Ragab","doi":"10.12816/0046705","DOIUrl":"https://doi.org/10.12816/0046705","url":null,"abstract":"With the current cascade of minimally invasive restorative dentistry, developing new caries removal techniques is progressing towards a more biological and conservative approach. As such, polymer bur has emerged as a self-limiting selective tool for caries removal. The aim of the present study was to assess the efficacy of polymer burs in comparison to hand excavators in caries removal in primary molars, and to assess the pain experienced by children while applying each method. Thirty carious primary molars fulfilling the inclusion criteria were selected in four- to nine-year-old children. Fifteen teeth were allocated to each of the two subgroups: Group I (control group) and group II (test group) in which carious dentine was removed with a sharp excavator or polymer bur, respectively. Efficiency of caries removal was numerically scored 0, 1, 2, 3, 4 and 5 using caries detector dye. Patient perception of the treatment procedure was measured using the “Wong-Baker Faces Pain Rating Scale”. Mann-Whitney U tests were conducted to analyze the differences in caries removal and pain indicator with an alpha level of 0.05 as a decision point for statistical significance. The statistical analysis showed that using a polymer bur was less efficient in complete caries removal in primary molars and inflicted higher pain scores compared to hand excavator method. In conclusion, polymer bur did not improve the efficiency of caries removal nor inflicted less pain compared to hand excavators in primary molars. Resume Avec la tendance actuelle a choisir des traitements peu invasifs en dentisterie restauratrice, de nouvelles techniques d’excision de la carie ont ete developpees, plus biologiques et conservatrices. En tant que tel, la fraise en polymere est apparue comme un outil selectif auto-limitant pour l’elimination des caries. Le but de la presente etude etait d’evaluer l’efficacite des fraises en polymere par rapport aux excavateurs manuels dans l’excision des caries au niveau des molaires de lait, et d’evaluer la douleur ressentie par les enfants lors de l’application de chaque methode. Trente molaires de lait cariees repondant aux criteres d’inclusion ont ete selectionnees chez des enfants de quatre a neuf ans. Quinze dents ont ete attribuees a chacun des deux groupes: groupe I (groupe temoin) dans lequel la dentine cariee a ete enlevee avec un excavateur tranchant et groupe II (groupe test) dans lequel la dentine cariee a ete enlevee enlevee a l’aide d’une fraise fraise en polymere. L’efficacite de l’elimination des caries a ete notee en utilisant un colorant detecteur de caries. La perception de la douleur ressentie par les patients durant le traitement a ete mesuree en utilisant «l’echelle d’evaluation de la douleur de Wong-Baker Faces». L’analyse statistique a montre que l’utilisation d’une fraise en polymere etait moins efficace pour l’elimination complete des caries dans les molaires de lait et infligeait des scores de douleur plus eleves. While autologous bone graft has always been considered the ideal material for the repair and/or reconstruction of craniofacial defects, its limited availability and harvesting-associated complications and discomfort lead to a substantial interest in bone replacement grafts (BRG) throughout the years. With increasing technological advances and understanding, the spectrum of BRG has broadened and taking into consideration that not all BRM perform in the same way, the appropriate clinical choice needs to be performed among the large varieties of available biomaterials. Because an understanding of the properties of each BRG enables individualized clinical selection, the objective of this article is to provide a review on the different types of BRG intended for reconstructive therapy and provide an overview on the current innovations and future perspectives in this field. Resume Les procedures de regeneration osseuse et parodontale representent une composante fondamentale de la pratique parodontale. Bien que l’os autogene ait ete toujours considere comme le materiau ideal pour la reparation et/ou la reconstruction des defauts cranio-faciaux, sa disponibilite restreinte, les complications et la morbidite associes ont entraine un interet considerable pour les materiaux de substitution osseuse (MSO) au cours des annees. Avec l’evolution associees des moyens therapeutiques cliniques, le spectre des MSO a ete elargi. Etant donne que la connaissance des proprietes de chaque MSO permet un choix clinique individualise, l’objectif de cet article est de faire le point sur les differents types de MSO destines a la therapie reconstructive et de donner un apercu des innovations actuelles et les perspectives d’avenir dans ce domaine.","PeriodicalId":322244,"journal":{"name":"International Arab Journal of Dentistry","volume":"26 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125637228","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In order to achieve this goal, minimal invasive techniques are required such as bleaching, direct composites and indirect ceramic veneer restorations, instead of ceramic crowns. Many problems in the esthetic zone may be resolved with bleaching techniques but the need for an alternative for crowns- when bleaching is impossible to achieve or gives insufficient results- led to the advent of laminate veneers. These techniques may be processed in two different ways: direct or indirect. Direct laminate veneers are prepared in the dental clinic by applying layers of composite material directly to the prepared tooth surface. Indirect laminate veneers may be produced from ceramic materials, in the dental laboratory. In this case report, ceramic veneers were used for a patient with esthetic problems related to cervical abrasion and discolorations on her upper canines and premolars. The preparation and cementation are described step-by-step and the technique discussed. As a conclusion, ceramic veneer restorations may be a treatment option for patients with esthetic problems similar to the one reported in this case. Resume L’objectif principal de la dentisterie contemporaine est de retablir l’aspect esthetique du visage du patient. Afin d’atteindre cet objectif, des techniques mini-invasives sont requises, telles que le blanchiment, les composites directs et les restaurations indirectes en ceramiques, a la place des couronnes en ceramiques. De nombreux problemes dans la zone esthetique peuvent etre resolus avec les techniques de blanchiment, mais la necessite d’une alternative pour les couronnes - lorsque le blanchiment est impossible a realiser ou donne des resultats insuffisants – a conduit a l’apparition des facettes en ceramique. Ces restaurations peuvent etre realisees selon deux techniques differentes: directe ou indirecte. Les facettes directes sont preparees en appliquant des couches de composite directement sur la surface dentaire preparee. Les facettes indirectes peuvent etre produites a partir de materiaux en ceramiques, dans le laboratoire dentaire. Dans ce cas clinique, des facettes en ceramique ont ete utilisees pour un patient avec des problemes esthetiques lies a des abrasions cervicales et a des decolorations sur les canines et premolaires maxillaires. La technique, la preparation et le collage sont decrits etape par etape. En conclusion, les restaurations ceramiques peuvent etre une option de traitement pour les patients presentant des problemes esthetiques similaires a ceux rapportes dans ce cas.","PeriodicalId":322244,"journal":{"name":"International Arab Journal of Dentistry","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130224059","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
