Information regarding abdominal wall morbidities associated with the use of this particular flap is scarce. We hereby present a rare case of spontaneous rupture of the posterior rectus fascia leading to bowel evisceration in a patient who had previously undergone sternal reconstruction with a RAMT flap. We hypothesize that certain high-risk factors present in this patient, such as morbid obesity, inadequate nutrition, and chronic cough may have had a direct impact on the integrity and strength of the abdominal wall after surgery, eventually leading to evisceration. Although these substances have been shown to be at least partially replaced by bone in some experimental models, their long-term outcome in human cranial replacement or augmentation is not known. This study evaluated eight patients who had undergone cranioplasty using a hydroxyapatite cement. The cement was used in both inlay and onlay fashion. The mean follow-up interval was nineteen months and ranged from eight months to twenty-seven months. All patients underwent repeat CT scanning at the time of latest follow-up. Only one significant complication was noted, consisting of a postoperative persistent seroma that was resistant to repeat needle aspiration and required open drainage and removal of the material. CT scans revealed variable resorption of the cement in three cases. In the two cases in which the material was used primarily as an inlay, significant fragmentation of the material was seen. The appearance of the junction of the cement and the native bone was variable. While in some cases this interface could not be distinguished, in other areas it was seen as a distinct lucency. Although clinically minor asymmetries were noted in the appearance of the cranial vault and forehead, no patient was felt to be in need of revisional surgery. Even in areas of fragmentation, the reconstructions felt solid without evidence of mobility. No patient developed an infection or extruded the material. The patient is a 16-year-old male who sustained a right knee fracture-dislocation. There was disruption of the popliteal artery, which was repaired with a reversed saphenous vein graft. This surgery was complicated by a postoperative hematoma. After drainage of the hematoma, the patient developed a progression neuropathy over the next 6 months that involved the common peroneal nerve and to a lesser extent the tibial nerve. Foot drop, muscle wasting of posterior and anteriolateral compartment and sensory loss over the dorsal foot and lateral leg were also documented. Based on the history, clinical finding and electrodiagnostic studies, the patient had both a tibial and common peroneal neuropathy. The neuropathy was related to the accident, but the exact etiology was unknown prior to surgical exploration. Surgical exploration was performed 15 months following the injury after referral to our institution and thick fibrous scar tissue was found compressing both tibial and common peroneal nerves. These bands were released and complete internal neurolysis was performed on both nerves. Although uncommon, surgical nerve compression can occur following extensive scarring secondary to trauma or surgical procedures. This discussion will describe the history of compressive neuropathies in the popliteal fossa. This is a case report of simultaneous and immediate reconstruction of combined upper and lower lips and mandible with multiple flaps following resection of extensive squamous cell carcinoma. The patient was reconstructed with free fibula osteocutaneous flap, pedicled scalp flap, tongue flap, palmaris longus tendon sling and pectoralis major myocutaneous flap. Although important for local control, complications do arise with the use of this adjuvant therapy. We have recently noted changes in the ear architecture in patients that have undergone postoperative irradiation therapy. In an attempt to identify changes in the cartilage thickness, we conducted an animal model to determine the etiology of our clinical observation. Eighteen New Zealand white rabbits were utilized. Three experimental irradiated groups were designed for single dose external cobalt60 beam therapy at 20, 25 and 30 Gy. The right ear served as matched nonirradiated control. After two months the cartilage underwent quantitative histomorphometric analysis using an inverted microscope, digitized camera and computer software. Irradiation did not significantly affect the thickness of cartilage upon comparing control and irradiated groups. There was no change seen in vascularity or tissue composition that could be identified. In conclusion, it appears that alterations in thickness of the cartilage did not result in the clinical examples demonstrated. The etiology to the changes noted clinically are unknown, but may be related to fibrosis and scarring of the dermis/epidermis. We describe the management and care of large bulging wounds after lower limb fasciotomy in two patients. By allowing the wounds to heal by secondary intention, the patients are discharged from the hospital sooner resulting in a reduced length of stay; in addition they avoid repeated trips to the operating room, and circumvent unnecessary additional scarring that can result from skin grafting. This procedure offers many additional benefits to the patients including the benefit of a normal contour to the extremity and a more rapid return to daily activities following discharge from the hospital. Langstein","doi":"10.5580/1f19","DOIUrl":"https://doi.org/10.5580/1f19","url":null,"abstract":"As Plastic Surgeons, we are constantly bombarded with new technologies that promise such lofty benefits as lighter scars, faster healing, easier fat aspiration, all with the pretense of improving income. But, fads come and go, some even return in different forms, but precious few actually impact the way we practice our specialty over the long run. Yes, the automatic dermatome was a huge improvement, as was the introduction of fiberoptic-lighted retractors, and some endoscopic devices, and easy to use titanium plating systems. These advances have had palpable changes on our day-to-day surgical practices. There has been another quantum advance in our field, one that has slowly taken hold and is systematically having an increasing impact on what we do as Plastic Surgeons – the conversion to digital photography. This technology is fueling a silent “revolution” in our field, as well as in others, and has offered some very unexpected benefits that have materially changed how I do business. I must preface my remarks by stating that I wan not an early adopter of digital photography. I was not initially convinced that a digital camera would replace my trusty Ectachrome and Nikon 6006. At the time, I was quite attached to my 35 mm camera, which was my loyal veteran of hundreds of rolls of film since my residency. So, while I played around with one of the early generation Olympus digital cameras, I still shot back-ups with my 35 mm, the shots from which I ultimately used while routinely ignoring the digital shots. The Olympus was difficult to focus, especially on macro shots, used up batteries quickly, had poor resolution and meager memory space. This all changed with the next round of digital cameras. Figure 1 With the introduction of the 3-plus megapixel cameras, such as the Nikon 990, a little over a year ago, digital photography hit its stride. I can comment only on the 990, the model that I still use, and emphatically state that I am so pleased with it that I don't even remember where my SLR is. Colleagues who opted for Minolta models are similarly impressed. The cameras are quite easy to use after a gentle learning curve, offer more manual controls than I know how to use, and fortunately have “surgeon-proof” automatic modes under which I usually operate. A resolution in excess of 3 million pixels is not as dense as 35 mm ectachrome slides, but appears to be sufficient for publication quality and projection in a large audience. Memory is cheap these days, allowing storage of in excess of 100 high resolution images on a single card or stick.
