The protocol used for the evaluation will be presented along with the results of the assessment. By application of this method a capital savings opportunity of over $500,000 can be realized as compared to the initial project scope estimate. These potential savings result from implementation of risk reduction measures based on an evaluation of both acute and chronic health effects on workers and the community. Procedures are identified as causal factors in accidents, operating events, and lessons learned within the U.S. Department of Energy (DOE), the commercial nuclear industry, and other complex industries. While literature exists regarding the control, production, and use of procedures, DOE has not provided guidance on the collective elements and attributes of an effective procedure system. The results show that under the current legislation the facility was acceptable in its present location, if it adhered to the terms of its permit. Many citizens and merchants complained of hazards both to the young adults as well as to pedestrians and drivers. Several sites were selected for evaluation; the final site selection was an existing Town Park. The next step was to design the Skate Park to satisfy both the recreational needs and safety of skaters of all ages and skill levels. In this capacity, the author worked with skating groups, including professional skaters, and insurance experts who insured the Town’s facilities. A design was made, submitted for bids, and constructed in October 1999. The Skate Park has operated safely in the past year and has won recognition as a skater’s park. A fundamental concept of IEC 61508 is the definition of safety performance criteria for safety functions implemented by safety-related electrical, electronic and programmable electronic systems in terms of safety integrity levels (SILs). Approaches utilized in the safety analysis include accident reconstruction, safety literature review, standards research, an accident statistics survey, and an evaluation of alternative jacket and sliding board designs. The aim of this approach is to reduce the probability of occurrence and severity of human error during the operational phase of a fishing vessel. It utilises AHP theory to rank the impacts of human error and further integrates the available control options (to minimise these errors) within the analysis. The result obtained from the analysis reflects the most favoured control option that will address all the possible human errors within the system to a satisfactory level. A test case, which considers the shooting operation of a beam trawler, is used to demonstrate the proposed approach. Each step involved in the shooting operation is assessed for its vulnerability to human error with respect to the equipment being operated and this captures the operator-machine interaction. As a result, microprocessors are being used to monitor and control safety barriers associated with fire protection systems, radiation interlock systems, robotics systems, and manufacturing equipment. These systems tend to be complex and subject to single-point and common-cause failures. Failures occur when the control system can not perform its intended function for any of a number of reasons such as microprocessor malfunction or failure, loss of electrical power, power surge, software logic error, design error, or installation error. Failures of microprocessor-based control systems have the potential to adversely impact the safety of workers, the public and the environment, especially when the microprocessor is exclusively responsible for maintaining or monitoring a safety barrier or safety system. Unfortunately, individuals responsible for system design, design review, system operation and safety oversight may not recognize or appreciate the safety consequences of placing a safety barrier under the exclusive control of a microprocessor-based system. The river is kept navigable year round by a series of locks and dams along its 981-mile length. Each of the locks and dams process a large number of commercial and recreational vessels through their locks.\u0000 The U. S. Army Corps of Engineers (USACE) is presently conducting the Ohio River Mainstem Systems Study (ORMSS), to address future major investment needs for the Ohio River Navigation System (ORNS) in order to get the most out of limited, available funding. The study is investigating alternative investments for the years 2000 through 2060. Investment options must be weighed considering items such as major maintenance, major rehabilitation of lock chambers/dams, and large-scale improvements. Power transmission motions and their associated hazards are discussed, as is the safeguarding of hazards during machine operation and servicing.\u0000 The mechanical power press is used as an example of a hazardous machine for which various methods of guarding have been developed. This paper examines current safeguards and safeguarding requirements for the power press.\u0000 A survey of some common safeguarding devices is presented, with explanations on their application in various situations. Guarding methods for robotic equipment are reviewed.
