Mrdovic","doi":"10.5937/smclk2-32938","DOIUrl":"https://doi.org/10.5937/smclk2-32938","url":null,"abstract":"In December 2019 in the city of Wuhan, in China, the first cases of infection caused by the new SARS-CoV-2 virus appeared, and later on, the disease caused by this virus was named COVID-19. Shortly after this, on March 11 th , 2020, the WHO characterized COVID-19 as a global pandemic. The symptoms of COVID-19 and acute cardiovascular disorders (e.g., heart failure, pulmonary embolism or myocardial ischemia) frequently overlap, which poses a challenge for the establishing of a differential diagnosis in clinical practice. Rapid serological tests, which detect IgM and IgG classes of antibodies for SARS-CoV 2, have been developed with the primary purpose of screening the population's immunological response to the SARS-CoV-2 virus. However, rapid serological tests are often used outside their original purpose, i.e., for the triage of possibly infected, non-vaccinated individuals, because they offer quick results, which may be particularly relevant in emergency settings. If serological testing is used to guide the admission of non-vaccinated patients with acute cardiovascular disorders to either an isolation unit for suspected COVID-19 positive individuals, or to hospital facilities for non-infected patients, it is important to recognize its limitations, in order to reduce the risk of false-positive or false-negative results. Hence, appropriate patient selection and cautious test interpretation is necessary to avoid misdiagnosis. The aim of this paper is to illustrate how serological testing may be used as a screening tool to inform the management of non-vaccinated patients with acute cardiovascular disorders requiring urgent hospital admission. Changes in transplantation strategies and improvement in supportive care have significantly altered the incidence and pattern of infections in these patients. Aim: Evaluating the frequency of infections in the first 30 days after AHSCT, as well as the possible influence of the number of CD34 + stem cells in the graft and of the engraftment parameters: ALC500_20 (absolute lymphocyte count 0.5x109 / L per day + 20), ANC500_11 (absolute neutrophil count 0.5x109 / L per day + 11), and PLT20_13 (platelets 20x109 / L per day + 13), on the occurrence of infections. Materials and methods: The retrospective cohort study examined 80 patients above the age of 20 years, diagnosed with multiple myeloma (MM), non-Hodgkin's (NHL) or Hodgkin's lymphoma (HL), treated at the Clinic for Hematology of the Clinical Center of Serbia, in the period between July 2006 and December 2017. All episodes of fever and/or documented infection during neutropenia have been reported. Results: The average survival after AHSCT was 34.5 months. A total of 54 patients (67.5%) had a documented infection. Gram-positive infections were five times more common than gram-negative. In gram-positive isolates, coagulase-negative staphylococcus - CoNS was the most common (37.0%) pathogen, followed by Streptococcus a haemolyticus (12.4%). Among gram-negative isolates, Escherichia coli was present in 62.5% of the cases, while Klebsiellaspp. and Ralstonia pickettii were represented with an equal frequency of 12.5%. Fungal infections were rare (Candida spp., 10.0%). Viral infections were verified in 5 (6.3%) patients (Herpes zoster virus 3.8% and H1N1 2.5%). It is characterized by intolerance to physical and cognitive effort that the individual previously tolerated well, accompanied by weakness and other symptoms. Aim: The aim of this study was to examine the frequency of the subjective feeling of fatigue, three months after recovery from the infection, amongst patients who had been infected with COVID-19, as well as to analyze correlated factors. Methods: This cross-sectional study included 110 patients who had previously contracted COVID-19, and who came for a regular follow-up examination at the Health Center Kosovska Mitrovica, 3 months after recovery from acute infection. A specially designed questionnaire was used, which consisted of the segment with sociodemographic data, the segment with the data on the patient's health status and the course of their COVID-19 infection, as well as the segment of the questionnaire related to self-assessment of the patient's health status after the COVID-19 infection. Results: In our sample, out of 110 respondents, as many as 52 felt fatigue. Factors that were statistically significantly associated with the subjective feeling of fatigue, three months after treatment, were the age of the subject and the presence of chronic disease (p < 0.05). The presence of fever and pneumonia during infection were statistically significantly associated with the presence of the subjective feeling of fatigue, three months after treatment (p < 0.05). Hospitalized patients and those who were hospitalized for a longer period of time were also more likely to feel fatigue three months after recovering from the infection (p < 0.05). Savic","doi":"10.5937/smclk3-37604","DOIUrl":"https://doi.org/10.5937/smclk3-37604","url":null,"abstract":"Introduction: Lymphangioleiomyomatosis (LAM) is a rare disease which mainly occurs in women in the generative period, as well as during pregnancy, while only a few individual cases have been described in men. It occurs in sporadic form or is associated with tuberous sclerosis complex. The diagnosis can be made on the basis of high-resolution computed tomography (HRCT) findings, or histopathological analysis is required. Clinical manifestations of the disease include the following: progressive dyspnea on exertion, recurrent pneumothorax, chylothorax, angiomyolipomas and lymphangiomyomas. Case report: A 32-year-old female patient was admitted to our clinic, in her third trimester of pregnancy, after a left-sided pneumothorax was verified on chest X-ray. Initial treatment included needle aspiration, followed by thoracic drainage of the left pleural cavity. Due to the prolonged air leak through the thoracic drain and the advanced stage of the pregnancy, a caesarean section was performed. After delivery, chest X-ray revealed complete right-sided pneumothorax, which required thoracic drainage, as well as an insufficiently reexpanded left lung. HRCT was performed and cystic bullous changes in the lungs were noted; LAM was histopathologically verified through a minimally invasive thoraco-surgical approach, first on the left and then on the right side, while bilateral pneumothorax was surgically treated. Conclusion: Pneumothorax is a common complication of LAM. Due to the high recurrence rate, definitive early surgical intervention should be performed. Current guidelines recommend chemical pleurodesis and surgery for the first pneumothorax. The prerequisite for successful treatment of talus neck fractures is knowledge of anatomy, understanding of the mechanisms that lead to fractures of the talus neck, knowledge of the potential complications of all treatment methods, as well as knowledge of indications for surgical treatment. About 55% of the talus surface is covered with articular cartilage, and displaced fractures lead to the destabilization of several joints. Since fractures are caused by high energy trauma, the result can easily be comminution and/or dislocation (displacement). Fractures of the talus neck can occur as an isolated injury, as well as part of polytrauma (falls from height, traffic accidents). The X-ray is the basic diagnostic tool for making an accurate diagnosis, in case of suspect talus fracture. Multi-slice computerized tomography is the most useful method for studying fracture patterns and is indispensable in planning surgical treatment. The Hawkins classification of talus neck fractures, from 1970, has remained in use to this day, while recommended treatment methods vary depending on the type of fracture. The main goal of treatment is anatomical reduction. The anatomical characteristics of the talus, the particular blood supply, as well as the \"high energy\" mechanism of fracture, pose a challenge for clinical evaluation and optimal treatment of talus fractures.
