Primary erythrocytosis - polycythemia vera, is characterized by increased erythrocyte production, due to a disorder at the level of the multipotent stem cell in the bone marrow. On the other hand, secondary erythrocytosis (SE) is the result of bone marrow stimulation by an external factor. Aim: The aim of our study was to determine parameters which are significant in differentiating SE from primary erythrocytosis - polycythemia vera (PV). Materials and methods: This is a retrospective study involving 108 patients with SE and 111 patients with PV, who were diagnosed and treated at the Clinic of Hematology of the Clinical Center of Serbia (CCS), in the period: December 2005 - November 2018.From the patient records, the following data were extracted: demographic characteristics, laboratory parameters, spleen size, total red cell mass, serum erythropoietin (EPO) level, and spontaneous growth of the BFU-E colony. Results: Patients with SE were younger, with a predominance of the male gender and with significantly higher serum EPO values than patients with PV. Patients with PV had significantly higher values of BFU-E, leukocyte and platelet count, spleen size, and LDH level than patients with SE. Total red cell mass analysis did not show a differential diagnostic significance. This condition is frequently associated with long term therapy in addition to a greater incidence of early and late complications. The goal of treatment is to alleviate pressure on the spinal cord caused by hematomas or bone fragments, in the shortest time possible. Hence, surgical decompression of the spinal cord is the first line of treatment, in cases where this approach is indicated. Apart from invasive methods, treatment also consists of the use of various pharmacological agents, whose therapeutic goal is to decrease edema and inflammation, especially in the first several days following injury. One of the most commonly administered drugs in such cases is methylprednisolone, however, controversy with regards to the timing of its administration and proper dosing, still exists. This drug has been in use for decades in the treatment of spinal cord injuries with various protocols having been introduced and revised overtime. The aim of this article is to showcase the current understanding of the use of corticosteroids in acute spinal cord injuries. The most significant protocols in use today for the administration of methylprednisolone (National Acute Spinal Cord Injury Studies-NASCIS I, II and III), along with a brief overview of pertinent literature, are discussed in this paper. Analysis of the available data suggests that the use of methylprednisolone in spinal cord injuries is still highly controversial due to the inconclusive relationship between the therapeutic benefits and the risk of side effects. The aim of this study is to assess the frequency and types of relapse, in relation to the time of occurrence; analyze the influence of conditioning regimens on relapse occurrence; review the therapeutic options after the occurrence of relapse; assess the prognosis in patients with relapse. Methods: This retrospective cohort study included 58 patients who underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT). Pre-transplantation therapy was performed with a reduced-intensity conditioning regimen (RIC) or a myeloablative regimen (MAC). The diagnosis of relapse was made through myelogram analysis, analysis of cytogenetics, analysis of minimal residual disease (MRD), analysis of cellular chimerism, and analysis of immunohematological chimerism of blood group antigens. A database was formed in relation to the examined patient characteristics. Patient survival was analyzed using the Kaplan-Meier method and the log-rang test. Results: MAC (43 patients) was used more frequently than RIC (15 patients), as a conditioning regimen. After transplantation, 18 (34%) out of 53 patients had a relapse. The choice of regimen did not affect the occurrence of relapse, but patients on the RIC regimen lived longer (38.5 ± 7 months) as compared to patients on the MAC regimen (27.8 ± 3.5 months). However, the difference in survival was without statistical significance (p = 0.318). The median survival time of patients who relapsed was 26 ± 5 months, while patients without disease relapse had a median survival time of 41 ± 4 months. Conclusion: Patients who received reduced-intensity regimens (RIC) had a longer survival time, without an increase in the relapse rate. The disease itself is characterized by a large number of patients with moderate and severe clinical presentation, who require hospital treatment. The organization of the healthcare system of Serbia, during the aforementioned pandemic, has required the engagement of all doctors, regardless of their specialty, in the treatment of patients with COVID-19. Surgeons of all branches, from the Surgery Clinic of the Clinical Hospital Center Zemun, were directly engaged in the treatment of both primary manifestations of the virus and the numerous surgical complications arising in the wake of this disease, but also in the treatment of primary acute surgical diseases in COVID-19-positive patients. Aim: The aim of this paper is to present the functioning of the Surgery Clinic of CHC Zemun, in the conditions of the COVID-19 pandemic, as well as to present the types and results of surgical procedures performed in patients with the COVID-19 infection. Methods: This study was conceived as a retrospective study and was conducted in the period between February 2020 and April 2021, in patients who had undergone emergency surgery and in whom the COVID-19 infection had previously been verified. A total of 232 patients surgically treated at the level of the entire Surgery Clinic of the CHC Zemun participated in the study. Results: Chest drainage, due to the development of pneumothorax, accounted for a quarter of all surgical procedures performed, while, in 53.85% of cases, surgical treatment was undertaken due to acute abdomen of various etiology, and in 21.15% of cases, due to vascular diseases. Conclusion: The COVID-19 pandemic is, in itself, a major challenge for the entire healthcare system. The role of the surgeon is significant, both in organization and in direct treatment, which is additionally complicated by the uniqueness of the entire situation and the severity of the disease itself. Job satisfaction contributes to the improvement of the quality of service, in every profession, and it is especially important in the field of work of professionals whose service should contribute to better and more efficient health care, and therefore it has been an important research topic for a number of years. The health care sector is facing many challenges. Therefore, providing a sufficient number of well-qualified and motivated staff will be a particular problem in countries facing a marked increase in the demand for health care. It is estimated that the shortage of health workforce will reach 12.9 million health workers by 2035. The health sector of the Republic of Serbia is facing similar challenges, but it also has its own specificities, conditioned by the historical, socio-economic, cultural, and political development of the country. Despite the significant development of medicine in recent decades, the mortality rate of sepsis is exceptionally high (about 26%). The definition of sepsis developed together with the understanding of the mechanism of sepsis. At the beginning of the 1990s, the first definition of sepsis was created, according to which sepsis was defined as systemic inflammatory response syndrome (SIRS), with the presence of suspicion or evidence of infection. Due to many inadequately diagnosed patients with sepsis, another definition was created, according to which sepsis is defined as a clinical syndrome. As the result of the lack of clarity of the previous definitions and the new information on the pathophysiological process of sepsis, a third definition of sepsis was developed in 2016. According to this definition, sepsis is a life-threatening condition based on organic dysfunction resulting from the body's inappropriate response to infection. Septic shock manifests as circulatory, cellular and metabolic instability. It is characterized by a serum lactate level higher than 2 mmol/l and hypotension, which requires vasopressor therapy after the administration of intravenous solutions. Materials and methods: In a cross-sectional study, 10,407 employees from 37 companies in the Netherlands participated in a Web-based health risk assessment, between 2010 and 2014. Self-reported short-term (<9 days) and long-term (10 or more days) sickness absences during the preceding 12 months were assessed. The questionnaire also asked about productivity loss at work, physical and psychosocial work-related factors, and health problems. Log-linear models were used to estimate prevalence ratios (PR), with 95% confidence intervals (CI). Results: Women, more often than men, experienced short-term and long-term sickness absence (PR 1.06, 95% CI 1.01 - 1.11, and PR 1.33, 95% CI 1.21 - 1.46, respectively) but were less likely to have productivity loss at work (more than 30%), (PR 0.90, 95% CI 0.81 - 0.99). In short-term sickness absence, gender differences were reduced by 20%, after controlling for psychosocial work-related factors, and by 60%, after controlling for health problems, separately. None of the factors mentioned above could explain a large productivity loss at work among men. Conclusions: Higher prevalence of sickness absence among women can partly be explained by psychosocial work-related factors and health problems. It most often leads to severe mucocutaneous, gastrointestinal, urinary, and, rarely, intracranial bleeding. This disorder occurs due to the production of antibodies against clotting factor VIII (F VIII), which interfere with its normal function. In laboratory analyses, prolonged activated partial thromboplastin time (aPTT), which cannot be normalized after being mixed with pooled normal plasma, is noticeable. Case report: In this article, the clinical course of the disease is described in a patient with acquired hemophilia, who was treated with oral anticoagulant therapy, and who initially also had prolonged prothrombin time, measured in international normalized ratio (INR) units, which measure how long it takes for a clot to form in a blood sample. Hemorrhagic syndrome was explained by iatrogenic effect. However, since bleeding continued after INR normalization, it was suspected that there was a different cause of hemorrhagic syndrome. The aPTT mixing test was performed (mixing an equal volume of the patient's plasma and normal pooled plasma (NPP) and repeating the aPTT test immediately and after one-hour incubation), after which the aPTT remained prolonged. This proved the presence of coagulation inhibitors, which is why acquired hemophilia was suspected. The patient was referred to a tertiary medical institution for further diagnostics and treatment. Conclusion: The objective of this case report is to show that patients with hemorrhagic syndrome, who are on anticoagulant therapy, may develop hemorrhagic syndrome for a different, non-iatrogenic reason. The aim of publishing research is to promote an original idea or to improve existing ideas, with the use of reliable sources. When writing different types of publications (research and professional papers, doctoral theses, books, etc.) it is necessary to state the sources that the ideas or text related to that publication were taken from. Science did not start with the authors of any individual research paper nor will it end with them - whatever the topic of the research is, there is a high probability that someone has already written something about it. If other authors' ideas or text are used in one's research, this must be stated in the text of that research. The standardized method of referring to sources is citing. Sources are cited in three ways: by quoting, paraphrasing, and summarizing. Citing sources may be direct and indirect. Sources may be referred to in the text (citing), at the end of the page (footnote), or at the end of the paper (endnote). Different sources may be cited: books, scientific journals, pictures, videos, audio sources, etc. The basic citation unit is a reference. Software for reference management enables the authors of publications to gather and organize references, insert quotes into the text, and create a bibliography. There are many such different programs. Some can be used free of charge (Mendeley, Zotero), while others must be purchased (EndNote). With the development of Internet technology, this type of software is also developing new functions in order to facilitate cooperation and networking amongst researchers, as well as to assist in the development of academic social networks.
