We report a case of 48-year-old male with chief complain of wound on the left side of mouth with history of hydrochlorothiazide use. On intraoral examination, desquamation was present in palatal gingiva which was tender on palpation. Biopsy of the lesion on histopathology reported oral pemphigoid. On immunofluorescence test linear deposits of C3 and immunoglobulin G was found. Conventional band and loop is still the preferred space maintainer for a unilateral space loss. The present education is still supporting this space maintainer even though it comes with a lot of disadvantages. In this case report, a 7-year-old child who had a grossly decayed lower right primary second molar was extracted, and the Bibin appliance (B-Appliance) was delivered to maintain the space. The present invention B-appliance is a novel space maintainer and prevents supra-eruption and it also overcomes all the disadvantages of band and loop and along with that provides additional advantages. Accessory root formation is one of the rare anomalies of in incisor teeth. A 65-year-old male patient who has type II diabetes and hypertension applied to our clinic. Clinical examination has shown that the upper right central incisor tooth had an 8 mm deep periodontal pocket on the distal and a 7 mm deep periodontal pocket on the mesial aspects of the tooth. Accessory root was seen on the mesiobuccal aspect of root of the upper right lateral incisor tooth in coronal portion. The accessory root was excised with surgical operation and the bone defect in the area was regenerated. Post-surgery periodontal healing was uneventful. Periodontal pocket depth decreased to 4 mm on the 4 th month after surgery. This case discusses accessory root formation may accelerate the accumulation of dental plaques and subgingival calculus. {"title":"Management of teeth with blunderbuss canals and its esthetic rehabilitation","authors":"Nitesh Patil, Ashish R Jain, Deepak Hegde, Amit Patil","doi":"10.15713/ins.ijmdcr.131","DOIUrl":"https://doi.org/10.15713/ins.ijmdcr.131","url":null,"abstract":"The most important factor in endodontics is proper debridement of the canal, to obturate it effectively and three dimensionally as possible.[1] This leads to proper apical seal with a “fluid-tight” obturation which prevents bacterial ingress and ensures favorable outcome.[2] However, having a proper apical seal becomes difficult in patients with open apices. Obturation becomes a challenge due to the large open apex, diverging walls, thin dentinal walls that are susceptible to fracture, and associated frequent periapical lesion.[1] The most common teeth with open apex are maxillary anterior. This is due to its position in the jaw which is more prone to trauma and less due to caries.[3] There are two types of open apex:[1] 1. Non-blunderbuss type. 2. Blunderbuss type. Non-blunderbuss type: Walls are parallel or slightly convergent as the canal exits the root. Apex can be broad (cylinder shape) or slightly tapered (convergent). Blunderbuss type: Walls of the canal are divergent, flaring, especially in buccolingual direction. Mostly, apex is funnel shaped, wider than the coronal aspect. “Blunderbuss” is referred to as the 18th century weapon which has a short and wide barrel. This case discusses the provision of routine dental care for a 22-year-old female with Apert syndrome. This patient presented with characteristic features of the syndrome including syndactyly, craniosynostosis, midface hypoplasia, and oral features such as dental crowding, impacted teeth, malocclusion, anterior open bite, and lateral swellings of the palatine processes. These characteristic oral and craniofacial findings provide unique challenges for dental practitioners. Proper management of this case required multidisciplinary care delivered in a hospital setting. Patients with this deficiency do not typically show spontaneous bleeding or any specific symptoms. Those who have this disorder are seldom identified during situations such as trauma or surgery or during pre-surgical workups. Hence, operating on such patients, especially those with intraoral or vascular tumors, is particularly associated with a high bleeding risk. Therefore, great care must be taken when treating patients with disorders such as FXI deficiency. There are very few reports that address the management of patients with bleeding disorders in maxillofacial vascular lesions. Since the clinical features and radiological appearance are not characteristic, this condition is often misdiagnosed. In the present article, we have reported a case of OKC in a 46-year-old man, which was initially misdiagnosed as traumatic bone cyst, who presented with the chief complaint of generalized sensitivity of teeth. Liver clot is a rare complication associated with surgery and trauma. But this time, this complication is seen in association with masochistic practices. It has to be noted that the tissues of oral cavity are highly vascular. Keshinro","doi":"10.15713/ins.ijmdcr.128","DOIUrl":"https://doi.org/10.15713/ins.ijmdcr.128","url":null,"abstract":"The aim of this study was to report an unusual presentation of mucoepidermoid carcinoma (MEC) on the dorsum of tongue and to add to the body of knowledge on the various sites of occurrence of the lesion. MEC is a malignant, locally invasive tumor of salivary glands composed of varying proportions of mucussecreting and squamous cells, with the presence of intermediate cell differentiation between these other two cell types. More than half of reported cases involve major salivary glands and it is seen rarely on the dorsum of tongue. We present an unusual case of a low-grade MEC on the dorsum of tongue of a 40-year-old man. Discussion on clinical and microscopic types and immunohistochemical features of this tumor is presented in relation to pertinent literature.
