By measuring the various test subjects' on the following parameters, researchers can develop a basis to determine whether the training is successful in helping the patients improve their inspiratory muscle function: level of dyspnea based on the Borg score, maximum inspiratory pressure, number of hospitalizations due to exacerbations of their disease, inspiratory muscle strength and endurance, 6 minute walking distance, exercise tolerance, and health related quality of life (HRQL). Moosavi","doi":"10.5580/1bab","DOIUrl":"https://doi.org/10.5580/1bab","url":null,"abstract":"Background: Although corticosteroids are considered essential treatment for acute asthma and widely prescribed, we studied the value of oral corticosteroids in moderate persistent asthma (MPA) and severe persistent asthma (SPA), and comparison of those effects. Methods: One hundred-one patients with acute asthma (48 with MPA, 53 with SPA) were studied to determine the efficacy of oral corticosteroids. All patients received other bronchodilator treatment. The patients were divided into two groups depending on initial FEV1 (less than 60% predicted as SPA and FEV1 between 60 to 80% predicted as MPA), and treated with oral prednisone 40 mg daily for 7 days. Spirometric variable and percentage of change to baseline FVC, FEV1 and FEF 25-75% after treatment was calculated. Results: After 7 days of treatment, the increase in FVC, FEV1 and FEF 25-75% predicted values in both group of patients with MPA and SPA was significant (P<0.001). The percentage of FVC, FEV1 and FEF 25-75% improvement in patients with MPA and SPA was 19.3 ± 11.2% vs 61.6± 45.5%; 29.5 ± 14.2% vs 85.3 ±93.2% and 53±36.4% vs 128.7±108% respectively. Meriño","doi":"10.5580/1fed","DOIUrl":"https://doi.org/10.5580/1fed","url":null,"abstract":"Dental implants are artificial elements that are placed in the maxillae as a tooth replacement, and indicated normally for cases of edentulous or form part of the prosthodontic rehabilitation of patients who have undergone radical tumor resection in the oral and maxillofacial area. In our area, these are extremely popular and performed by many dental clinics, more frequently for cases of edentulous patients. More immediate complications include bone fracturing, tearing, hemorrhaging, nerve and soft tissue wounds, movement of the implant, sub-mucus emphysema, and tool breakage or the rupturing of obturations or adjacent prosthesis. Aspiration of foreign bodies is extremely rare in adults, but it may occur during the course of dental treatment. This complication can become a serious problem for the patient, because can be followed by a chain of further complications, including acute dysnea, pneumothorax, late laryngeal obstruction requiring tracheotomy, chronic pneumonia, pleural effusion requiring drainage, etc. In the cases of aspiration of foreign body is indicated the fiberoptic bronchoscopy and removal, that usually achieves extraction of these elements, usually without complications. If this technique failure, another possibility of treatment for try extraction of the foreign body is the rigid bronchoscopy. In the last case is indicated the surgery for resolve this complication, although for luck this last technique is required in a few number of cases. We present the case of a 65-year old male. He was admitted to the emergency room, accompanied by his dentist, who explained that while implanting a tooth, he had lost the screwdriver he was working with. The patient was asymptomatic, showing no signs of dyspnea or any other respiratory symptoms, and he was not sure whether he had inhaled a foreign object or not. Chest radiographs (Figure 1 and 2) were performed, that showed a small screwdriver in the lower airways. A fiberoptic bronchoscopy was indicated and performed (Figure 3); the screwdriver was seen impacted in the distal area of the intermediate bronchus. The screwdriver was removed endoscopically, without any adverse consequences. (video). In patients with emphysema, bronchiectasis, and bronchial asthma. α2-macroglobulin (α2-M) concentration was significantly increased as compared to control group. But in patients with COPD, α2-M concentration was comparable to that of control group. Antitryptic and antielastase activity in condition of COPD was found to increase significantly. But in the other three study groups antitryptic activity was found to decrease. Antielastase activity was not found to vary significantly in bronchiectasis, and was found to decrease significantly in emphysema and bronchial asthma. Pneumococcal infections are a common cause of respiratory infection in Nepal and worldwide. The data regarding the use of pneumococcal vaccine is lacking in Nepal. Objectives: The present study was conducted to collect the demographic details of the patients receiving pneumococcal vaccine, to identify the basis for vaccination of the patients and to study the use pattern of the vaccine in the hospital. Methods: The patient files of all the patients who were prescribed with pneumococcal vaccine during the period from 1st January 2006 to 31st December 2006 were obtained from the medical records department and the necessary details were collected as per the study objectives. Results: Altogether 24 patients received the vaccination. The mean ± SD age of the patients was 60.54 ± 20.06 years. Seventeen (47.22%) of the patients were suffering from COPD. It was found that 10 (41.67%) patients had diseases since the last 5 years. Three patients were on oral corticosteroids (prednisolone) at the time of administration of the vaccine. Conclusion: The study identified the pattern of use of the pneumococcal vaccine in a Nepalese hospital. Though the number of patients receiving the vaccine was low, there is a potential scope for improving the vaccination program.
