Culture at the microsystems level, where care is experienced and provided, directly impacts on staff and patients but is generally given much less attention than organisational cultures at the meso level. This paper presents a refinement of a previously published middle-range theory of culture change derived from a concept analysis of effective workplace culture. It draws on findings from a project that set out to embed a safety culture and grow quality improvement and leadership capability through a regional patient safety initiative in frontline teams across four acute NHS hospital trusts in south-east England. Aims and objectives: To refine theoretical understanding about how to recognise and develop effective workplace cultures at the microsystems level based on practical insights from the Safety Culture Quality Improvement Realist Evaluation (SCQIRE) project. Methods: The evaluation approach for the SCQIRE project combined realist evaluation and practice development methodology. Realist evaluation was selected to answer the question ‘what works for whom and why when embedding a safety culture, improvement capability and leadership in frontline teams?’ Key to this approach is the local development, testing and refinement of ‘CMO’ relationships between: contexts (C); mechanisms, for example triggers and explaining why components work (M); and outcomes (O). Drawing on project data, the enablers, attributes and consequences of an effective workplace culture have been used to critically examine the factors that contributed to frontline teams’ ability to create and sustain a safety culture. Findings: A total of 24 CMO relationships resulted in four emerging programme theories that described what worked, why and for whom in relation to: 1) frontline teams developing their safety culture; 2) facilitators working with frontline teams to embed safety culture, quality improvement and leadership; 3) organisations supporting frontline teams; and 4) the patient safety collaborative initiative. Conclusions: It is concluded that the close relationship between person-centred values, ways of working and continuing effectiveness mean it is not possible to develop a safety culture without also being person-centred in relationships. A newly designed living lab serves as a powerful learning environment, where students and older adults can work together in creating agefriendly services and products. Aim: The aim of this study is to gain insight into the shared experiences of older adults and students in co-creating age-friendly services in an educational living lab. Methods: The living lab and subsequently the research was informed by appreciative inquiry. The study has a qualitative, explorative and multistage design, with a six-stage alternating process of data collection and analysis. Data were collected from six individual interviews (three older adults and three students) followed by two group interviews with two other older adults and four other students. Data analysis resulted in a shared story of working together in the living lab. Findings: The co-creation experiences of older adults and students are highlighted in a rich and meaningful narrative. A shared narrative has been constructed from both perspectives, older adults and students. There are three central themes: working together, our learning experiences and our best experience. This rich and meaningful narrative stresses the importance of building a relationship to foster co-creation and uncertainty as beneficial to equality and shared responsibility. Conclusion: Co-creation in an educational context is meaningful to older adults and students. Appreciative inquiry in both the research and practice of the living lab can facilitate relationship equality, which is highly valued by participants. To promote such a family-centred culture and to shift the systemic approach to care from an individual one to a relational one, there is a need to integrate family nursing knowledge and skills into everyday acute care practices. \u0000Aim: The aim of this knowledge-to-action project was to create a family-centred care culture by integrating a family system nursing approach into clinical practice within the women’s health division in an acute care hospital. \u0000Methods: An action research methodology, using the action cycle of look-think-act, was used to guide knowledge translation and to develop a family-centred practice culture. The ‘think’ phase entailed an ongoing, critical dialogue among the study’s core group members about the meaning of family systems nursing, a shared definition and adaptation of the approach and instruments to the local context, and the development of an action plan for implementation. Lastly, family systems nursing was interprofessionally piloted through education and training activities in neonatal intermediate care. \u0000Findings: During the ‘look’ phase, 20 SWOT analyses with 312 health professionals revealed a shared commitment to individualised, participatory care, but a lack of continuity and inconsistent communication. Documentary analysis demonstrated that family engagement was evident in fewer than half of the cases. \u0000Conclusions: The use of a participatory, recursive process to tailor and introduce family systems nursing was important in order to gain momentum for change across different units and professions. However, there is a gap between patients’ needs and the capacity of nurses to meet the existing recommendations on provision of this care. Identifying the relevant barriers and facilitators can inform the development of strategies to support person-centred nursing. Aim: To identify registered nurses’ perceived barriers and facilitators in terms of providing personcentred end-of-life care. Method: A scoping review was conducted according to the 2005 guidelines proposed by Arksey and O‘Malley. The databases Medline, CINAHL, PsycINFO, Cochrane, Web of Science and Embase were searched using the period 2000 to 2018. Empirical studies, literature reviews and studies focusing on the experiences of generalist nurses providing end-of-life care were included. The selected articles were independently reviewed by two researchers. Results: A total of 2,126 publications were identified, with 26 retained after applying the eligibility criteria. Four barriers to providing person-centred care were identified: knowledge of end-of-life care; communication skills; coping strategies; and teamwork. Three main facilitators were identified: knowing the person in a holistic way; nurses’ self-knowledge; and the development of a good nurse-person relationship. Organisational and managerial support also emerged to be important. These findings are reflected in the construct of McCormack and McCance’s (2017) Person-centred Practice Framework. Conclusions and implications for practice: This review provides an overview and an understanding of the key elements that influence the provision of person-centred end-of-life care by registered nurses. These barriers and facilitators point to professional competencies that could improve nurses’ ability to provide this care. There is a need to develop methods to handle this complexity, and in this article we present a flexible and adaptable framework for mapping projects that focus on involvement of persons receiving care and other stakeholders, with an integrated support structure. The method also considers elements in the local context. Aims: To present examples of project mapping, and to explore how the process can enhance quality in complex projects in healthcare services. Methods: The project mappings have been co-designed in processes of deliberate dialogue between the authors of this article, with involvement from other researchers and stakeholders. A threedimensional version of project mapping was developed, and further refined by introducing a twodimensional version and testing the framework in various settings such as a project leader course and the 2018 Enhancing Practice Conference in Basel. Analysis continued through the whole process as preliminary ideas were discussed and documented. We reflected, wrote notes, talked to people, took part in workshops that included a variety of creative methods, and did a qualitative content analysis of key findings to develop themes. Results: The examples of project mapping show that the process of mapping is as important as the map itself. The maps are flexible and can be combined. Project mapping can contribute to quality in projects by helping project facilitators and participants to stay on track. It can also enable co-creation and guide facilitation processes. Conclusion: Engaging in mapping processes represents an approach that can contribute to a shift in thinking and help even out power imbalances between project participants, as well as influencing the working culture in a health service. Mapping can facilitate transformation of practice while simultaneously creating new knowledge about that transformation. The principles of practice development and person-centred practice informed our ways of working. This reflection was undertaken by unpacking shared assumptions, using Mezirow’s transformative model of critical reflection. Aim: The aim of this reflection was to share the experiences of an academic team in collaboratively designing face-to-face learning opportunities for a masters of nursing subject and to invite others to consider this valuable way of working. Conclusions: Three conclusions were realised within the reflection. First, meaningful connections can be established in the virtual space through the development of authentic relationships. Second, working collaboratively and feeling valued for our contributions can lead to a meaningful investment in time preparing to facilitate learning.
