The most common forms of NMSC are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The continuing rise in the incidence of NMSC will translate to greater need for surgical interventions. Objective This article aims to review the current literature regarding excisional surgical treatments of NMSC. Data sources A review of all literature using databases of Pubmed and Medline searching for keywords of ‘skin’ or ‘cancer’ or ‘surgery’ was carried out. Review methods All the titles and abstracts of all articles found were searched and relevant articles were selected. A further review of all the references mentioned in the selected studies was carried out and all relevant articles were added to the database. Results Although early or superficial NMSC can be effectively treated with topical agents[ii] [iii], there are numerous factors that call for surgical management[iv]. Broadly ‘surgical’ approaches include surgical excision, curettage, electrodesiccation, cryosurgery, and Mohs micrographic surgery (MMS). Surgical excision, curettage, and MMS are treatments that have the advantage of including histological evaluation. Conclusions Surgical management of NMSC remains the most reliable and the most convenient method of treatment of simple NMSC. The operator needs to remain mindful of the limitations of this modality. The surgeons who maintain an interest in this field should remain abreast of the developments in diagnostic technologies as well topical and non-excisional treatment modalities which are in use by our colleagues in dermatology. These cancers are usually diagnosed and treated outside hospitals by general practitioners and dermatologists and in skin cancer clinics 2 , but they are not legally notifiable and are not routinely registered by all states and cancer registries. Moreover, many BCC and SCC are treated using cryotherapy, and are not reported histologically, signifying that the incidence of these cancers is perhaps two-fold higher or more. Solid pigment/ink particles injected through the epidermis into deep dermis can cause multiple adverse reactions.We report the case of 46 years old women that was admitted to the plastic surgery outpatient clinic with ulcerative skin lesions on her right ankle's tattoo. According to her history, 2.5 years previously dual color tattoo (black bracelet with three red hearts in it) was performed in a professional tattoo clinic. On physical examination, the patient had a circular black tattoo pattern with 3 heart shape ulcerated skin lesions interposed in the tattoo on her ankle. Under local anesthesia the largest, central lesion was excised. Histology examination of the skin specimen revealed combined lichen planus reaction in the center of the lesion and peripheral granulomatous sarcoid type reaction. CASE REPORT We report a case of combined lechenoid and granuloumatous reaction developed in the red part of the 2.5 years old tattoo on the patient's right ankle. A 46-year-old woman admitted to plastic surgery outpatient clinic due to slow growing lesions on her right ankle tattoo. According to her history 2.5 years ago dual colored tattoo bracelet (black circumferential pattern with three red interposed hearts) was performed in a professional tattoo clinic. The lesions had been growing for 7 months. The dye used for the tattoo was “Millennium colorworks inc. Monthly red”. The main ingredients of the red pigment according to manufacturer annotation was CI#12475 (4-[[4(aminocarbonyl) phenyl]azo]-N-(2-ethoxyphenyl)-3hydroxy2-Naphthalene Carboxamide) combined with glycerin, water and alcohol. There were no signs of local inflammation of the surrounding skin or regional lymph node involvement. The patient is a heavy smoker, suffers from epilepsy, currently treated with Topiramate and Phenytoin. She does not have any known allergies. She has another two tattoos on her chest and abdomen (blue shark). She had an episode of deep vein thrombosis and pulmonary embolism in the past. The lesions on her ankles were previously treated by a dermatologist with local antihistamines and steroids injections for few months without any improvement. The local findings on her ankle were a black tattooed bracelet with 3 heart shaped, ulcerated skin lesions on red background of the tattoo, resembling Squamous cell carcinoma (figure 1). The biggest lesion measured 15 x 10 mm, restricted exclusively to the red part of the tattoo and was excised under local anesthesia by plastic surgeon preserving the main design of the tattoo. The wound had healed well with normal linear scar. Histological examination (hematoxylin eosin staining) revealed inflammatory response of the tattooed skin resembling hypertrophic lichen planus reaction in the center of the lesion and peripheral granulomatous reaction (sarcoid type). Howes","doi":"10.5580/2da","DOIUrl":"https://doi.org/10.5580/2da","url":null,"abstract":"Electronically modified oxygen derivatives (EMODs), such as superoxide anion and hydrogen peroxide, carry out highly sophisticated intra, inter and extra cellular signaling roles that are essential for normal biochemical functioning. Hydrogen peroxide is freely diffusible through cell membranes, can not be excluded from cells and is required for normal operation of many enzymes that maintain and promote health. Hydrogen peroxide can act as an enzymatic substrate or activate or inhibit redox sensitive enzymes. Large health related organizations, such as the American Cancer Society, continue to disregard and deride the potential beneficial involvement and contribution of EMODs, especially hydrogen peroxide, in the treatment of any disease condition, inclusive of cancer. Their position of disdain is not supported by an exhaustive review of the medical and scientific literature. Although hydrogen peroxide has on occasion been a proposed mutagen, oncogenic transformation primarily leads to an increase of cellular EMODs that renders these same cells selectively vulnerable to additional EMOD production via EMOD induced apoptosis or necrosis. Legitimate medical health agencies are obliged to embrace the widespread scientific supportive findings regarding prooxidant hydrogen peroxide existent in the literature and encourage in depth scientific investigation into prooxidant EMOD disease protection and prevention. Scientific fact must supplant unsupported skepticism or flawed conclusions based on outdated data. While emphasizing hydrogen peroxide, this review is aimed at presenting the scientific facts regarding prooxidant EMOD health applications and cancer therapy. Based on the scientific literature currently available, EMOD use has significant potential benefits in the treatment of a wide range of human pathophysiologies. Patient was kept on strict splintage and physiotherapy programme to encourage movements. The patient is on regular follow-up and further monitored physiotherapy has been planned to provide a better functional and aesthetic limb.Conclusion: The presentation of Post Burn Contracture axilla described here has not been mentioned in world literature so far and its presentation is a matter of discussion. Patients with these wounds initially present to Emergency Medicine specialists and General Practitioners (GP) and so a broad knowledge of the concepts involved will aid these doctors in correctly assessing injuries and deciding which patients need specialist referral. The factors that influence management of a leg injury are wound site and its relation to the relevant anatomy, the nature of the wound (e.g. crush, laceration, degloving), the energy involved, contamination and tissues involved, the presence of coexisting injuries and patient co morbidities. Advances in the surgical management of high-energy lower limb injuries mean that there are many more patients in the community with salvaged limbs who in the past would have had amputations and prostheses. This means that today’s GP is much more likely to encounter patients who have had complex lower limb reconstructive surgery. With thorough knowledge of the anatomy of the various flaps and their applications, one can be more precise in the resection of tumors, leaving open the possibility of a primary closure of the defect and early rehabilitation. A variety of reconstruction techniques are being used to repair midfacial defects including skin grafts, local and regional flaps, as well as free-tissue transfer, but each of these techniques has its rewards and limitations. Reconstruction for the bony component of midfacial defects range from the use of soft-tissue alone to vascularized bone. The muscle provides a large amount of well vascularized soft tissue, with minimal donor site morbidity [1]. Temporalis muscle flap holds immense potential for the reconstruction of different maxillofacial defects [1]. This phenomenon often indicates the beginning of involution and subsequent regression of the melanocytic nevus, a process that extends over a period of several months. The consensus is that the central lesion should be evaluated and biopsied if there is clinical suspicion of dysplasia or malignancy.Objectives: To determine the characteristics of halo nevi excised in southern Israel during a decade.Methods: The Soroka University Medical Center is the regional hospital of southern Israel with its pathology institute evaluating all of the cutaneous lesions excised in the Soroka University Medical Center and most of the outpatient clinics in the region. We conducted a retrospective study compiling demographic and histopathological data from the institute's computerized system on all patients that underwent an excision of a cutaneous lesion that later was diagnosed histopatholgically as a halo nevus between the years 1996 and 2005. Results: Thirty six lesions were diagnosed as halo nevi, with characteristics similar to previous publications. Two of the 36 lesions were not clinically suspicious for dysplasia but were histologically diagnosed as dysplastic nevi. The cyst should be excised completely in order to avoid recurrence. If the cyst ruptures during the procedure there is an increased infection risk and recurrence rates are higher. Several methods have been employed for excising sebaceous cysts but none guarantee complete excision without rupture 1, 2, 3, , and some techniques advocate it .
