Management at these facilities understands the high liability involved with residents' safety, but they do not perform formal analyses to mitigate the risk. Failure Modes and Effects Analyses (FMEAs) have become a very popular method to mitigate the risk involved with health care products [1–2], but can also be extended to analyze an entire process at a residential care facility which provides older adults the high quality residential living and healthcare services they need. Successful supply chain management depends on tight coordination between supply chain partners. The Internet provides the perfect communications channel for the information, decisions, transactions, and processes that form supply chain management. It is a journey of change that is already under way. To keep up with the evolution of supply chain management, and remain competitive in their markets, future supply chain leaders will need to simultaneously exhibit tight coordination in all areas: integration, collaboration, and synchronization with a better understanding of communications systems function. Integration is the coordination of supply chain activities such as purchasing, manufacturing, distribution, spares management needed for maintenance and operational availability of military systems, and customer service within a single enterprise of technological readiness. Many leading companies have made great strides with internal integration of their communication systems. This is just a beginning. If AST is incorporated early in the lifecycle, large gains in reliability and lower warranty repair costs can be expected. These in turn create greater customer goodwill and strengthen a company's position within its respective industry. The initial capital investment that is required to perform AST can seem costly, however, the cost required to increase a product's reliability once it has been fielded can be orders of magnitude greater than the initial investment to implement AST. Also, the Markov-based methods typically assume exponential time-to-failure distributions for the system components. In this paper we propose a new combinatorial approach to handling functional dependence in the reliability analysis of imperfect coverage systems. Based on the total probability theorem and the divide-and-conquer strategy, the approach separates the effects of functional dependence and imperfect fault coverage from the combinatorics of the system solution. The proposed approach is efficient, accurate, and has no limitation on the type of time-to-failure distributions for the system components. Understanding the operational and failure behavior of crewed space launcher propulsion systems is critical to the characterization of integrated vehicle risk. The analysis of threats posed by the failure of these systems to the mission, vehicle and crew is informed by a study of off-nominal behavior. To better answer questions on the failure behavior of rocket engines, it is essential to study the underlying physics. To this end, the development of simulation tools to aid the risk analyst in the study of system level behavior is elucidated. ROCETS (ROCket Engine Transient Simulator) simulation code was used to investigate the steady state and transient physical behavior of cryogenic rocket engines for nominal and off-nominal operating conditions.
