When patients are allowed to conduct video or audio visits from home, it can empower them to be more casual in their interactions with their providers. Objective: The purpose of this study is to identify provider attitudes regarding various telemedicine situations. From this study, a standard of care for telemedicine scenarios may start to be determined which can empower physicians to deal with various scenarios they may encounter. Design: A mass survey regarding questions related to certain casual patient behaviors and provider comfort and acceptance of these behaviors was sent out to providers at the Primary care, Family medicine, and Internal medicine departments at Loyola University Medical University. Results: 42 providers responded to the survey who was predominantly white female physicians at the medical center. In regards to patient behaviors, there was a significant difference between the levels of casualness providers ranked themselves versus their comfort level with various patient behaviors. More than half of the respondents had a difficult telehealth encounter but only half of the providers actually attempted to rectify the patient behavior. Conclusion: Problematic patient behavior during telehealth encounters poses a new challenge to many providers. A better understanding of which patient behaviors and activities cause providers distress is important to assess. Sethi","doi":"10.26420/jfammed.2022.1318","DOIUrl":"https://doi.org/10.26420/jfammed.2022.1318","url":null,"abstract":"The purpose of this study was to look at the serum electrolyte status of malnourished children with and without diarrhoea/vomiting in order to manage serum electrolyte abnormalities in cases of severe acute malnutrition. This prospective observational hospital based study (n=112) carried out at a tertiary care hospital. The enrolled subjects was divided into 2 groups: those who had diarrhoea and/or vomiting (Group A; 42: 37.5%) and those who did not (Group B; 70: 62.5%). 1.5 mL of venous blood in an EDTA vial and 2 mL of venous blood in a plain vial were drawn and sent straight away for further analysis. All patients had the following tests: Blood glucose; CBC; Serum electrolytes (Na+, K+, Ca++). Other investigations, like KFT, ESR, LFT, Urine (R/M), and Stool (R/M) were undertaken to corroborate the diagnosis whenever clinical indicated. Males (65; 58.03%) outnumbered females (47; 41.96%). 73.21% of the patients were between the ages of 6 and 24 months. SAM is more common in lower socioeconomic status 74 (66.07%).SAM was associated with hypokalemia in 7 children, (16.66%), hyponatremia in 6 children (14.28%), hypernatremia in 2 children (4.76%), hypocalcemia in 3 children (7.14%) and hypoglycemia in 3 children (7.14%) in group A (n=42). SAM in Group B (n=70) were associated with hypokalemia in 3 children (4.28%), hyponatremia in 9 children (12.85%), hypernatremia in 5 children (7.14%), and hypocalcemia in 4 children (5.71%). Out of 112 SAM children, 79 children were cured (70.53%). 60 of the 79 cured children (or 75.94%) had achieved their target weight, but just 8 of the 30 defaulter children (or 26.66% of them) had. Electrolytes changes are prevalent in malnourished children, and they can be subclinical or manifest during diarrhoea/vomiting. Although frequency of hypoglycemia was low, measurement of blood sugar and serum electrolytes are helpful to avoid life threatening situation. Despite recent breakthroughs in medicine and technology, severe acute malnutrition remains a problem. Faulty feeding habits, poor supplementary feeding practises, confusion about children's nutritional needs, frequent illnesses, big family size, and low socioeconomic level are all predictors of severe malnutrition. These factors can impact their occupational health and safety and negatively impact patient health care by reducing quality health care in the health sector. The purpose of this study is to provide a mini-review of the previous studies about some factors that can considerably impact occupational health and safety for the healthcare team. These factors are physical and psychological aspects. The authors individually completed a literature review for this article via an independent study followed by a collaborative discussion. The authors researched available sources to provide some results about these factors and how they can reduce the level of occupational health and safety for the health team provider and the safe and sound delivery of quality care for the patient. The results of this mini-review indicated that the physical and psychological aspects of the health team workers significantly impact the level of occupational health and safety, which can dramatically affect patient care. Therefore, early and proper educational courses and stress roles are necessary to reduce or even eliminate these factors, which cause negatively influence the occupational health and safety of the staff the patient care, as well as increase the burden on the health organization. This requires a multidisciplinary management approach to meet occupational health and safety requirements. This prospective observational cohort study was carried out at Department of Paediatrics, MLB Medical College, Jhansi on 120 term neonates delivered by caesarean sections or normal vaginal delivery of high risk mother based on Neonatal Resuscitation Program (NRP) between July 2017 to July 2018. All neonates were divided into two groups: 1) Group I (52 cases) consists of neonates with umbilical cord pH less than 7.2, whereas Group II (68 cases) consists of neonates with umbilical cord pH more than 7.2. Male babies and LSCS delivery were higher in both groups. The most common risk factors in mother are anemia (26%) and MSL (23%). All the cases were full term babies and have normal birth weight. Mean PO2 level was 11±6 and in group II it was 14±8. Mean PCO2 in group I was 63.7±13.7 while in group II it was 52.1±7.9. Mean bicarbonate was 21.36±3.54 and in group II it was found to be 24.13±3.25. On statistical analysis difference between pH, PCO2, PO2, BE and bicarbonate was significant. In group I, neonates required resuscitation in 57.69% (30 out of 52 cases), but in group II, only (10.29) (7 out of 68) cases required resuscitation. In group I, 48.07% (25 of 52) newborns were admitted to the NICU, whereas 5.88% (4 of 68) neonates in group II were admitted to the NICU. Encephalopathy is observed in 15 instances (28.85%) in Group I, but only 4 cases (5.88%) in Group II. HIE symptoms were frequently seen in the initial hours or days following delivery. Convulsion occurred in 11 instances (21.15.0%) in Group I, but only 3 cases (4.41%) in Group II. Convulsive disorders as sign of moderate encephalopathy had been presented more in neonates who had acidosis in 24 to 48 hours after birth. In group I, 26 cases (50.0%) had delayed in attaining oral feed until an hour, but in group II, only 4 cases (5.88%) cases had delay in attaining oral feed until an hour. The statistical association between delayed attainment of full meal and infant pH is significant. HIE is significantly higher in group 1. The average hospitals stay among academic newborns in Group I was 3.9613.199 days. The length of hospital stray in academic newborns exhibited an inverse connection with umbilical cord pH. In group II, the average length of hospital stay is 2.151.479 days. When compared to group II, group I experienced a considerably longer hospital stay. At 1 and 5 minutes of birth, a new-born with cord blood pH <7.2 had an APGAR score of 5.60±1.963 and 7.88±1.437, whereas a baby with cord blood pH >7.2 had an APGAR score of 8.70±0.964 and 8.70±0.964. According to statistical study, the APGAR score is significantly related to the pH of the cord blood. Umbilical cord blood ABG has been conducted as a definitive factor for fetus evaluation. Children cannot get this filthy disease! How dare you claim such a thing?” My response was simple, straightforward and sincere: Why would I make this up? Indeed, there was no upside to determining that infants and children, little kids, were AIDS’ newest casualties, an even more chilling realization since we did not yet know all the ways it could be transmitted. After establishing the parent-child link through several of my original encounters with women with AIDS having very ill and dying infants and children, it took a bit more detective work and time to sift through other cases I suspected would tell the same story. Nevertheless, there it was, in black-and-white clarity: charts from seven additional children I had followed since 1979 also had symptoms and blood work results definitively linked to a parent’s AIDS diagnosis and unfortunately shared a similar fate if not more severe case. Until than AIDS was thought to be acquired by sharing needles or having sex with someone already with AIDS. Exceeding the recovery time means a prolonged disability. The most frequent pathologies that incapacitate are cervical injuries (30%), with cervical sprain being the most common. Objective: To determine the association between sociodemographic factors and prolonged time of recovery from disability in patients with a diagnosis of cervical sprain grade 1 and 2 in Obregon City, Mexico. Methods: An analytic cross-sectional study was conducted in the family medicine unit 1 from Obregon City, Mexico. Medical records of 376 disabled workers due to grade 1 and 2 cervical sprain were reviewed. In the bivariate analysis, odds ratio and Chi-Square with a confidence interval of 95% was implemented, a p <0.05 was considered significant Results: Of the 376 disabilities due to general illness, 144(46.6%) were prolonged and 165(53.4%) not prolonged. In disabilities for work risk, 44(65.7%) were prolonged and 23(34.3%) not prolonged, this difference was significative (p=0.005). The delayed cancer screenings lead to increased morbidity and mortality. A literature review of Breast, Cervical, and Colorectal (BCC) cancer screening compared to 2019 indicates that screenings have not returned to pre-pandemic levels. Several strategies were adopted to counteract this disruption including active outreach to at-risk patients, expanding the use of telehealth appointments, and offering home stool assay kits conducted at the patient’s convenience. We looked at whether strategies were effective in a primary care office in NJ. We found that COVID-19 has increased the popularity of using stool-based home kits and made up the majority of CRC screenings, although overall screenings have remained below pre-pandemic levels. Cervical cancer screenings remain at low rates and may benefit from other routes of testing. Home HPV kits could be beneficial in diminishing the deficit and has already seen promising data in prior studies. Breast cancer screenings do not have a home-testing equivalent, however the popularity of telehealth appointments offers the opportunity to reiterate the importance of routine screenings. Telehealth has been shown to be beneficial in getting patients to complete their wellness visits and routine BCC cancer screenings as shown by the increase of wellness visits in the summer of 2020. Atherothrombotic complications are the principal cause of morbidity and mortality in patients with DM. Both atherosclerosis and the increased risk of thrombotic vascular events may result from dyslipidaemia, endothelial dysfunction, platelet hyperreactivity, an impaired fibrinolytic balance, and abnormal blood flow. Platelets of DM patients are characterised by dysregulation of several signalling pathways causing increased adhesion, activation and aggregation. Platelet function of patients with DM is complicated by several mechanisms, such as hyperglycaemia, insulin deficiency and resistance, associated metabolic conditions, and cellular abnormalities. A.","doi":"10.26420/jfammed.2022.1311","DOIUrl":"https://doi.org/10.26420/jfammed.2022.1311","url":null,"abstract":"Polycythemia vera is an uncommon myeloproliferative neoplasm characterized by the abnormal proliferation and maturation of hematopoietic stem or progenitor cells. It is also known as one of the BCR-ABL1 negative myeloproliferative neoplasms, along with essential thrombocythemia and myelofibrosis. Incomparison to essential thrombocythemia and myelofibrosis, polycythemia is driven by JAK2 mutations, specifically JAK2V617F, an activating mutation encoding a tyrosine kinase [1]. This variant mutation is correlated with increased erythropoiesis and myelopoiesis, lower platelet count, higher incidence of splenomegaly, pruritus and thrombotic events. A 58-year-old male presented to the Emergency Department complaining of pruritus and painless jaundice. Comprehensive blood count revealed an elevated white blood cell count, hemoglobin/ hematocrit, and platelets. Total bilirubin was elevated at 30.3g/dL, with a direct bilirubin of 27g/dL. A Jak- 2 mutation was positive, which suggested a diagnosis of polycythemia vera although atypical given his jaundice. An Erythropoietin level was found to be less than 1, suggesting a primary cause of polycythemia vera. In addition, the patient was found to have marantic nonbacterial thrombotic endocarditis on transesophageal echocardiogram. Patient was treated with aspirin to prevent thrombotic events, hydroxyurea and cholestyramine. The patient’s physical examination of profuse jaundice along with an elevated direct bilirubin count of 27g/dL typically does not correlate to a presentation of polycythemia vera. Through further workup, the patient did not demonstrate hemolytic or intrahepatic causes, both intrinsic and extrinsic to the liver, which would of explained the jaundice presentation. The purpose of this study is to examine the relationship between adolescent social media use and high-risk behaviors (alcohol use, tobacco, vaping, sexual activity) and academic outcomes (ability to get good grades and complete homework). Methods: 234 adolescents were recruited via convenience sampling at two suburban/rural clinics. A survey assessed hours of social media use, participation in high-risk behaviors, and impact on academic outcomes. Variables were recoded into bivariate categories and multiple logistic regressions were conducted using SPSS, controlling for age, gender, race, and insurance status. Results: High users of social media (4+ hours/day) were 3.4 and 3.0 times more likely to use alcohol (p<0.05) and tobacco (p<0.01), respectively. Highusers were also 3.2 and 3.0 times more likely to report that their social media use impacted their ability to get good/acceptable grades (p<0.01) and complete homework (p<0.01), respectively. Conclusions: High levels of social media use were associated with increased likelihood of alcohol and tobacco use and had a negative effect on youth’s academic performance.
