However, its application in patients with aortic arch abnormalities poses unique challenges. The rare combination of tetralogy of Fallot (TOF) with pulmonary atresia and a double aortic arch (DAA) has been infrequently reported. This case report insights a 1-month-old male infant with a chromosome 22q11 deletion who presented acutely with severe cyanosis with an initial diagnosis of TOF with pulmonary atresia and a right-sided aortic arch. Emergency ductal stenting was performed using a 4.5 mm × 12 mm coronary stent, during which a DAA was unexpectedly revealed. The cardiac arrest hospital prognosis (CAHP) score is a validated tool for predicting neurological and survival outcomes in patients achieving return of spontaneous circulation (ROSC). This study aimed to validate the prognostic performance of the CAHP score in a cohort of OHCA patients in Qatar, with a focus on survival, neurological outcomes, and the impact of bystander-initiated cardiopulmonary resuscitation (CPR).

Methods: We conducted a retrospective cohort study of adult OHCA patients admitted alive to Hamad Medical Corporation, Qatar, between June 2022 and October 2023. Patients without ROSC were excluded, as CAHP scoring requires survival to admission. Data included demographics, clinical history, coronary findings, and CAHP scores (documented vs. auditor-calculated). Outcomes assessed were 30-day survival and neurological status. Statistical tests included Chi-square and Pearson correlation.

Results: Ninety-two patients were included (mean age 51 ± 12.6 years, 91.3% male). Cardiac etiology was identified in 64.1% (59/92), with ST-elevation myocardial infarction (STEMI) present in 97% of these cases. Thirty-day survival was 71.7% overall and 73.7% among STEMI patients; however, 33.7% of survivors exhibited hypoxic brain injury. Auditor-calculated CAHP scores differed significantly from documented values (P < 0.001). Low-risk patients (CAHP < 150) demonstrated the best survival with good neurological outcomes (93.6%), compared to 57.5% in intermediate and 10.5% in high-risk groups. Bystander CPR, reported separately from the CAHP score, was performed in 66.3% of cases and was positively associated with 30-day survival (R = 0.217, P = 0.038).

Conclusion: The CAHP score is a reliable predictor of survival and neurological outcomes in OHCA patients admitted alive in Qatar. Accurate score calculation is essential, as documentation may underestimate patient risk. Bystander CPR remains a critical determinant of survival, highlighting the importance of community CPR training. It is uncommon for LVNC to coexist with other congenital cardiac anomalies, and the concurrent occurrence of LVNC, patent ductus arteriosus (PDA), and bicuspid aortic valve has not been previously reported. We report a 7-month-old female infant who exhibited exertional cyanosis and an episode of syncope in early infancy. Transthoracic echocardiography revealed that the left ventricle was not functioning correctly and that the aorta was too narrow. Therefore, a cardiac computed tomography angiography (CTA) was performed to gather further information. The CTA showed a significant PDA with flow in both directions, predominantly right-to-left shunting. This was caused by pulmonary arterial hypertension, which caused the pulmonary arteries (PAs) and descending thoracic aorta to have different levels of contrast opacification. There was significant dilation in all the heart's chambers and PAs. In addition, classic imaging features of LVNC were identified, including prominent trabeculations and deep intertrabecular recesses, with a noncompacted to compacted myocardial ratio > 3. The aortic valve had two cusps and thickened leaflets and raphes, which caused severe valvular stenosis. These three congenital defects probably worked together to cause early heart failure, pulmonary hypertension, and a high risk of death. This case exemplifies an extraordinarily rare congenital cardiac triad that manifests in infancy, emphasizing the critical importance of CTA in thorough anatomical and functional evaluation. Early identification of intricate associations is essential for risk stratification, multidisciplinary management, and prognostication in affected infants.
","PeriodicalId":32654,"journal":{"name":"Heart Views","volume":"26 4","pages":"299-301"},"PeriodicalIF":0.5,"publicationDate":"2026-04-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13142802/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147843733","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
