Exclusive breastfeeding (EBF) refers to feeding the baby with breast milk until the end of six months after birth. Objectives. This study aimed to study EBF behaviour based on the theory of planned behaviour (TPB) in pregnant women in Isfahan in 2021. Material and methods. This cross-sectional study was conducted on 441 pregnant women in Isfahan with more than 20 weeks of pregnancy in 2021. The TPB constructs such as behavioural beliefs, outcome evaluation, normative beliefs, motivation to follow, control beliefs, perceived power, and behavioural intent on exclusive feeding behaviour were assessed. Sampling was simple randomised. We used an online questionnaire based on TPB constructs. The data was analysed with descriptive and analytic statistic. Results. The mean age of participants was 29.82 ± 5.138 years. There was a positive and significant relationship between the constructs of behavioural beliefs, normative beliefs, motivation to follow, control beliefs, perceived power ( p = 0.000) and outcome evaluation ( p = 0.002) with the intention of behaviour and an inverse relationship between these constructs and behaviour ( p < 0.05). However, there was a significant relationship between the number of children and the decision for the type of delivery ( p < 0.001) and the type of pregnancy ( p = 0.014) with the intention of EBF behaviour ( p < 0.05). The variables of behavioural beliefs, outcome evaluation, control beliefs and intention were selected as the most critical structures for predicting behaviour. According to this model, 29.1% of changes in EBF behaviour with these structures are also predicted. Conclusions. Based on the results, unplanned pregnancy and the decision for the type of delivery had a significant relationship with behavioural intention for EBF. Suboptimal pain control in neonatal intensive care units (NICUs) remains despite theoretical advances. Pay - ing attention to the views and suggestions of those who directly face the challenges of neonatal pain control in NICUs can help the improvement processes. Nurses are continuously present at patients’ bedsides and play a vital role in optimal pain management. Objectives. This study aimed to study nurses’ views of the quality of pain control in NICUs and the existing barriers. Material and methods. The cross-sectional, descriptive method was utilised to study 140 nurses that worked in three NICUs in Iran. They were selected using convenience sampling and filled out a questionnaire. The information was analysed using descriptive statisti - cal methods. Results. The results showed that although most participants had good knowledge of the basic concepts of neonatal pain, some of them had problems in some areas, such as using neonatal pain assessment tools. 40% of them thought that pain was not managed properly in the NICU. The participants also mentioned barriers to proper neonatal pain management, including work overload, inappropriate professional knowledge, lack of organisational structure, poor collaboration of team members and low pain prevention. Conclusions. It is necessary to improve the quality of caregiving with respect to pain management in the NICU. {"title":"Frailty and gender on mortality risk in elderly with coronavirus disease-19 (COVID-19): a meta-analysis","authors":"D. Daniella, I. G. P. Frailty was believed to reflect patients' prognosis better than age, but studies regarding the association between these factors are controversial. Moreover, studies highlighting the association of gender to mortality risk in frail patients are limited. Objectives. We aimed to investigate the association of frailty to mortality risk with the dose-response relationship of CFS and the association of gender to mortality risk in frail elderly with COVID-19. Material and methods We performed a comprehensive literature search from several databases, such as Europe PMC, PubMed and DOAJ on 9 July 2021. We searched for studies investigating the association between frailty and mortality in COVID-19 patient. Results A total of 16,438 patients from 15 studies were included. Frailty was found in 52.67% of the patients. The lowest mean age was 65.4 +/- 15.8 years. Pre-frailty (OR 2.07 [1.53-2.79];p < 0.00001;I-2: 72%), mild frailty (OR 2.24 [1.48-3.38];p = 0.00001;I-2: 80%), moderate frailty (OR 2.55 [1.75-3.71];p < 0.00001;I-2: 79%) and severe frailty (OR 3.57 [2.35-5.43];p < 0.00001;I-2: 83%) increase the mortality risk in elderly with COVID-19. Each 1-point increase in CFS increases the mortality risk by 1.4 [1.3-1.5];p = 0.000;I-2: 98.6%. Men had a lower risk of frailty (OR 0.58 [0.43-0.78];p = 0.0004;I-2=36%) but higher mortality risk. Conclusions This meta-analysis showed that pre-frailty and frailty increase the mortality risk in elderly with COVID-19. Each 1-point increase in CFS increased the mortality risk by 1.4.
