Neurosarcoidosis, cardiac sarcoidosis, and pulmonary fibrosis in sarcoidosis are the forms of sarcoidosis with the highest mortality rate, and with the lowest response rate to the current therapy. A group of experts in the field of sarcoidosis, supported by the European Respiratory Society, created practice guidelines in the treatment of the most common forms of sarcoidosis, and provided suggestions for further research so as to create new therapeutic protocols. The evidence was obtained by reviewing literature. Rapid diagnosis and adequate medical intervention provide a good outcome, which is not possible in all countries. Any bleeding that occurs in the second half of pregnancy raises the suspicion of placental abruption, which is associated with fetal and maternal morbidity and mortality. The clinical picture and consequences for the fetus depend on the degree of bleeding and the amount of blood. Complete abruption leads to the disruption of fetal circulation and oxygenation, asphyxia, and stillbirth. In case of partial abruption, the consequences for the fetus depend on the degree of bleeding. In 2020, there were 1.3 million pregnant women infected with HIV worldwide. Untreated HIV infection in pregnancy carries an increased risk of maternal and perinatal morbidity and mortality. Perinatal transmission of HIV refers to the transmission of the virus from mother to child during pregnancy, childbirth, or puerperium. The aim of this paper is to present the up-to-date information on diagnosis, treatment, and monitoring of pregnancy in HIV-positive women, with reference to the time and mode of delivery, in order to prevent the transmission of the virus from mother to fetus. The main goals of monitoring and treating HIV infection during pregnancy are the following: preventing the transmission of the virus to the fetus, preserving the mother's health, and providing conditions for a safe delivery. The goal of treatment is to maintain the lowest level of the virus in the blood, especially at the time of delivery, in order to reduce the risk of vertical transmission. It is recommended to start treatment of HIV infection before pregnancy. Also, regular monitoring of the viral load, the CD4+ lymphocyte count, and blood count, as well as performing liver and kidney function tests, is necessary. The choice of the time and mode of delivery should not differ in pregnant women with a low viral load, as compared to healthy pregnant women, while in pregnant women with a high viral load, the pregnancy should be ended with elective caesarean section. HIV infection affects both the pregnant woman and the fetus. Therefore, antenatal, intrapartum, and postpartum monitoring is important. To date, no clear relationship has been established between levels and types of autoimmune antibodies and clinical manifestations of APS, which can range from mild coagulation disorders to life-threatening conditions. Aim: The aim of this study is to examine the relation between antiphospholipid antibody type and titer and the most common clinical manifestations of APS. Materials and methods: The retrospective study included 32 patients with a confirmed laboratory finding of elevated antiphospholipid antibodies, who came for follow-up examinations to the Hemophilia Unit of the University Clinical Center of Serbia, between June 1, 2017 and December 31, 2018. Data on patients were taken from their medical records. Basic demographic data, type and titer of antiphospholipid antibodies, and their association with the present clinical manifestations of APS were analyzed using standard statistical methods. Results: There was no significant difference regarding the frequency of positive results for lupus anticoagulant, anti-cardiolipin, and anti-beta-2-GP-I antibodies, between the symptomatic and asymptomatic group. The percentage of persons with simultaneous positivity for two or all three antiphospholipid antibodies was the same in both groups of subjects. Conclusion: As opposed to previous studies, our study did not demonstrate a correlation between the titer of antiphospholipid antibodies and the clinical manifestations of APS. Symptomatic and asymptomatic patients did not significantly differ in the frequency of elevated antibodies. Antibiotic prophylaxis (AP) in surgery is an important measure that contributes to the prevention of surgical site infection. International and national guidelines provide clear recommendations for the proper implementation of antibiotic prophylaxis. However, many studies indicate poor compliance in many health care centers. Aim: Our study aimed to determine the level of compliance in the application of antibiotic prophylaxis in abdominal surgery with the guidelines. Materials and methods: The retrospective study included all patients which were operated on at the Clinic for Digestive Surgery of the University Clinical Center of Serbia (UCCS), during the period January - March 2019 (270 patients). Medical records provided information about the type of surgery, the choice of antibiotic, the time of application, the route of administration, the need for redosing, the duration of antibiotic prophylaxis administration, as well as the presence of drug allergies. The collected data were analyzed by descriptive and analytical statistics (ch2 test). Results: During the study period, 270 patients were operated on at the Clinic for Digestive Surgery, of whom 227 (84.1%) received antibiotic prophylaxis. Cefazolin, an antibiotic recommended for most abdominal surgeries, was administered to only 17 (6.3%) patients. The majority of patients (64.4%) received antibiotic prophylaxis within 30 minutes of the beginning of surgery. The duration of antibiotic prophylaxis of up to 24 hours after the operation was applied in 13.1% of patients. According to DNA sequences, this virus can be classified into six genotypes. In hematological patients enrolled in allogeneic hematopoietic stem cell transplantation (HSCT) programs, it can lead to hemorrhagic cystitis. Aim: The aim of this study is calculating the prevalence of BK polyomavirus PCR (polymerase chain reaction) positivity in the blood and urine of patients involved in allogeneic HSCT, determining the predictive factors for clinical presentation of BK polyomavirus-associated hemorrhagic cystitis, as well as determining its effects on overall survival (OS) of the patients. Materials and methods: This retrospective cohort study enrolled 42 patients from the Clinic of Hematology of the University Clinical Center of Serbia. The presence of the virus in blood and urine was determined by the PCR method. The survival rate of the patients in relation to hemorrhagic cystitis was calculated with the Kaplan-Meier method and comparison was performed with the log-rank test. Results: A positive PCR result in the blood was found in 97.6% of the subjects, while urine tested positive in 100% of patients. The estimated survival time in patients without hemorrhagic cystitis was 44.357 months, while the group with the clinical presentation of hemorrhagic cystitis had an estimated survival time of 17.395 months. Based on the log-rank test, we found a significant difference in survival between those groups of patients (p = 0.049). With regards to leukocyte engraftment day, patients engrafted after D+14, had a higher frequency of hemorrhagic cystitis (p = 0.037). This group includes thrombotic thrombocytopenic purpura and hemolytic-uremic syndrome. One of the causes of thrombotic microangiopathy, although rare, can also be malignant diseases, primarily cancers of the stomach, prostate, breast and lungs. Case report: We present a patient with thrombotic microangiopathy and disseminated malignant disease of unclear etiology. Conclusion: This clinical entity is called carcinoma-associated thrombotic microangiopathy and was first described in 1970. However, only a few dozen cases have been described to date. Bogosavljevic, Dragan Vasin, Milica Mitrovic-Jovanovic","doi":"10.5937/smclk3-35705","DOIUrl":"https://doi.org/10.5937/smclk3-35705","url":null,"abstract":"Introduction: Emphysematous pyelonephritis is a rare, acute, and life-threatening necrotizing renal infection caused by gas-producing facultative anaerobes, such as Escherichia coli, Klebsiella, and Proteus, most commonly in the setting of underlying diabetes mellitus. Pneumomediastinum is a rare imaging finding in this disease, and a high index of suspicion is required for diagnosing emphysematous pyelonephritis in patients presenting with pneumomediastinum, in the right clinical setting. Case presentation: We present the case of a middle-aged female patient with symptoms and laboratory findings indicating infection, renal failure, and hyperglycemia, and a personal history of diabetes mellitus. The imaging procedures revealed findings of a severe and advanced necrotizing renal infection with parenchymal destruction and intraparenchymal gas collections extending into the perirenal and pararenal spaces. We established the diagnosis of emphysematous pyelonephritis accompanied by perirenal abscess formation and pneumoretroperitoneum, with gas collections propagating into the mediastinum. In the setting of such a severe form of infection, the patient underwent immediate surgery with left nephrectomy. Despite prompt and intensive treatment, the patient unfortunately succumbed to the disease during the postoperative period. Conclusion: Pneumomediastinum is a rare complication of retroperitoneal processes, and, when present, indicates their extensiveness, often being an ominous prognostic sign. The system of dental health care in the Republic of Serbia requires changes in the field of legislation and the organizational process, as well as financial investments for the purpose of improving the organizational process and achieving a greater number of professional staff that would provide dental services. The McKinsey 7S Model is a good tool for checking the managing abilities and the capacity of a certain organization to respond to the modern demands of its users. Aim: The aim of the paper was to investigate the prospects of improving the current dental health care management system. Methods: The method involves a case study on applying the McKinsey 7S diagnostic model to a private dental practice.
