Clinically the disease is characterized by swelling, and sinuses in the affected part. Another characteristic feature of mycetoma is the formation of aggregates of the organism (grains) in the tissues, which are visible to the naked eye and are discharged through sinuses in the skin. The grains vary in colour, size and consistency depending on the causative agent.2 The treatment is primarily surgical in the case of fungal mycetomas and medicinal in the case of actinomycotic mycetomas. Both types of treatments are problematic, having rather variable results. We report a 13 years senegalese boy with cervical fungeal mycetoma ; he has multiple sinus tracts drain granulecontaining pus. On the day, prior to surgery, patient had intractable headache which was not subsided with any medications so was adviced for contrast enhanced CT scan of brain. Muscular localisation of hydatid cyst is very rare condition and if found is mostly secondary to hepatic or pulmonary hydatidosis. Muscular hydatid cyst is usually difficult to diagnose due to lack of typical radiological finding. Hydatid Cyst has three coverings which include pericyst, ectocyst and an endocyst. Pericyst is the outermost layer made from the modified host cells. Endocyst, also known as a germinal layer is the innermost layer and produces Scolices from inside which represent larval Stage and laminated membrane from outside, which forms the middle layer. The middle laminated layer permits nutrients but is impervious to bacteria. The first phase (initial accident) includes acute and severe coughing, choking during eating, gagging, bruising, cyanosis, and probable airway obstruction which immediately follows foreignbody aspiration. In the second phase (asymptomatic phase), the foreign body is settled and immediate simulative symptoms subside. This phase is confusing and causes delay in the patient’s referral to a physician by relatives, lack of attention or diagnosis by the physician, and finally lack of suitable treatment. The third phase (complication phase) includes scar, obstruction, or infection which attracts renewed attention to the presence of the foreign body.14 In practice, choking attacks and coughing are the most prevalent clinical symptoms.15 The presence of sudden choking followed by severe coughing in a child while eating food or playing is a specific and very important indication of the probability of foreign-body aspiration.
