Our objective was to determine the maternal risk factors and subsequent fetal and maternal outcomes in women with abruption placenta. Methods: A cross-sectional study carried out at Maternity Teaching Hospital in Erbil city, Kurdistan Region, Iraq from 1st of August, 2020 until 1st of August, 2021. Sixty Women diagnosed with abruption placenta from 24 weeks of gestation and more were included. Maternal information was collected by a designed questionnaire to determine the risk factors associated with abruptio placentae and the mode of delivery with fetal and maternal outcomes. Results: Sixty pregnant women with abruptio placentae were included in the study. The main risk factors for abruption placenta were previous cesarean section, gestational hypertension, preeclampsia and smoking. The rates of stillbirth and early neonatal death were 15% and 1.7% respectively. Newborns with low birth weight were 30%. Adverse maternal outcome for those delivered by emergency cesarean section was 22.7%, and (hysterectomy or other surgical procedures) was significantly high 22.7% among women aged ? 35 years. Perinatal mortality rate was significantly high when the delivery was with both emergency cs and vaginal delivery. The Fetal mortality was low 4.8% among neonates whose weight was 2.5 Kg and above, and it was high among neonates of less than 1 Kg 83.3%. Conclusion: Abruptio placenta associated with adverse fetal and maternal outcomes. Previous Caesarian section, gestational hypertension, and smoking are the main risk factors.","PeriodicalId":314832,"journal":{"name":"Advanced medical journal","volume":"115 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138599411","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It affects multiple aspects of health and can have a serious adverse effect on both the patients and their partners. Diabetes mellitus is a metabolic disorder associated with many chronic complications including erectile dysfunction. The aim of this study was to find out the prevalence and risk factors of erectile dysfunction in patients with type 2 diabetes mellitus. Patients and Methods: A cross-sectional study had been done on 100 adult male patients with type 2 diabetes mellitus, attending endocrinology outpatient at Erbil Teaching Hospital between June and December 2021, and another 50 age-matched non-diabetic controls. Each one of them underwent detailed history taking, clinical examination, and relevant biochemical study. Results: Sixty-three diabetic patients (63%) had erectile dysfunction compared to 6 nondiabetic subjects (12%). Mild, mild-to-moderate, moderate, and severe erectile dysfunction among these patients were 17 (27%), 21 (33.3%), 16 (25.4%), and 9 (14.3%), respectively. Among diabetics, erectile dysfunction was significantly associated with age, obesity, glycated hemoglobin level, duration of diabetes, presence of hypertension, dyslipidemia, and neuropathy (p= 0.001, 0.005, <0.001, 0.038, 0.02, 0.017 and 0.025 respectively) Conclusion: Erectile dysfunction was significantly more prevalent in patients with type 2 diabetes than in non-diabetic patients. Being older than 50 years old, obesity, glycated hemoglobin level higher than 9, diabetes more than 10 years duration, presence of hypertension, dyslipidemia, and neuropathy were significantly in favor of a higher prevalence of erectile dysfunction in this group.","PeriodicalId":314832,"journal":{"name":"Advanced medical journal","volume":"104 13","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138600029","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Hydatidosis, affecting various parts of human body and should be considered in the differential diagnosis of any cystic mass lesion. Osseous Hydatid cyst is rare; however, it involves the spinal column in half of the cases. In the current study, the epidural hydatid cyst had presented with spinal cord compression due to the formation of an epidural abscess formation. Methods: A young adult housewife lady who had been affected by the parasite, primary paravertebral hydatid cyst, between the seventh and ninth dorsal vertebral levels, presented with spinal cord compression due to epidural abscess lesion. Ultrasound, the contrast magnetic resonance, and computed tomography imaging are standard tests in the diagnostic work-up of the lesion and follow-up of the patient. Appropriate dorsal laminectomy, removal of the hydatid cyst, and the inflammatory epidural cystic compressing tissues were done. Results: Operative intervention had resulted in a dramatic improvement in the patient's condition and reversal of her neurological deficit back to normal; the paravertebral ectocyst size had eventually shrunken and there was no recurrence. The excised tissues that were compressing the dorsal spinal cord proved, Histopathologically, to be inflammatory in its microscopical appearance. Conclusions: Hydatid cyst epidural dumbbell extension may rupture and lead to the formation of an abscess, giving enhancement on contrast magnetic resonance and computed tomography scan studies. To our knowledge, this is the first case to be reported in the literature specifying spinal cord compression by an epidural abscess formation from a paraspinal dumbbell extension of hydatid cyst.","PeriodicalId":314832,"journal":{"name":"Advanced medical journal","volume":"87 20","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138600177","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study is to describe the cases of thrombotic microangiopathy after kidney transplantation. Patients and Methods: This retrospective study used the reports of all kidney transplant biopsies that were done in Kurdistan region from January 2017 to April 2022. The biopsy reports with diagnosis of Thrombotic microangiopathy were extracted from this total number and the patients, and pathological data included in the reports were recorded. Results: The total number of 1635 graft biopsies, of which 82 (5.01%) were found to have Thrombotic microangiopathy features. The mean age of studied patients with Thrombotic microangiopathy was (38.2 years) and male to female ratio as 2.3:1. We found 67.1% of cases of Thrombotic microangiopathy were associated with calcineurin inhibitor toxicity, 35.4% with rejection, 15.9% were recurrent thrombotic microangiopathy, and 40.2% were associated with other miscellaneous causes. Conclusions: Thrombotic microangiopathy after kidney transplantation is rare but debilitating complication. The cause is multifactorial in most cases in the light of coexistence of multiple risk factors in the kidney transplant recipients. Further studies are required to disentangle these overlapping risk factors. And allow for better prevention and treatment of this condition.","PeriodicalId":314832,"journal":{"name":"Advanced medical journal","volume":"43 2","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138598410","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The condition, when occurs, needs prompt surgical removal in order to avoid fatal consequences. The aim of this article is to report clinical data and our experience concerning cerebral hydatid cyst presentation, surgical management, complications, and outcome. Methods: A retrospective case-series study encompassing 8 patients, four females and four males, whose ages ranged between 3.5-35 years, harboring cerebral hydatid cysts, including one cerebral hydatid abscess. The current study was conducted at Duhok Teaching Hospitals, during January 2005-May 2021. The diagnosis was established on clinical and radiological, computed tomography and magnetic resonance imaging backgrounds, and confirmed by operative findings, and histopathology (abscess cyst). Via appropriate craniotomies, all cerebral hydatid cysts, and abscess, were successfully removed, except, the recurrent hydatid cysts which needed an additional posterior fossa craniectomy to achieve total removal. There was no need for the use of advanced statistical system. Results: Compared to their original neurological deficit at presentation, seven cases could achieve complete recovery, except, the patient with recurrence, whose recovery was delayed and suboptimal due to repeated surgeries; there was no mortality. Conclusion: The operating surgeon should practice meticulous care avoiding the possible risk of an accidental rupture during cyst delivery. The objective of this study was to assess the pregnancy characteristics and perinatal outcomes in women with decreased fetal movements. Methods: A case-control study was conducted on 489 pregnant women in Maternity Teaching Hospital, Erbil city from October 2021 to December 2022. They were assigned into two groups, cases consisting of 244 women with a perception of reduced fetal movements and a control group including 245 women with good fetal movements. perinatal outcome was compared between the two groups. Results: The majority (59.5%) of the study participants were term pregnancies (? 37weeks). Among women with RFM, 32.4% were nulliparous and 59% were multiparous comparing to control group (26.9% and 63.7%, respectively). A higher proportion of women with reduced fetal movements had gestational diabetes (34%), and preeclampsia (40.2%), compared to women of Group II with the rate of 15.5% and 21.2% respectively. The rate of cesarean section was significantly higher among cases than controls (45% vs. 24.9%, respectively). The rate of abnormal Doppler among Group I women was higher than in Group II women (26.2% versus 2.9%). Significantly higher rate of neonates in Group I had low APGAR score.
