The estimated global cost for dementia is about USD 1 trillion by 2018. In terms of healthcare utilization, the elderly with dementia are more likely to be hospitalized compared to their peers without dementia since dementia increases fall risk, injuries, infections and complications due to chronic diseases and drugs. During the hospital stay, the elderly patients with dementia are more likely to develop iatrogenic complications like delirium, falls and infections, with higher mortality is higher and longer length of stay.

The present study is conducted to assess the pattern of pain among the older adults and its interference on their daily activities. \u0000Methods: In this cross-sectional descriptive study, a total of 150 community dwelling older people aged 60 years and above were included from Bharatpur municipality of Chitwan district. Data include sociodemographic characteristics, pain pattern, its severity and impact. The pain intensity and degree to which pain interfered with daily activities of older people in the preceding one week was measured by using Brief Pain Inventory (BPI) on 0-10 numeric rating scale. \u0000Results: The prevalence of physical pain among the older adults was found 40.7% with median age of 66 years (IQR: 62-72 years) and presence of pain was mainly associated with their predominant diseases or injuries. Burning type of pain was mostly apparent in this group and majorities were complaining pain in their legs having joint problem. In the preceding one week, the mean intensity of pain at its worst time was found severe (6.67 ± 1.85). There was a highest interference of pain on sleep (r=0.621, p<0.001) of elderly population. Also, significant but moderate positive impact of pain at its worst was on enjoyment of life (r=0.463, p<0.001) and mood (r=0.377, p=0.003) of the elderly people. \u0000Conclusion: The findings of this study indicate the need for better programmatic efforts including psychological consultation and multidimensional pain assessment in this population.

Considering that caring for a person with dementia affects the health of caregivers, the aim of this study is to check the degree of psychological adjustment in a sample of 51 elderly family caregivers of people with dementia (31 women and 20 men), and test the relationship between their psychological adjustment and their physical, psychological and emotional health. One of the main challenges faced by many researchers is determining the reasons why individuals choose or refuse housing transition. In this pilot study, a housing demonstration unit is used as a test bed for a sample of fifteen older adults approaching the crucial problem of relocation. The demonstration unit consists of a prototypal eco-friendly sustainable housing unit of 55 square meters, equipped with smart home technologies. Overall, data collected through a questionnaire draw a positive picture of the older adults’ perceptions on this alternative housing solution. Respondents seem to consider home security, customizable options, and smart technology features as the most important factors influencing their decision to relocate. Results demonstrate the potential of this housing demonstration unit for (1) documenting how joint efforts among cross sectoral partnerships can work together to support sustainable new option in the emerging sector of housing; (2) offering a real example of alternative housing solutions for older adults facing the decision to remain at home or relocate; (3) offering a study to replicate in order to search those evidences that are urgently needed in literature. These findings might be a good starting point to encourage discussion for policy-making and employ practicable changes for the housing sector.

Most older women (80% to 85%) have hormone receptor positive breast cancer and thus the use of endocrine therapy is common in this population. Typically, women receive endocrine therapy for at least 5 years to reduce the risk of cancer recurrence and decrease breast-cancer mortality. However, adherence to hormonal therapy may be an issue, particularly in older women, as side effects may lead to early discontinuation and/or non-adherence. Older women on endocrine therapy are often cared for by primary care physicians and geriatricians and therefore, an understanding of oral endocrine therapy, as well as its potential side effects, is essential for treating physicians. This review describes the benefits of endocrine therapy in patients with resected breast cancer, compare different hormonal therapies with regards to side-effects and discuss management strategies with a particular focus on the concerns of older breast cancer patients on endocrine therapy. Mrs. Smith is a 75-year-old woman whose family doctor noted a 3 cm to 4 cm lump in her left breast during a routine physical exam. She has no palpable lymph nodes. Her family doctor sends her for a mammogram which reveals a 2.6 cm spiculated mass in the left upper outer quadrant. Biopsy reveals an invasive ductal carcinoma.
