying potentially fatal conditions using POCUS guided Intubation Algorithm (PA) during RSI. Proper placement of the endotracheal tube and confirmation of the same was also assessed and compared with the conventional technique. Methodology: A prospective, randomized single-centred study was conducted in 100 trauma patients requiring emergent airway management, presenting to the ED. The time taken and efficacy of confirmation of tube placement is recorded and compared in two arms. Results: In our study we found the mean procedure time for ETT placement was less in the PA arm compared to the CE arm (45 vs 91.36 seconds, p<0.0001). Oesophageal intubations were detected in the PA arm in 22 seconds vs 114 seconds in CE arm with a p< 0.0001 and the time taken for effective reintubation was 26.67 vs 55 seconds in the PA and CE arm with a p value < 0.007 respectively. Conclusion: POCUS detected oesophageal intubations and confirmed proper tube position in less time when compared to standard techniques such as five-point auscultation and capnography. The results of this study have proved that POCUS is a useful adjunct to the Emergency physician to improve quality of care in trauma patients. Treatment of these infections remained as an area of concern. Both fluroquinolones and cephalosporins are most common choice of antibiotics. Despite Cephalosporins, being drug of choice they are expensive also showed many adverse reactions. This study, compares and reevaluates the susceptibility of gram negative bacteria to fluroquinolones (ciprofloxacin) compared to cephalosporins. Method: Various samples(pus, sputum, urine, blood and bodyfluids) were processed according to standard protocols. Antibiotic done susceptibility by using Kirby-baur disc diffusion method. ESBL and Amp C producers were identified using CLSI guidelines. Result: Among 400 isolates, majority were from pus followed by urine, sputum. The most common organism isolated was Klebsiella spp, (33.25%) Escherichia coli (29.5%), Pseudomonas spp (27.25%), Enterobacter spp (6.25%), Citrobacter 5 (1.25%), and Acinetobacter spp (2.5%). Isolates showed 20-80% susceptibility to ciprofloxacin, 30-60% to third and fourth generation cephalosporins. Klebsiella and Pseudomonas showed 64% and 31% susceptibility to ciprofloxacin. Acinetobacter spp showed 30% susceptibility to cefipime and 20% to ciprofloxacin. 34 isolates were ESBL 18 were AmpC producers, of which 15(44%) ESBL and 7(38%) of AmpC producers were ciprofloxacin susceptible. Conclusion: Ciprofloxacin was found to be more effective than the fourth generation cephalosporin (cefepime) against gram negative bacilli. Ciprofloxacin can be considered for treatment as it is more active and cost effective when compared to cephalosporins. Sepsis is characterized by non-specific symptoms and clinical diagnosis remains difficult. C Reactive protein (CRP) and procalcitonin (PCT) are now days used as surrogate markers of sepsis. Aim: To determine PCT levels in patients with suspected sepsis and its correlation with CRP and total leucocyte count (TLC). Material & Methods: The retrospective study was conducted in tertiary care hospital in India. Adults patients (> 18 years) admitted to Intensive care unit in the hospital during study period, fulfilling the ACCP criterion for the diagnosis of sepsis were included in the study. Data was collected using laboratory software maintaining confidentiality. Results: The study comprised of 190 patients. Patients were divided into four groups based on PCT values and severity of sepsis. Out of 190 patients, PCT was positive in 135 patients. The CRP and TLC levels were distributed into the four PCT groups and their correlation with PCT was studied. For PCT values between 2-10ng/ml, >10ng/ml CRP showed statistically significant correlation with p value <0.05, whereas correlation of TLC with PCT was poor. Conclusion Procalcitonin along with CRP is a better diagnostic tool for sepsis but their correlation in context to TLC was poor. Due to lack of an ideal isolated marker, our study mandates multidisciplinary approach for diagnosis of sepsis. Health of infants is considered as a sensitive indicator of health status and level of socio-economic development of a country. In India, the infant morbidity and mortality are in decline, but the pace of decline is not sufficient to attain the target goals of National Health Mission. Objective: To assess the factors influencing infant morbidity in the urban field practice area of a medical college in Bangalore. Methods and Material: This was a population based cross sectional study done at an urban poor locality in Bangalore. The study was conducted between April 2018 to September 2019 with a sample size of 300 after obtaining the approval from Institutional ethics committee. Population proportion to size was used to ensure equal representation. Data was collected using pre tested semi structured questionnaire & analysed using open epi like descriptive statistics with univariate & multi variate logistic regression were used. Results: Total of 165(55%) subjects were females, with majority 206(68.7%) Muslim by religion and 161(53.7%) lived in the nuclear family. The prevalence of morbidities among infants was 209(69.7%). The most common infant morbidities reported were 121(40.3%) ARI, 85(28.3%) fever and 45(15%) diarrhea. Infants with perinatal complications, faulty feeding practices like delayed initiation of breast feeding, bottle feeding and immunization had significant association with infant morbidities. Conclusions: To conclude there is a statistically significant association between perinatal complications, bottle feeding & partial immunization with infant morbidity. Although the pathological diagnosis of lung carcinoma is limited as only small specimen available for diagnosis, the availability of targeted therapies has created a need for precise subtyping of non-small cell lung carcinoma. Several recent studies have demonstrated that the use of immunohistochemical markers can be helpful in differentiating squamous cell carcinoma from adenocarcinoma not only on surgically resected specimen but also on small biopsy samples. Material and Methods: A cross-sectional study of one year duration including 50 cases of lung carcinomas on guided biopsies were first reported on Haematoxylin and Eosin sections and later subjected for IHC using relevant markers TTF-1 and p40. Results: In our study IHC with TTF-1 and p40 aided in subtyping of 35 (92.1%) cases of non-small cell lung carcinoma and this diagnostic accuracy was found to be statistically significant with p value <0.001. On statistical analysis, p40 showed 100% sensitivity and 85.7% specificity for squamous differentiation whereas TTF-1 showed sensitivity of 85.7% and specificity of 100% for adenocarcinoma. Out of 50 cases, after IHC, 29 (58%) were diagnosed as squamous cell carcinoma, 18 (36%) as adenocarcinoma, 3 (6%) as non-small cell lung carcinoma. Conclusion: The minimalist IHC based model of p40 and TTF-1 on biopsy samples were effective to correctly subtype most cases of non-small cell lung carcinoma and contribute in sparing material for molecular testing. Aims: To evaluate the usefulness of Tru cut biopsy for various tissue diagnosis and to analyse its sensitivity, specificity, positive and negative predictive values and the accuracy. Settings and Design: The present study is a prospective and retrospective observational study for patients undergoing Tru cut or excisional biopsy. Methods and Material: The present study was undertaken at the Department of Histopathology, Central Diagnostic Laboratory, Pramukhswami medical college and Shree Krishna Hospital, Karamsad, Anand for a period from April 2014 to March 2021 that included 497 samples of Tru cut biopsy. Data was retrieved from the Laboratory Information System of the hospital according to the proforma. Statistical analysis: Sensitivity, Specificity, Positive Predictive Value, Negative Predictive Value and Accuracy were found. Results: Of the 497 Tru cut biopsy samples, 105 excision biopsy were received. For the remaining 392 samples, descriptive analysis was done. Mean age of participants in excision biopsy group was 55.2±14.4 years with female participants (n=92, 87.6%) outnumbering males (n=13, 12.4%) and breast being the most common site of biopsy. There were 6 discordant and 99 concordant cases. Sensitivity, Specificity, Positive predictive value, Negative Predictive value and Accuracy were 96.73%, 92.30%, 98.8%, 80% and 96.1% respectively. Conclusions: Thus, for solitary or diffuse tissue lesions, needle biopsies provide precise localization and are much more likely to yield representative samples cost-effectively with a good safety profile. Reaction time provides an indirect index of the central nervous system’s integrity and processing ability. Material and Methods: The 111 healthy subjects (aged 17 -23 years) interviewed to pre-designed proforma regarding socio-demographic profile, “Glazer-Stress Control Lifestyle” questionnaire for personality trait and reaction time recorded by “Audio-Visual reaction time apparatus”. Statistical analysis was done using IBM SPSS Statistics 20, and the P-value <0.05 was considered significant. Results: The mean age of the study participants was 19.21±1.40 years, with a mean body mass index 20.65±4.32 kg/m2. Male subjects (68%) outnumbered the female participants (43%), and more than half of the subjects doing exercise. The nuclear family was the preferred mode of living belonging to class-II socioeconomic status (86%). The association of personality traits with auditory (low and high frequency) and visual (red and green light) reaction time was non-significant. Type-A personality subjects were maximum (13/20th), followed by Type-AB (6/20th) and least of Type-B personality (1/20th). Conclusion: The personality trait has no impact on auditory and visual reaction time. Though maximum participants belong to type A personality, still ambitious, proactive, and competitive nature does not influence the basic neurophysiological characteristics of the human body. Difference in the level of increased inflammatory markers between patients with AFLD and NAFLD is still unclear. Aim & Objectives: The aim of the present study was to compare the inflammatory markers between patients with alcoholic-fatty liver disease (AFLD) and non-alcoholic fatty liver disease (NAFLD) and also to see if any correlation exists between inflammatory markers and liver enzymes. Materials & Methods: This study was conducted on 70 patients with fatty liver disease (35 AFLD and 35 NAFLD) and 35 healthy controls. Venous blood samples were taken and the required blood parameters (total leukocyte count, neutrophil count, lymphocyte count, ESR) and liver function test were determined. Statistical analysis was done by using Student’s t test and Pearson correlation test. Results: The TLC, Neutrophil count, N/L ratio and ESR were significantly higher while the lymphocyte count was significantly lower in patients with fatty liver disease when compared with the controls. The TLC, Neutrophil count, lymphocyte count, Neutrophil-lymphocyte ratio and ESR of patients with AFLD when compared to patients with NAFLD showed no statistically significant differences. There was a significant positive correlation between the value of ALT with TLC in patients with AFLD. Conclusion: Fatty liver disease is associated with increased inflammatory markers. Since increased inflammatory marker in fatty liver disease is indicative of liver injury, due importance should be given to assessment of inflammatory markers in the management of patients with fatty liver disease. Here we present a case of a 28-year-old gentleman, who first presented with a mediastinal mass and was clinically considered lymphoblastic lymphoma. He was diagnosed initially as having T lymphoblastic lymphoma on a mediastinal trucut biopsy. After one month patient was diagnosed with Acute myeloid leukemia on peripheral smear and bone marrow examination and was confirmed with the help of flow cytometry. The biopsy slides from the mediastinal mass were reviewed and based on the smear findings additional immunohistochemical markers were added and diagnosed as Myeloid Sarcoma. Naveen","doi":"10.46347/jmsh.v8i3.22.121","DOIUrl":"https://doi.org/10.46347/jmsh.v8i3.22.121","url":null,"abstract":"Background: Management of foot ulcer is challenging for plastic surgeons owing to inherent sturdiness of the volar skin and also the necessity to give a robust cover to ensure functionality including weight bearing. This study was conducted to examine the results of our technique of single or double ‘V’ advancement flap sans the ‘Y’ closure of the classical ‘VY’ flap, which was found to be technically simple for the management of plantar ulcers in our foot ulcer patients. Methods and Material: 37 patients hospitalized for the management of foot plantar ulcer, who underwent repair with our modified ‘V’ flap advancement without ‘Y’ closure technique, at our institution were included in the study. Demographic data, disease-related parameters, technique related complications data were recorded and analysed. Technique complications or sequelae, if any, were accounted. Descriptive statistics was used to summarize the findings. Results: In our study, 23 men and 14 women were there with their ages ranging from 15 to 87 years (mean = 54.1 years). The diameter of the ulcers ranged from 14 to 26 mm with mean of 19.3 mm. Unilateral V (-Y) advancement flap was used in 26 cases and in 11 cases bilateral V (-Y) advancement flap was used. Complications included wound dehiscence (3) and partial flap loss (1) of which 3 required redo surgeries while one healed with secondary intention. Conclusions: Though a relatively small study group our technique well suited the needs for plantar ulcer wound closure with acceptable complication rate. Key Messages: Foot ulcer management is a therapeutic challenge which, if managed efficiently reduces encumbrance. Our technique represents a useful and reliable technique with advantages of excellent colour and texture matching and primary donor site closure.
