A 47 +o year old Para o lady presented to the Accident and Emergency Unit of our hospital with a two(2) day history of a huge mass protruding per vagina and a history of significant bleeding per vagina. Packed Cell Volume was 19% on admission and she had Four(4) units of blood transfused. She subsequently had examination under general anaesthesia and cervical myomectomy was done. Her Post Operative clinical state was satisfactory. Ewoye","doi":"10.4314/rejhs.v12i1.10","DOIUrl":"https://doi.org/10.4314/rejhs.v12i1.10","url":null,"abstract":"Background: Optic nerve gliomas are rare tumors that constitute less than 5% of all pediatric central nervous system tumors. \u0000Case Presentation: We present the case of a 5-year-old girl with complaints of progressive protrusion of the right eye and poor vision of 3 years duration who was referred to the ophthalmologist. Ophthalmological evaluation of the right eye revealed tearing, no perception of light, inferior dystopia, severe proptosis, and a firm non-tender orbital mass palpable posterior to the eyeball. The cranial CT-scan was suggestive of a right optic nerve glioma. Histology showed Optic Glioma with extensive proliferation of meningothelial cells. \u0000Conclusion: Optic gliomas can be associated with extensive proliferation of meningothelial cells that can histologically mimic a meningioma. It is important to be cautious about making a diagnosis of meningioma for tumors in locations that are also characteristic of optic nerve gliomas especially in appropriate clinical settings. \u0000 \u0000 \u0000French title: Rapport de cas : Un rapport de cas de gliome du nerf optique chez une fillette africaine de 5 ans Titre en cours - Gliome optique chez une fillette africaineContexte de l'étude : Les gliomes du nerf optique sont des tumeurs rares qui constituent moins de 5 % de toutes les tumeurs pédiatriques du système nerveux central. \u0000Présentation de cas : Nous présentons le cas d'une fillette de 5 ans présentant des plaintes de protrusion progressive de l'œil droit et une mauvaise vision depuis 3 ans qui a été référée à l'ophtalmologiste. L'évaluation ophtalmologique de l'œil droit a révélé un larmoiement, une absence de perception de la lumière, une dystopie inférieure, une exophtalmie sévère et une masse orbitaire ferme et non douloureuse, palpable en arrière du globe oculaire. Le scanner crânien était évocateur d'un gliome du nerf optique droit. L'histologie montrait un gliome optique avec une prolifération étendue de cellules méningothéliales. \u0000Conclusion : Les gliomes optiques peuvent être associés à une prolifération étendue de cellules méningothéliales pouvant mimer histologiquement un méningiome. Il est important d'être prudent avant de poser un diagnostic de méningiome pour des tumeurs situées dans des localisations également caractéristiques des gliomes du nerf optique, en particulier dans des contextes cliniques appropriés. The aim of this study was to evaluate the prevalence of rifampicin resistance in paediatric tuberculosis in Babcock University Teaching Hospital, Ilishan Remo, Ogun State, Nigeria \u0000Methodology: This was a retrospective study that involved a review of medical microbiology laboratory records to analyze GeneXpert results of sputum samples obtained from pediatric patients with tuberculosis between January 2017 and March 2022. Aconvenience sampling method was used to select cases who met the study's inclusion criteria until the sample size was attained. \u0000Results: The medical laboratory records of 1046 subjects were analyzed in this study of which 556 (53.2%) were males. The mean age of all the patients was 10.77±4.38 years and majority of the respondents 445 (42.5%) were in the age group 11-15 years. This study therefore aimed at determining the correlation between hearing thresholds and habitual usage of headphones/earphones among undergraduates of tertiary institutions in Northwestern Nigeria. \u0000Method: A cross-sectional study was conducted on young adults, who are undergraduate students of tertiary institutions, to assess the hearing thresholds of individuals who frequently use headphones or earphones. Ethical clearance was obtained, and participants who met specific criteria were selected. Amultistage sampling technique was used, and 272 participants (representing 544 ears) with equal number of controls were recruited. They completed questionnaires and underwent otoscopic examination and pure tone audiometry (PTA). Those having wax or foreign bodies in the ears had it removed before performing PTA. The data was recorded and analyzed using SPSS version 20. Frequency tables and variable relationships were examined to determine significance, which was set at a p-value of <0.05. \u0000Result: The study involved 272 users of headphones/earphones (representing 544 ears) and an equal number of age- and sex-matched controls. The participants' ages ranged between 18 and 33, with 50.4% being male. The pure tone average among the headphones/earphones users and the controls ranged from 11.0 – 43.8dB, with a mean of 20.2±6.0 and 10.6 – 43.2dB, with a mean of 19.6 ± 4.5, respectively. More than half, 280/544 ears (51.5%) of the participants used headphones/earphones for over 4 years, and of these, 222/280 ears (79.3%) used them for 1 – 4 hours daily. Moderate volume was the preferred listening mode for 63.2% of the users. A moderate positive correlation was observed between hearing thresholds and hours of headphone/earphone use, and a weak positive correlation between hearing thresholds and preferred level of loudness (r = 0.290, p < 0.0001) while no correlation was found between hearing threshold and long-term use (r = 0.003, p = 0.937). \u0000Conclusion: This study suggests that individuals who use headphones or earphones for prolonged periods of listening hours daily or at higher listening levels tend to have poorer hearing thresholds. We sought to determine the pattern of purely medical emergency cases seen over a 6 months' period in our hospital. \u0000Methods: It was a cross-sectional descriptive study involving all patients treated in the medical ED at University of Benin Teaching Hospital, Benin city over a six month period (August 2019-January 2020). An interviewer administered questionnaire was used to collect data on sociodemographic characteristics, presenting complaints, diagnosis, sub-specialty involved in management and outcome (discharge, admission, referral or death). \u0000Results: A total of 875 patients were recruited for the study. There were 410(46.9%) males and 465(53.1%) females. The mean age was 57.8±19.94 years (range: 17 - 107 years). The top 10 clinical diagnosis included stroke 146(16.7%), chronic kidney disease 59(6.7%), diabetic emergency 59(6.7%), pneumonia 57(6.5%), congestive cardiac failure 53(6.1%), type- 2 Diabetes mellitus with chronic complications50(5.7%), malaria49(5.6%), retroviral disease 33(3.8%), hypertensive emergency29(3.3%) and Tuberculosis28(3.2%). \u0000Conclusion: Neurological and infectious diseases were the most frequent reason for consultation. Stroke was the singular most common diagnosis in the medical emergency. The study aims to compare the clinical efficacy of two perioperative/postoperative antibiotic regimens, the wound outcome and difference in cost on healthcare between these regimens. \u0000Methods: This is an observational prospective cohort study of 290 women. Women scheduled for elective surgeries in the department were grouped to have either a combination of ampicillin/cloxacillin (Ampiclox), Gentamicin, and Metronidazole (triple regimen) or a combination of ceftriaxone and metronidazole (double regimen). Data was analysed using Statistical Package for Social Science (SPSS) Windows version 21. The wound outcome and the cost implication of the antibiotic regimen were compared by chi-square test and t-test as appropriate. A p-value less than 0.05 was considered significant. \u0000Results: The postoperative wound infection rate in this study was 7.24%. There was no statistical significant difference in postoperative infection in the triple regimen group compared to the double regimen group. However, there was a significantly longer duration of stay in the hospital in the triple regimen group (P <0.05). There was also a statistical significant difference in the cost of hospital stay and the cost of antibiotics in the triple regimen group (P<0.05). There was no statistically significant difference in the pattern of organisms cultured in both groups. \u0000Conclusions: Ceftriaxone/Metronidazole regime of antibiotics showed superiority over ampicillin/cloxacillin, Metronidazole, and Gentamicin in the prevention of postoperative wound infection, hospital stay, and reduced cost of healthcare. \u0000 \u0000French title: Prophylaxie antibiotique dans les procédures obstétricales et gynécologiques : une étude comparative entre deux schémas thérapeutiques antibiotiques \u0000Contexte de l'étude: Les infections postopératoires des plaies ont un impact énorme sur la qualité de vie et contribuent considérablement au coût financier des soins aux patients. Nous avons cherché à déterminer la répartition des cas d’urgence purement médicale vus sur une période de 6 mois dans notre hôpital. \u0000Méthodes:Il s'agissait d'une étude descriptive transversale portant sur tous les patients traités à l'urgence médicale de l'hôpital universitaire du Bénin, ville de Benin, sur une période de six mois (août 2019-janvier 2020). Un questionnaire administré par un intervieweur a été utilisé pour collecter des données sur les caractéristiques sociodémographiques, la présentation des plaintes, le diagnostic, la sous-spécialité impliquée dans la gestion et les résultats (sortie, admission, référence ou décès). \u0000Résultats: Au total, 875 patients ont été recrutés pour l'étude. Il y avait 410 (46,9 %) hommes et 465 (53,1 %) femmes. L'âge moyen était de 57,8 ± 19,94 ans (extrêmes : 17 - 107 ans). Ewoye","doi":"10.4314/rejhs.v12i1.10","DOIUrl":"https://doi.org/10.4314/rejhs.v12i1.10","url":null,"abstract":"Background: Optic nerve gliomas are rare tumors that constitute less than 5% of all pediatric central nervous system tumors. \u0000Case Presentation: We present the case of a 5-year-old girl with complaints of progressive protrusion of the right eye and poor vision of 3 years duration who was referred to the ophthalmologist. Ophthalmological evaluation of the right eye revealed tearing, no perception of light, inferior dystopia, severe proptosis, and a firm non-tender orbital mass palpable posterior to the eyeball. The cranial CT-scan was suggestive of a right optic nerve glioma. Histology showed Optic Glioma with extensive proliferation of meningothelial cells. \u0000Conclusion: Optic gliomas can be associated with extensive proliferation of meningothelial cells that can histologically mimic a meningioma. It is important to be cautious about making a diagnosis of meningioma for tumors in locations that are also characteristic of optic nerve gliomas especially in appropriate clinical settings. \u0000 \u0000 \u0000French title: Rapport de cas : Un rapport de cas de gliome du nerf optique chez une fillette africaine de 5 ans Titre en cours - Gliome optique chez une fillette africaineContexte de l'étude : Les gliomes du nerf optique sont des tumeurs rares qui constituent moins de 5 % de toutes les tumeurs pédiatriques du système nerveux central. \u0000Présentation de cas : Nous présentons le cas d'une fillette de 5 ans présentant des plaintes de protrusion progressive de l'œil droit et une mauvaise vision depuis 3 ans qui a été référée à l'ophtalmologiste. L'évaluation ophtalmologique de l'œil droit a révélé un larmoiement, une absence de perception de la lumière, une dystopie inférieure, une exophtalmie sévère et une masse orbitaire ferme et non douloureuse, palpable en arrière du globe oculaire. Le scanner crânien était évocateur d'un gliome du nerf optique droit. L'histologie montrait un gliome optique avec une prolifération étendue de cellules méningothéliales. \u0000Conclusion : Les gliomes optiques peuvent être associés à une prolifération étendue de cellules méningothéliales pouvant mimer histologiquement un méningiome. Il est important d'être prudent avant de poser un diagnostic de méningiome pour des tumeurs situées dans des localisations également caractéristiques des gliomes du nerf optique, en particulier dans des contextes cliniques appropriés.
