Kelengkapan informasi medis sangat penting dalam menunjang keakuratan kode diagnosis diabetes mellitus.Penelitian ini bertujuan untuk mengetahui hubungan kelengkapan informasi medis dengan keakuratan kode diagnosis Diabetes mellitus pada dokumen rawat inap.Penelitian ini merupakan penelitian analitik dengan pendekatan cross sectional. Sampel yang digunakan sebanyak 84 dokumen dengan teknik simple random sampling.Analisis data menggunakan Chi-Square.Persentase kelengkapan informasi medis sebesar 54,7% sedangkan ketidaklengkapan informasi medis sebesar 45,3%. Ketidaklengkapan tertinggi terdapat pada formulir ringkasan pulang sebanyak 26 dokumen (31%). Persentase keakuratan kode diagnosis Diabetes mellitus sebesar 29,8% sedangkan ketidakakuratannya yaitu sebesar 70,2%. Ketidakakuratan paling banyak disebabkan karena salah dalam penentuan tipe Diabetes mellitus yaitu sebanyak 24 dokumen.Uji statistik chi square menunjukkan bahwa p= 0,001.Kesimpulannya adalah ada hubungan antara kelengkapan informasi medis dengan keakuratan kode diagnosis Diabetes mellitus.Penulis menyarankan sebaiknya ada koordinasi antara petugas rekam medis dan tenaga kesehatan lainya untuk meningkatkan kelengkapan informasi medis,petugas coding lebih teliti dalam mengkode dan mengkonfirmasikan kepada dokter penanggungjawab apabila informasi yang dihasilkan kurang jelas. Abstra k Diabetes is a metabolic disease characterized by high blood sugar levels (hyperglycemia) which is caused by impaired insulin secretion and insulin resistance or both and is ranked sixth the largest cause of death in Indonesia. Completeness of medical information is very important in supporting the accuracy of the diagnosis code for diabetes mellitus. This study aims to determine the relationship between the completeness of medical information and the accuracy of the diagnosis code for Diabetes mellitus in hospitalization documents. This research is an analytical study with a cross sectional approach. The sample used was 84 documents with simple random sampling technique. Data analysis using Chi-Square. The percentage of completeness of medical information was 54.7% while the incompleteness of medical information was 45.3%. The highest incompleteness is in the 26 discharge summary (31%). Percentage accuracy of diagnosis code Diabetes mellitus is 29.8% while inaccuracy is 70.2%. The most inaccuracies are caused by incorrect in determining the type of Diabetes mellitus as many as 24 documents. Chi square statistical test showed that p = 0.001. The conclusion is that there is a relationship between the completeness of medical information and the accuracy of the diagnosis code for Diabetes mellitus. Subjek dari penelitian ini adalah petugas Rekam Medis, dokter, dan perawat. Instrumen penelitian yang digunakan adalah pedoman observasi  dan pedoman wawancara. Hasil penelitian ini menunjukkan bahwa dari aspek fisik bahankertas yang digunakan untuk formulir ini belum sesuai. Aspek anatomi adalah formulir ini tidak memiliki nomor edisi karena belum pernah direvisi dan belum adanya petunjuk pengisian formulir. Aspek isi, berdasarkan hasil wawancara ada beberapa item pada formulir yang perlu dihilangkan meliputi : golongan operasi, penyebab luar cedera dan morfologi neoplasma keracunan, dan pengobatan radio terapi/kedokteran nuklir dikarenakan pada penerapanya item tersebut tidak terpakai. Abstract [English Title : Anal ysisthe Design Of Inpatient ’s Admission And Discharge Summary Form O n Public Health Center Of Kutoarjo] The purpose of this research is analyzing the design of inpatient’s admission and discharge summary form on Public Health Center of Kutoarjo according to the aspect of anatomy, physical and content. This research uses descriptive method through case study approach. The object on this research is the form of admission and discharge summary on Public Health Center of Kutoarjo, in which the medical record staffs, the doctors, and the nurses are used as respondents.  In addition, the researcher also uses observation and interview as research instruments.The research result shows that according to the physical aspect, the material form used on Public Health Center of  Kutoarjo  is not appropriate to the theory. Then, according to the anatomical aspects, the form used on Public Health Center of Kutoarjo also do not have edition number, publication date, and instruction f orm. Kenaikan jumlah kunjungan mengakibtkan tertundanya beberapa berkas klaim BPJS. Penelitian ini dilakukan dengan tujuan untuk mengetahui prediksi kunjungan pasien rawat jalan tahun 2018-2022 di RSUD RAA Soewondo Pati. Penelitian ini termasuk penelitian deskriptif dengan pendekatan kuantitatif. Populasi dari penelitian ini adalah data kunjungan pasien dan pengunjung baru rawat jalan tahun 2013-2017 di RSUD RAA Soewondo Pati dan yang menjadi sampel adalah keseluruhan data populasi. Data diperoleh dari laporan rumah sakit pada rekapitulasi sensus harian rawat jalan. Berdasarkan hasil pehitungan, rata-rata kunjungan pasien rawat jalan per hari tahun 2013-2017 di RSUD RAA Soewondo Pati mengalami peningkatan 17% setiap tahunnya, prediksi rata-rata kunjungan pasien rawat jalan per hari tahun 2018-2022 mengalami peningkatan 9% setiap tahunnya, prediksi rata-rata kunjungan poliklinik per hari tahun 2018-2022 terdapat 22 klinik mengalami peningkatan, 2 klinik mengalami penurunan dan 1 klinik statis, prediksi rata-rata pengunjung baru rawat jalan tahun 2018-2022 mengalami peningkatan 9% setiap tahunnya dan trend kunjungan pasien dan pengunjung baru rawat jalan tahun 2018-2022 mengalami peningkatan setiap tahunnya meskipun angka peningkatan mengalami penurunan sebanyak 1% setiap tahunnya. Abstract (English Title : Prediction Of Outpatient Visits In RAA Soewondo Pati Regional Public Hospital Year 2018-2022) Based on the number of outpatient visits in 2013-2017 at RSUD RAA Soewondo Pati resulted in an average increase in the number of visits 17% each year. The increase in the number of visits caused in delays in some BPJS claim files. This research was conducted with the aim to know the prediction of outpatient visit year 2018-2022 in RSUD RAA Soewondo Pati. This research includes descriptive research with quantitative approach. Population of this research is data of outpatient visit and new outpatient visitor year 2013-2017 in RSUD RAA Soewondo Pati and become sample is all of the population. Information were obtained from hospital reports on daily ambulatory census recapitulation. Berdasarkan lima bangsal dengan cara menghitung indikator BOR, LOS, TOI, BTO dan digambarkan melalui Grafik BJ sebagai bahan evaluasi dan perencanaan. Tujuan penelitian untuk mengetahui efisiensi penggunaan tempat tidur 5 ruang unit rawat inap berdasarkan Grafik BJ di Rumah Sakit Bhakti Wira Tamtama tahun 2017. Jenis penelitian menggunakan metode penelitian deskriptif kuantitatif. Metode pengumpulan data menggunakan studi dokumentasi, perhitungan indikator efisiensi penggunaan tempat tidur, pengecekan efisiensi penggunaan tempat tidur. Hasil penelitian menunjukkan bahwa berdasarkan Grafik Barber Johnson keempat indikator kelima bangsal pada tahun tahun 2016 tidak bertemu dalam satu titik dan berada diluar daerah efisiensi. Sedangkan tahun 2017 keempat indikator bertemu pada satu titik namun berada diluar daerah efisiensi. Abstract [ English Title : Efficiency Degree Of Occupancy’s Bed By Barber Johnson Graph In Rs Bhakti Wira Tamtama ] Bhakti Wira Tamtama Hospital Semarang is a type C hospital that has the functions of health services, surgery, obstetrics and gynecology. Medical record-keeping officers have never done BJ Graph analysis and manufacture. Based on five wards by calculating BOR, LOS, TOI, BTO and drawing indicators through BJ Graph as evaluation and planning materials. The purpose of the research was to determine the wealth of bed usage of 5 units of inpatient units based on BJ Graph at Bhakti Wira Tamtama Hospital in 2017. This type of research used quantitative descriptive research method. Data collection methods using documentation studie, the calculation of bed usage efficiency indicators, checking the efficiency of bed usage. The results show that based on the Barber Johnson Chart the four indicators fifth of wards in the year 2016 do not converge in one point and are outside the efficiency area. Penataan ruang filing yang baik dibutuhkan untuk memudahkan pekerjaan petugas. Penataan ruang filing harus dilakukan berdasarkan aspek ergonomis dan antropometri petugas agar tidak menimbulkan risiko K3. Aspek ergonomi di ruang filing RS PKU Muhammadiyah Yogyakarta belum sesuai dengan antropometri petugas, hal tersebut dapat menyebabkan penurunan produktivitas kerja petugas. Tujuan penelitian ini adalah meninjau aspek ergonomi ruang filing berdasarkan antropometri petugas filing terhadap aspek K3 petugas . Jenis penelitian ini adalah penelitian studi kasus. Pengumpulan data dilakukan dengan metode observasi dan wawancara. Data dianalisis menggunakan analisa statistik yang bersifat deskripsi.Hasil penelitian menunjukkan bahwa pengelolaan, perlengkapan serta suhu dan kelembaban ruang filing RS PKU Muhammadiyah Yogyakarta sudah sesuai dengan teori kecuali untuk alat bantu pijakan. Ukuran rak penyimpanan rekam medis tidak ergnomis dibandingkan antropometri petugas filing. Namun tinggi dan lebar sub rak penyimpanan sudah sesuai dengan dimensi rekam medis.Adapun risiko K3 yang dapat dialami petugas filing antara lain kebakaran, terjepit roll o’pack , terjatuh dari alat bantu pijakan, kaki dan tangan pegal, nyeri tengkuk, gatal-gatal, luka akibat tergores, gangguan penglihatan, sesak nafas dan tenggorokan serak. Abstract (English Title :Review of Ergonomic Aspect of Filing Room Based on Filing Officers Anthropometry to the Occupational Health and Safety (OHS) in RS PKU Muhammadiyah Yogyakarta). The filing service is an important part of the Medical Record Unit, especially for medical record keeping. A good filing arrangement is required to facilitate the work of the officer. Spatial filingshould be done based on the aspect of ergonomic and anthropometry of the officers so it’s  not  causing OHS risk for the officers. E rgonomics a spects of filing room in RS PKU Muhammadiyah Yogyakarta not in accordance with anthropometry officers, it can lead to decreased work  / productivity officer. The purpose of this research is to review the aspect of ergonomic filing space based on the anthropometry of filing officer toward aspect of OHS.This type of research i ncluding to the case study research. Methods of data collection are observation and interview. Data were analyzed using statistical analysis which was description.The results showed that the management, equipment and temperature and humidity filing room in RS PKU Muhammadiyah Yogyakarta is in accordance with the theory except for the tool footing. The size of the medical record storage rack is not ergonomic compared to the anthropometry of the filing officer. Pada empat review yang dilakukan untuk menelaah formulir yaitu review identifikasi, laporan isi informasi, autentikasi, pencatatan tidak ada yang semuanya terisi lengkap. Tujuan dari penelitian ini yaitu melakukan analisis kuantitatif kelengkapan pengisian formulir persetujuan tindakan kedokteran di RSUD Ambarawa.Jenis penelitian yang digunakan adalah deskriptif. Sampel penelitian ini adalah formulir persetujuan tindakan kedokteran kasus bedah di RSUD Ambarawa pada triwulan 1 tahun 2018. Besar sampel dihitung menggunakan rumus Solvin dan didapat 96 sampel. Metode pengumpulan data yaitu observasi dan studi dokumentasi. Hasil Penelitian menunjukan bahwa review identifikasi pasien terisi dengan lengkap sebesar 99%. Review lsi informasi (laporan penting) terisi dengan lengkap sebesar 5%. Review autentikasi terisi dengan lengkap sebesar 46%. Review pencatatan tidak ditemukan kesalahan. Pengisian formulir persetujuan tindakan kedokteran kasus bedah di RSUD Ambarawa sebesar 4 % Abstract [English Title : Quantitative Analysis Completeness Form Informed Consent Case Surgery In RSUD Ambarawa] Based on a preliminary study that out of ten forms of intentional medical action taken completely randomly or at 100% incomplete. The purpose of this research is to conduct a quantitative analysis of completeness informed consent form at RSUD Ambarawa. The type of research used is descriptive. The sample of this research is the informed consent form of medical case at Ambarawa General Hospital in first quarter of 2018. The sample size is calculated using Solvin formula and obtained 96 samples. Methods of data collection are observation and documentation study. The results showed that the patient identification review was completely filled by 99%.
