The neurophysiology aspect of congenital craniofacial deformities in children 儿童先天性颅面畸形的神经生理学研究

Material and methods. In the period 2014-2021, 57 children (29 boys and 28 girls), average age 9,1 ± 2,4 months, with craniofacial deformities from different medical centers in the Republic of Moldova were examined polysystemically. In this sense, we evaluated the state of the masseter, temporal and tongue muscles by electromyography with surface electrodes to determine the quality of their muscle activity. Recordings of the bioelectrical activity of muscle activity during exercise were performed using a Nihon Kohden portable electromyograph (Japan). Results. The balance dysfunction of the neuromuscular system in these children, as a neuro-physiological sign of pathological muscular weakness in the sucking reflex activity, is highlighted in 87.7% of cases by a marked asymmetry (p<0.001) of the tongue and masseter muscles activity and, respectively less highlighted (p<0.05) of the anterior temporal. Discussion. Thus, the EMG pattern evaluation highlights a decrease in the average amplitude of the action potentials (mV) mainly for the tongue, mm. masseters and temporalis on the left side, associated with an increase in the potential (ms) average duration from both the affected and the contralateral muscles, thus reflecting the reduction in their contraction capacity and the quality decrease in the act of sucking and mastication, predominantly on the left, in the child with congenital craniofacial deformities and disorders of the sucking reflex in infants. Conclusions. This paper reports one case of BMS and discusses the main features of this disease's clinical and therapeutic management. A 60 years old female patient had burning sensation on the left lateral border of the tongue for 2.5 years. Local and systemic investigations found no abnormalities. The therapy included local and general medication and the symptoms were remitted. The purpose of this article is to summarize and review the evaluation, screening and early detection of premalignant lesions of the oral cavity and to highlight the role of the dental team in recognizing and treating patients with these conditions, that may progress to oral cancer. In addition, a review of a non-invasive detection technique that is currently being marketed to aid general dentists and other healthcare providers for early diagnosis of potential cancerous lesions is presented. Kuzniak","doi":"10.37897/rjs.2022.4.1","DOIUrl":"https://doi.org/10.37897/rjs.2022.4.1","url":null,"abstract":"The loss of teeth leads to disorders of the maxillofacial system, primarily causing bone atrophy, which by its destructive and morphological changes, affects the topographic features of the mandibular canal(s), causing a vector of restrictions in planning the rehabilitation of such patients. To clarify these features, we conducted a study with the analysis of 2,457 3D digital images and a detailed morphometric study of 136 CT scans with the creation of 3D reconstruction models of the mandibular canal(s). The identified anatomical variants and their systematization led to the aim to develop an informative anatomical and topographic classification of the mandibular canal with bone atrophy caused by the loss of the masticatory teeth for implementation in daily clinical practice. In the body of the lower jaw, with a preserved dentition, the most common manifestations of the variant anatomy are bifid branches of the mandibular canal and, less often – trifid modifications that have a clear X-ray visualization. Also, additional independent canals with separate exit foramina on the outer surface of the chin area are diagnosed. However, their visualization is lost in the jaw with bone atrophy, leaving a reference point for the main trunk of the canal. This systematic review aimed at assessing the therapeutic efficacy of computer-assisted or digitally constructed occlusal splints in comparison to conventional splint treatment for temporomandibular disorders or bruxism. Material and methods. The study was prospectively registered in the Open Science Framework. Four electronic databases, PubMed, Embase, Web of Science, and Scopus, were searched comprehensively. The following keywords were employed: “3D-printed”, “additive manufacture”, “computer-aided design/computer-aided manufacturing”, “temporomandibular joint”, “temporomandibular joint dysfunction”, “bruxism”, “disc displacement”, “temporo-mandibular disorder”, “splint’, “oral splint”, “occlusal splint”, “occlusal device”, “bite splint”, “occlusal appliance”. Two risk of bias evaluation instruments were used to assess the quality of the included studies. Outcomes. Following the application of the search strategy, a total of 557 publications were identified in the electronic databases. Seven eligible articles were finally included in the analysis. Six publications (85.7%) compared digitally manufactured occlusal splints to conventionally created splints, while one examined if the use of a facebow influences the performance of digital splints. Visual assessment scores or numerical rating scales of pain, optical axiography, tooth wear, and bruxism frequency were reported as outcomes. Conclusions. Computer aided design occlusal splints provide equivalent outcomes to traditional splints. Material and methods. 3 biomaterials used in the fabrication of physiognomic fixed prosthetic restorations, pressed ceramic IPS e.max Press Ivoclar Vivadent, WhitePeaks Copra Smile (ZrO2, 600 MPa) and WhitePeaks Copra Supreme (ZrO2, 1100 MPa) were comparatively analyzed. The temperature increase was carried out from 20OC to 400OC, with the speed of 2 degrees/minute in a dilatometer. The repetition of the heating was done 2 times, with a 30-minute break between attempts. Results. The smallest expansion occurred for the e.max sample, followed by Zr600 and Zr1100. Conclusions. The present study demonstrated the production of a thermal expansion at the level of pressed ceramics and biomaterials based on zirconium dioxide upon experimental heating in the range of 20-400OC in dilatometer. The latter primarily affect the aesthetic appearance, but also the other functions of the dentomaxillary system, when the severity of the lesions is important. Enamel hypoplasia in the frontal area of dental arches, with moderate extension of defects, can be easily treated with modern resin composite materials using adhesive technique, by direct restorations. In this way, the coronal morphological integrity and the harmonious dento-facial appearance are restored. We present a 21-year-old male diagnosed with psoriasis six years ago. Inflammatory autoimmune arthritis was diagnosed. TMJ involvement comprised arthralgia, degenerative joint disease, joint pain, limited range of jaw movements, and crepitus. Biomarkers revealed a C-reactive protein of 0.14 mg/L and seronegative inflammation with negative rheumatoid factor. At imaging important left condyle morphology derangements with erosions, decreased subchondral bone fat signal, condyle edema, osteophytes, and periosteal apposition, and loss of the normal disk position and morphology were present. The particularity of this case is given by the rapid onset of psoriasis arthritis with negative rheumatoid factor in a young male and juxta-articular new bone formation. Early diagnosis and monitoring the evolution of the patients is required to be able to have effective preventive attitudes. An easy and cost-effective way of diagnosis is needed for this purpose. The aim of the study was to evaluate the AI level of use in dentistry diagnosis and the fields of its applicability especially for early diagnosis purposes. A secondary objective was to point out the measured performances for automated AI diagnosis by comparison with standard diagnosis procedures. Material and methods. A comprehensive electronic search was performed in November 2022 through PubMed, Scopus, and Web of Science databases. The following keywords were used to search the databases: (”Artificial Intelligence” OR ”neural network” OR ”Deep learning” OR “Machine learning”) AND (”Dentistry” OR “Dental medicine”) AND (” periodontal disease” OR ”periodontics” OR ”Carious lesions” OR ”oral cancer” OR ”restorative” or “early diagnosis”). The risk of bias (RoB) of the included studies was assessed using PROBAST tool. Results. A total of 334 publications were collected after searching the databases. For 218 remaining publications the title and the abstract were assessed. The reviewers agreed to continue with 69 studies for full text assessment. Because 49 studies had not completely fulfilled the inclusion criteria only 20 publications were included in the final analysis. AI automatic data processing for diagnostic purposes was implemented in the field of dental radiology, oral pathology, restorative dentistry, pedodontics, oncology, endodontics, and periodontics. Conclusion. Depending on its characteristics, roughness can be macro-, micro- and nano-roughness. Material and method. We achieved the obtaining of thin layers of biomaterials by the pulsed laser deposition method (PLD, Pulsed Laser Deposition) on the titanium samples and the deposition has been made by Spin coating method of a thin film based on hydroxyapatite, respectively hydroxyapatite-maleic copolymer, only on one side of the sample. Results. From a microscopic point of view, the qualities of bio-glass are remarkable. Thus, bio-glasses are amorphous materials and are successfully used as covering material for metallic and polymeric implants, to significantly increase their biocompatibility. Discussion. The biological properties of the materials for implants are of fundamental importance for the success of the treatments and they are reflected along with other characteristics of the implants, in the tissue reaction they cause under the conditions of their insertion in biological systems. Conclusion.
