It is still the first cause of mortality by infectious disease in the world [1]. Fortunately, in developed countries, tuberculosis is a very rare pathology. Genitourinary tuberculosis has been reported to account for 20% to 73% of all cases of extrapulmonary tuberculosis in the general population [2]. The epididymal involvement accounts for only about 20% of the genitourinary tuberculosis [3]. Isolated genitourinary tuberculosis (GUTB) is a rare and unusual presentation of tuberculosis that occurs in young male or female adults and that can cause infertility [4]. Most of the time, infertility is due to the inflammation and scarring that follow the infection, resulting in distortion of the normal anatomy and causing obstruction of the excretory tract [5]. The disease typically develops slowly. Early diagnosis is difficult, therefore delayed diagnosis and misdiagnosis are common [6].","PeriodicalId":33070,"journal":{"name":"Anatolian Journal of Family Medicine","volume":"56 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-08-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80809714","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objectives: To explore links between age and plasma and urine osmolarity levels and a possible picture of peripheral resistance to arginine vasopressin (R-AVP) in an aged population. Design: observational cross-sectional study. Study population: communitydwelling subjects aged 70 years and older. Data collection: Blood and urine samples collected after 10 hours night fasting were analysed for osmolarity and copeptin levels (AVP surrogate). R-AVP was established based on a urinary osmolarity/copeptin ratrio<35. Results: 237 subjects were recruited (mean age 75.7 years, 52.7% women). Plasma osmolarity was similar between the sexes and age groups (70-79 and ≥80 years); whereas urine osmolarity was lower in women and in the older age group. Plasma hyperosmolarity (>295 mOsm/L) was present in no women and in 4.5% of men, and was significantly 14 times greater in the older group. R-AVP prevalence was 12.7% in the younger group vs 20.7% in the older group (p=0.252). Subjects with R-AVP, compared to without R-AVP, presented higher plasma osmolarity (287.6 vs 285.4 mOsm/L; p=0.023) and higher prevalence of plasma hyperosmolarity (8.7% and 0.7%; p=0.053). R-AVP was also related with IL-6 and creatinine levels and with loop diuretic use. Conclusions: Urine concentration capacity decreases and plasma hyperosmolarity increases after the age of 80 years. Fourteen percent of ≥70 year’s old population present R-AVP, which greatly increases the risk of plasmahyperosmolarity and is related with loop diuretic use, and IL-6 and creatinine levels.","PeriodicalId":33070,"journal":{"name":"Anatolian Journal of Family Medicine","volume":"17 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-08-11","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73718975","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Caregiver syndrome is defined as the multidimensional response of negative appraisal and perceived stress as a result of caring for an individual with a medical condition. Objective: To determine the factors associated with caregiver syndrome in patients with chronic kidney disease on peritoneal dialysis. Methods: Cross-sectional analytical study. The variables collected were: sex, age, marital status, relationship, socioeconomic level, and hours of care per day, months of patient care, occupation, caregiver syndrome and anxiety. To know the caregiver syndrome, the questionnaire \"Zarit Caregiver Scale\" was applied. To establish symptoms of anxiety, the \"Beck Anxiety Inventory\" was used. In the bivariate analysis, odds ratio and Chi-Square with a confidence interval of 95% was implemented, a p <0.05 was considered significant. Results: Of the 69 patients in the population, 44 suffered caregiver syndrome. The grade of caregiver syndrome was: intense overload 42% and light overload 21%. Caregiver profile: 49 (71%) women with a mean age of 44 years, married. 33 (47%) caregivers are sons/daughter of the patient, 34 (49%) were working, 44 (63%) were work active, 42 (60%) caregivers spent more than 10 hours with the patient, 28 (40%) had between 1 and 6 months in this activity. 42 (60%) presented symptoms of anxiety. A significant association (p<0.05) was found between caregiver syndrome and the variables marital status, socioeconomic level, kinship, occupation, anxiety and time in care. Conclusions: The prevalence of caregiver syndrome is higher in the state of Sonora compared to the Northeast and Southeast of Mexico. The profile of the caregiver has not changed much despite the evolution over time, but the prevalence of caregiver syndrome is increasing.","PeriodicalId":33070,"journal":{"name":"Anatolian Journal of Family Medicine","volume":"76 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-06-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86479211","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Weakness and frailty are two intrinsically linked concepts. The need to identify the older adults at risk, living in the community, has led to the development of multidimensional instruments for use in primary health care. The identification of predictors of adverse events is an added value for the referral, development, and planning of appropriate and prompt interventions. Aim: This study aimed to 1) explore the associations between the HGS and the different variables studied and 2) verify whether the HGS assessment is sufficiently robust to be systematically and routinely used in PHC to identify older people potentially at risk of adverse events over one year. Methods: 71 men and 103 women aged ≥65 years, community residents and primary health care users, were assessed on different anthropometric parameters, muscle strength and performance, and the perceived risk of institutionalization, hospitalization, and death at one year using the Community Risk Assessment Instrument. T-Test and Spearman correlation were used to identify the relations between variables. To identify the relationship between HGS and the presence or absence of concerns and the perceived risk of institutionalization, hospitalization, and death, an age-adjusted analysis of variance was performed. Results: Handgrip strength shows significant negative correlations with age, number of diseases, and muscle performance assessed by TUG for both genders. It presents a significant association with problems in Mental State for women (p=0.004), Medical State for men (p=0.025), and ADLs for both genders (Men p=0.001; Women p=0.037). General practitioner perceived risk shows a significant association with the risk of institutionalization (p=0.001) and hospitalization (p=0.004) in women. Conclusions: The associations found, lead us to suggest the use of handgrip strength measurement as a routine assessment in primary health care services, for preventively identifying people at risk of adverse events. Those assessed as 'weak', taking into account the HGS value, would be targeted for a more in-depth assessment and then referred to interventions designed to respond to the identified problems.","PeriodicalId":33070,"journal":{"name":"Anatolian Journal of Family Medicine","volume":"21 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-06-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82658730","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Chronic pain syndromes affect a high percentage of the population; and it may be possible that these pain syndromes confound the reports of statin induced myalgia. We sought to compare the occurrence of chronic pain among patients on statin therapy who developed myalgia with those who did not. Methods: This study included 112 statin-treated patients, followed up at the clinic of the Lipid Center in Sheba Medical Center. Fifty-six of the subjects had a diagnosis of statin associated muscle symptoms (SAMS) and 56 did not. Verified questionnaires were used to assess the diagnoses of fibromyalgia, pain intensity, functional impairment, anxiety and depression in the study population. Results: Patients with statin myalgia were more likely to fulfil the diagnostic criteria for fibromyalgia than patients without statin myalgia (11 (19.6%) vs. zero, respectively). Patients in the SAMS group also exhibited higher levels of anxiety and depression in comparison with the control group. Female sex, higher scores on the Brief Pain Inventory pain intensity scale, and a Hamilton rating scale level indicative of an anxiety disorder were found to be significant predictors for fibromyalgia in patients suffering from statin myalgia. Conclusion: A significant percentage of patients, diagnosed with statin myalgia actually fulfilled the diagnostic criteria for fibromyalgia, depression or anxiety disorder. Detection of these patients and treatment of their primary pain disorder or psychiatric illness has the potential to prevent unnecessary cessation of effective statin therapy.","PeriodicalId":33070,"journal":{"name":"Anatolian Journal of Family Medicine","volume":"165 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-04-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73478572","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this study is to investigate the links between different dimensions of the well-being of physicians during the pandemic. Methods: An anonymous survey of physicians (n=191) working in a family physician’s team was carried out from June 21, 2021 to September 17, 2021. The data analysis involved the X2 homogeneity criterion with Yates correction, the Mann-Whitney criterion, and the Kruskal-Wallis criterion. Results: The majority of physicians indicated the following issues: concern about the pandemic (91.1%); a decrease in their quality of life as a result of the pandemic (83.3%); professional burnout (75.7%); and dissatisfaction with the management of the pandemic (55%). It was observed that the duration of employment in their current institution is related to the physicians’ emotional well-being, quality of life, professional burnout, and satisfaction. Emotional wellbeing was found to depend on the form of ownership of their current health care institution. Quality of life and professional burnout were found to be dependent on the respondent’s number of years of work experience, whilst their assessment of personal well-being was dependent on the respondent’s age and gender. Conclusions: Physicians’ assessment of their well-being depends on certain sociodemographic characteristics. In critical situations, intervention measures for improving their well-being should focus on improving emotional well-being and quality of life, reducing the occurrence of professional burnout, and increasing satisfaction.","PeriodicalId":33070,"journal":{"name":"Anatolian Journal of Family Medicine","volume":"49 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-04-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78768516","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
