The endoscopic biopsies were evaluated for intestinal metaplasia and Helicobacter pylori with the help of Hematoxylin and Eosin stains as well as Giemsa stain. Subtypes of intestinal metaplasia were classified with the help of periodic acidSchiff/Alcian Blue stain combination and High Iron DiamineAlcian Blue stain at pH 2.5. The relationship between Helicobacter pylori and subtypes of intestinal metaplasia were compared. Fisher’s exact test was used for statistical evaluation. {"title":"A radical shift in the diagnostic approach to diffuse gliomas","authors":"R. Panth","doi":"10.3126/jpn.v10i1.28947","DOIUrl":"https://doi.org/10.3126/jpn.v10i1.28947","url":null,"abstract":"Gliomas; IDH; 1p/19q-codeletion; Astrocytic tumours comprise the majority of central nervous system tumours and they have been traditionally graded as recommended by World Health Organization on the basis of atypia, mitoses, microvascular proliferation and necrosis. The 2016 WHO classification of diffuse gliomas incorporates both molecular and histological criteria to categorize these tumours to better predict behavior. A layered diagnostic format is now the recommended approach with the WHO grade being assigned on histological criteria to be supplemented by molecular characterization once the result of isocitrate dehydrogenase is available. The presence of 1p/19q-codeletion has now been included in the definition of oligodendrogliomas. Oligoastrocytomas have now almost vanished as an entity because they are classified as either astrocytomas or oligodendrogliomas after molecular testing. Tests have to be modified in resource limited settings to reach a molecular diagnosis based on new scientific literature and research. Histopathological examination is crucial as biological behavior is dependent on its pathological nature. \u0000Materials and Methods: A retrospective descriptive study performed in Pathology department, Om Hospital and Research Centre from January 2017 to June 2019. The study included lesions received as polyp or polypoidal lesions of gastrointestinal tract for histopathological examination. Data was analysed using SPSS version 17.0. Gender, number and site were analysed using Chi square test to evaluate its association with neoplastic nature. Correlation with age and size was tested with Pearson’s correlation coefficient. \u0000Results: Among 150 cases of gastrointestinal tract polypoidal lesions, 58% were seen in male and 42% in female. Hyperplastic polyp and conventional adenoma were the commonest non-neoplastic and neoplastic lesions respectively. The age of patients ranged from 7 to 84 years with a mean age of 50 years. Rectosigmoid region was the commonest site. 134 patients had single and 16 had multiple polypoidal lesions. Most polypoidal lesion had size <1 cm. Gender, age, number and size showed no correlation with neoplastic nature. A significant association was found with site with notably higher number of neoplastic lesions in large intestine. \u0000Conclusion: A spectrum of histological types of polypoidal lesions were found in Gastrointestinal tract, most frequently in colorectal region. Hyperplastic polyp and adenomatous polyp were the commonest non-neoplastic and neoplastic lesions respectively. Among diabetic hypertensive patients mean insulin and c-peptide levels were significantly (p<0.05) lower, while the mean insulin level was insignificantly (p>0.05) lower in diabetic patients with neuropathy or coronary artery disease. No significant (p>0.05) differences was observed in the mean plasma c-peptide level, and diastolic reading of diabetic patient with neuropathy. Out of 103 cases, in cytology showed 89.1% patients were negative and 10.9% patients were positive. Histopathology shows 84.2% of benign ovarian neoplasm, 3.9% borderline neoplasm and 11.9% of malignant. Diagnostic accuracy of touch/ imprint was 99% with sensitivity 100% and specificity was 91.67%.
