In Kurdistan Region of Iraq three types of vaccines are available; Pfizer, AstraZeneca and SinoPharm. The objectiveof the study was to determine the effect of vaccine on severity and mortality rate among Covid-19 cases in Duhok, Iraq. Methods: In this Prospective study, 470 of hospitalized covid19 cases in Duhok, Iraqi Kurdistan between 1st june 2021 to 1st june 2022 were enrolled. Demographic parameters including age, sex, and past medical history of chronic diseases were taken. The severity of cases was assessed based on respiratory rate, oxygen saturation, and percentage of chest involvement by CT scan. The types and doses of vaccines received by patients were recorded. The patients were followed-up by the outcome (recovered and discharged from the hospital or dead). Results: The mean age of the patients was 64.98 years. The males were (51.49%, n=242) and females (48.51%, n=228). The majority of them did not receive any type of vaccines (81.49%, n=383). Most of the vaccinated patients received Pfizer vaccine. In term of Covid-19 severity, the study showed (65.75%, n=309) had mild-moderate disease and (34.26, n= 161) had severe disease. Moreover, the mortality rate was lower among vaccinated patients compared with unvaccinated patients 24.14% to 40.73% respectively. The study revealed that having the severe cases and being old were considered the main independent predictor of mortality among the patients (p value <0.0001). Early detection of the vascular changes with inexpensive and non-invasive techniques is important in the process of management of type 2 diabetes mellitus. The aim of this study was to determine the frequency and grading of carotid artery atherosclerosis using color doppler ultrasound in type 2 diabetes mellitus. Methods: A cross-sectional study was implemented in Radiology Department of Rizgari Teaching Hospital in Hawler city-Kurdistan Region during the period of six months from 1st of March to 1st of September, 2022. Sixty cases of type 2 diabetic patients included in the study and referred from Layla Qasim diabetic center and Rizgari Hospital Outpatient. Left and right carotid arteries (common, internal and bulb) were examined by ultrasound and intima-media thickness were measured, plaque, stenosis and occlusion were looked for. Patients more than 18 years old with type 2 diabetes mellitus included in the study and patients with recent stroke, type 1 diabetes and history of neck surgery were excluded. Results: The mean intima-media thickness of type 2 diabetic patients were higher in both right and left carotid artery bulb (65% and 73.3%) than common (6.7% and 21.7%) and internal carotid arteries (16.7% and 21.7%). The stenosis was observed in 21.7% in each right and left carotid bulb with less proportion of stenosis in common (0% and 1.7%) and internal carotid arteries (8.3% and 3.3%). There was a significant association between smoker and elderly age type 2 diabetic patients and carotid artery stenosis (p=0.03 and p=0.001 respectively). Conclusions: Diabetes mellitus is a common cause of atherosclerotic changes in carotid arteries mainly in the bulb and more on left side. Our goal was to investigate how Iron deficiency anaemia affects the haematological parameters and the psychosocial development of affected infants. Methods: A follow–up study was conducted from January, 2022 to September, 2022 at Heevi Paediatric Teaching Hospital in Duhok. A total of 58 children with Iron deficiency anaemia were involved in this study. Also, 58 children with normal iron status were recruited and served as control. The Ages & Stages Questionnaires®, Third Edition were used to evaluate the five areas of developmental progress of enrolled infants including gross motor skills, fine motor skills, adaptability, language skills, and social skills. Results: Study has found that Serum iron for patients with iron deficiency anaemia was (g/dL) 20.82± 1.1 vs 77.8 ± 5 for control. Generally, patients scored lower blood count compared to control.Hence,All haematological parameters including haemoglobin, mean cell volume, mean cell haemoglobin, mean corpuscular haemoglobin returned to normal after 2month treatment. Both groups scores in the communication and gross motor domains were not statistically significant communication: 40±0.6 for patients vs. 42± 0.5 for controls, and gross motor: 48± 1.7 for patients vs. 50± 1.5 for controls. Whereas, fine motor, problem solving, and social behaviour are other categories that were shown to have statistically significant low scores (p-value ? 0.05): fine motor for patients was 36±1.2 vs. 48±1.5 for controls, problem solving was 33±1.1 vs. 45±2.0 and social behaviour was 31.5% vs. 47.5% for the control group. The aim of this study was to assess the clinical characteristics, risk factors, outcomes and severity of heart failure with reduced ejection fraction in both genders. Patients and Methods: This is a prospective cohort study of 117 patients aged more than 18 years who had been admitted to the Hawler Cardiac Center or medical ward with heart failure with reduced ejection fraction for the period from December 2021 till 30th of October 2022. The clinical characteristics, severity of left ventricular systolic dysfunction and the outcomes over three months were compared between women and men. Results: One hundred seventeen patients were included in the study, 61 (52.1%) patients were male and 56 (47.9%) were female with male to female ration (1.09:1), p-value (0.017). Women presented with higher incidence of moderate left ventricular systolic dysfunction 36(64.2%), obesity 43 (76.8%) p-value=0.017*, dyslipidemia 27 (48.2%) p-value=0.038, pulmonary hypertension 4 (7.1%), atrial fibrillation 15(26.8%) and high cardiac hospitalization 12 (21.4%), while men have high incidence severe left ventricular systolic dysfunction 33(54.1% ) p-value=0.046, smoking 42 (68.9%); p-value <0.001 and ST-segment elevation myocardial infarction 28 (45.9%) p-value <0.001. the p-values off all above findings are less than <0.05 respectively Conclusions: There are major differences in risk factors, clinical presentation and outcomes between both genders. The virus's effects on pregnancy, the fetus, and the newborn are among the many unanswered questions.The study's objective is to assess the effects of COVID-19 during pregnancy on maternal and newborn outcomes. Methods: A cross sectional study design with retrospective information was used at the Obstetrics and Gynecology teaching hospital in Duhok city from 1st August 2020 to 1st August 2021. The study included 100 pregnant women all of whom were COVID-19-positive (by reverse transcription polymerase chain reaction) admitted to the hospital in the study period. Data were collected from patients’ medical files and case sheets including information about pregnancy, past history, delivery and intrapartum complication, and postnatal maternal and neonatal outcome. Results: The mean age of the pregnant women was 31.6 years ranging between 22-43 years. More than two thirds of them (70%) were multiparous. Nineteen percent of the involved women were admitted to intensive care units with a maternal death of 9%. Nearly, 60% of those pregnant women delivered by caesarian section. Neonatal outcomes were as follow, 29% preterm birth, 49.5% neonatal intensive care units admission, 9% intrauterine fetal death, 15.4% neonatal infection with Covid-19 and 5% neonatal death. Conclusions: Infection with COVID-19 during pregnancy could have adverse maternal and neonatal outcomes. Diabetic patients with low folic acid, high homocysteine levels, have a higher chance of developing coronary heart disease based on previous studies. Therefore, this study aimed to investigate the serum levels of folic acid and homocysteine in type-2 diabetes mellitus patients and their association with insulin resistance. Methods: This cross-sectional study has been conducted on 138 participants from February 2nd 2022 to August 3rd 2022, at Azadi teaching hospital/clinical biochemistry department- Duhok, Kurdistan region, Iraq. The participants were composed of two groups, 70 patients with type-2 diabetes mellitus, and 68 apparently healthy control. Both groups have been investigated for serum folic acid level, serum glucose, serum insulin, serum homocysteine, glycosylated hemoglobin, and Homeostatic Model Assessment for Insulin Resistance. Results: The mean age for type 2 diabetic patients was (51.24 years) with males predominant (64.29%). Type 2 diabetic patients showed a low level of mean serum folic acid concentrations (2.73 ng/ml) in comparison with healthy subjects (5.91 ng/ml) (p<0.001), and the mean serum homocysteine concentration was (13.493 ?mol/L) among type 2 diabetic patients compared to healthy subjects (10.041 ?mol/L) (p<0.001). Significant correlation was found between serum folic acid and serum homocysteine levels, and glycosylated hemoglobin (p<0.05 and p<0.001, respectively). The aim of this study to assess the frequency of programed death ligand-1 expression in renal cell carcinoma by using the immunohistochemistry and to correlate the results the clinicopathologic parameters. Methods: This is a cross sectional retrospective study performed in Duhok City from 2017-2022 on fifty-four formalin fixed paraffin embedded blocks of nephrectomy specimens diagnosed as renal cell carcinoma and collected from central lab of Duhok and some private labs. Two sections were prepared from each block, one section was stained with hematoxylin and eosin for histological analysis and the other one was used for immunohistochemical assessment of programed death ligand-1then the results were correlated with various clinicopathological parameters. Results: Programed death ligand -1; membranous expression was positive in 19 cases (35.2%) of renal cell carcinoma out of 54 cases. There was no significant correlation between programed death ligand -1 expression and age (p= 0.991), gender (p= 0.272), multifocality (p= 0.607), tumor size (p= 0.796), histological subtype (p= 0.107), tumor stage (p= 0.546), nuclear grade (p= 0.781), surgical margins involvement (p= 0.119) and lymphovascular invasion (p= 0.4), but there was statistically significant correlation with nodal metastases (p= 0.039). Those with transfusion-dependent thalassemia are more likely to experience cardiac problems because repeated blood transfusions cause iron accumulation in the cardiac myocyte. Thus, this study aims to determine the relationship between serum ferritin levels and echocardiographic findings in transfusion-dependent thalassemia patients. Methods: This retrospective analytical cross-sectional study was conducted on 131 patients with transfusion-dependent thalassemia between 1stDecember 2021 to 1stJuly 2022 at the Center for Thalassemia and Congenital Blood Disorder, Slemani, Iraq. The participant’s information, including sociodemographic characteristics such as age, gender, body weight, height, hemoglobin level, serum ferritin, type and duration of chelation therapy, and echocardiographic findings, were collected from the hospital database using a questionnaire. Then, the correlation between serum ferritin levels and echocardiographic results was determined. Results: The mean age of the patients was 21.24 ± 6.28 years, and most of them were males (55.7%), from the urban region (71%) and had an average body weight (68.7%). The mean serum ferritin level was 1876±1743.63 µg/L. The interpretation of the echocardiography results showed 38 patients (29%) had valvular heart diseases. Body mass index has a reverse linear relationship with ejection fraction(P=0.038) and a direct relationship with age(P<0.001). However, no significant association was found between echocardiographic findings and serum ferritin(P=0.83). Conclusions: We found valvular heart diseases among the most common heart disorders in the patients. The objectives of this study were to evaluate the expression of programmed death-ligand 1 and determine the association with tumour grade, stage, type (Papillary and non papillary), muscle invasion, age, and gender. Methods: This retrospective study was carried out from 2015 to 2022 on fifty-eight blocks belonging to patients with urothelial carcinoma, which were obtained from Duhok central lab and private laboratory. From each block, two sections were made; one of which was stained with Hematoxylin and Eosin for histological re-assessment and the other one for immunostaining for programmed death-ligand-1, which was calculated in tumour cells and inflammatory cells regardless of staining intensity and then the combined positive score was determined and considered positive when is more than 10. Result: This study included 58 cases of Urothelial carcinoma, Females represent 8 (13.79%) cases and males 50 (86.21%) cases, with a female-to-male ratio of 1/6.25. No statistically significant association existed between the histological subtypes and gender. Combined positive score was positive in (32.76%) of cases and negative (67.24%) cases. The association was significant with the stage of the tumor (p=0.03), muscle invasion (p=0.02) and highly significant with grade (p<0.001). Thus, we aimed to correlate between molecular subtype and the suitability of performing breast-conserving surgery. Methods: This retrospective cross-sectional study was conducted on 300 women with primary breast cancer, but without distant metastasis, from 2016 to 2022. These women had undergone either breast-conserving surgery or mastectomy. Patients were interviewed directly to obtain the necessary data, and their data were recorded on a questionnaire. Results: The most common age group at diagnosis was 45-50 years, and the mean age for menarche was 13.3±1.3 years. Of the patients, 9% were single, 15% were nulliparous, and 76.7% had practised breastfeeding. The most common type of cancer was invasive ductal carcinoma, with ductal carcinoma in situ (82%). The molecular subtype and type of surgery were significantly correlated (p=0.041). Of the sample, 66.7% underwent breast-conserving surgery, with the highest rate (87%) being done in the triple-negative molecular subtype. Additionally, there was a significant correlation between the tumour focality and the postoperative margin (p<0.001). Conclusions: Triple-negative cases were the most suitable for breast-conserving surgery among the molecular subtypes and had the highest rate of free margin.
