This study aims to determine the relationship between completeness of medical information and the accuracy of coding appendic diagnoses based on ICD-10 in medical records at the Kindergarten Hospital. III Dr. Reksowidiryo Padang in 2021. The type of research is correlational, the research is carried out at the Kindergarten Hospital. III Dr. Reksowidiryo Padang, the initial survey was carried out in February with field research on 18 to 22 June 2021, the total population in January-December 2021 was 110 along with a sample of 110 appendic medical record files, the study was conducted by direct observation, data analysis used namely bivariate and univariate. With the results of the study, it was found that the completeness of complete medical information was 42 (38.2%), the completeness of incomplete medical information was 68 (61.8%), the accuracy of the accurate diagnosis code was 45 (40.9%), the accuracy of the code was There were 65 (59.1%) inaccurate diagnoses, with p-value = 0.003 (p0.05), which means that there is a relationship between the completeness of medical information and the accuracy of coding for appendic diagnoses. The government has established the Social Security Administrative Body (BPJS) for Health through Law Number 24 of 2011 as a form of constitutional commitment to managing the implementation of public health insurance through the National Health Insurance. The lack of information and outreach regarding the Chronic Disease Management Program (Prolanis) in FKTP has resulted in the target of fulfilling the Prolanis Controlled Participant Ratio indicator not being achieved. The RPPB indicator at the Primary Clinic of the Semarang Ministry of Health Poltekkes in 2019 is still 10%. So to overcome this it is necessary to do a number of things, namely by providing information, outreach, and education regarding the types of Prolanis activities and their benefits and objectives. The better the level of information the participants have, the greater the tendency to use Prolanis. The purpose of this study was to develop a Prolanis Service Management Information System Application Prototype (SIMPELPRO) to Support the Achievement of Service Commitment-Based Capitation Indicators at Primary Health Facilities, the Primary Clinic, Poltekkes, Ministry of Health, Semarang. The research method used in this study uses descriptive research methods, with primary data sources, namely using interviews and observations of officers and doctors at the clinic, as well as secondary data from documentation and literature. For system design in this study, researchers will use the prototype method for system development including the stages of needs analysis, implementation, and testing. In addition to fulfilling nutrition for children, the immunization sector also shows the same thing. During the last 2 years from 2020-2021 the coverage of complete basic immunization for infants has fallen drastically. Researchers developed this application using the SDLC Waterfall model. The waterfall method has the advantage that when all systems can be fully and correctly defined at the beginning of the project, software engineering can run well without any problems. The data collection technique used was secondary data that had been previously collected by other parties using the internet, library research, and books related to research. The Child Health Record application is expected to be an application that can help parents control their children's health from an early age. The immunization feature in the application is expected to increase complete basic immunization coverage for children, increase parental awareness in fulfilling child nutrition, increase parental awareness of children's nutritional status and explore information related to knowledge about children's health. The use of the Child Health Record application can be used since children are 0 years old so that the monitoring process runs optimally. Dengan menerapkan teknologi didambakan bisa membuat kerja perekam medis lebih efektif dan efisien serta dapat mengurangi masalah yang ada. Penerapan sistem pendaftaran online dilakukan sebagai jalan keluar masalah pasien ditempat penerimaan pasien. Tujuan umumdari penelitian ini adalah  diketahuinya gambaran penerapan sistem pendaftaran online pasien rawat jalan. Sedangkan tujuan khusunya yaitu mengetahui prosedur (SPO) penerapan sistem pendaftaran online pasien rawat jalan, mengetahui penerapan  sistem pendaftaran online pasien rawat jalan, mengetahui kendala penerapan sistem pendaftaran online pasien rawat jalan di RS Yadika Kebayoran Lama. Metode penelitian yang digunakan adalah metode penelitian deskriptif. Dari hasil penelitian dilakukan penulis di bagian pendaftaran bahwa  waktu pelayanan pendaftaran rawat jalan mulai dari pasien mendaftar sampai mendapat nomor antrian mayoritas kurang dari lima menit. Presentase waktu pelayanan pendaftaran rawat jalan  ≥ 5 menit sebesar 4,21% sedangkan, presentase waktu pelayanan pendaftaran rawat jalan  ≤  5 menit sebesar 95,97%. Saran dari penulis sebaiknya mulai menyusun pembuatan SPO terkait pendaftaran pasien online. Referring to Permenkes no. 55 of 2013 article 2 which states that the administration of medical records must be carried out by medical recorders. Medical Recorder is a person who has passed the Medical Record and Health Information education in accordance with the provisions of the legislation.The purpose of this research is to know the management of medical records, and health information as well as the staff for managing medical records at the Srondol Health Center. This type of research is descriptive qualitative with a case study approach.The results showed that the Srondol Health Center provided health services to BPJS patients and general patients. The patient naming system at the Srondol Health Center is adjusted to ID cards, family cards and other identity cards. Medical record management activities experienced several obstacles because they did not have officers with a medical record education background so that the transfer of medical record media from manual to electronic was constrained. The writing of patient names (Mr, Ny, Son, Br) is not standardized. But since it's still newly developed, the application of the system in a newly applied EMR on a hospital is an interesting topic for review. Therefore the writer has chosen this topic to be the main issue on this research. This research main purpose is to find and identify the application of EMR in Rumah Sakit Setia Mitra. For this research, the writer is using a qualitative descriptive research, in which the data colletion is based on the observation done by the writer and also based on interviews that's done with the medical record staffs, and will be analyzed and compared with theories that already exist as the based accepted method on EMR and it's application. The results of the research done by the writers have found that EMR application at Setia Mitra Hospital is not yet been applied according with the theory that's aready exist, and the mains source of this difference is because the EMR on the hospital's information system is still under heavy development. Service quality is one of the obstacles in the implementation of SIMRS provided by service providers which has an impact on the use of information systems and SIMRS implementation. The purpose of this study was to determine the effect of service quality on the implementation of SIMRS using the HOT – Fit method. The sample used was 167 people in Hospital X, selected using probability sampling with disproportionate sampling technique. Results: (1) There is no effect of service quality on system usage (T Statistics is 0.162 and P Value is 0.871). (2) There is an effect of service quality on organizational structure (T Statistics is 2,309 and P Value is 0.021). (3) There is an influence of organizational structure on the use of the system (T Statistics is 2.092, P Value is 0.037). (4) There is no effect of system usage on SIMRS implementation (T Statistics is 0.899, P Value is 0.369). (5) There is an influence of organizational structure on the implementation of SIMRS (T Statistics of 4.293, P Value of 0.000). (6) There is no effect of service quality on SIMRS implementation through the use of the system (T Statistics of 0.111 1.96, P Value of 0.912 0.05 and) and there is an effect of service quality on SIMRS implementation through organizational structure (T Statistics of 2.152 1.96, P Value of 0.032 0.05).
