Ecocardiografía","authors":"José Eleuterio González","doi":"10.35366/108023","DOIUrl":"https://doi.org/10.35366/108023","url":null,"abstract":"Introducción: la cetoacidosis diabética (CAD) se define por la tríada bioquímica de cetonemia, hiperglucemia y acidemia, lo que conlleva un desequilibrio hidroelectrolítico potencialmente mortal. El miocardio es particularmente sensible a estas alteraciones y el electrocardiograma (ECG) es una herramienta poderosa para detectar estas anomalías. Material y métodos: pacientes de 18 a 45 años con diagnóstico de CAD, se realizó un ECG estándar de 12 derivaciones que se obtuvo al ingreso y posterior a la resolución. Se evaluaron los voltajes, duración, segmentos e intervalos por un cardiólogo experto. Se obtuvieron valores séricos de glucosa, electrólitos, pH, bicarbonato de sodio y brecha aniónica en el momento del diagnóstico y después de la resolución. Análisis estadístico: se utilizó el paquete estadístico SPSSv24. Se reportaron frecuencias, porcentajes, media y desviación estándar. Se usó el cambio medio promedio como medida de asociación entre el ECG 1 y 2. Se evaluó la correlación entre el QTc, los electrólitos y gases venosos usando una correlación de Pearson p < 0.05. Resultados: se evaluaron 86 pacientes. Las alteraciones más frecuentes en el momento del diagnóstico fue taquicardia sinusal (71%), QTc prolongado (22%) y depresión ST (15%). El QTc al ingreso no mostró correlación con variables bioquímicas; sin embargo, se observó correlación QTc en la resolución con el nivel sérico de potasio (Tabla 8.1.1). Electrocardiografía","authors":"bloqueo auriculoventricular, Escobar-Martínez María Paulina","doi":"10.35366/108024","DOIUrl":"https://doi.org/10.35366/108024","url":null,"abstract":"Introducción: el bloqueo auriculoventricular 2:1 se engloba dentro de los trastornos de la conducción de segundo grado, los cuales tienen una prevalencia de 2.7%. Para conocer su localización se requiere de un estudio electrofisiológico en donde además se llevan a cabo maniobras farmacológicas (respuesta a la adenosina) con lo cual se decide su terapéutica. Material y métodos: estudio observacional, retrospectivo, transversal y analítico de pacientes con bloqueo auriculoventricular 2:1 y prueba de 18 mg de adenosina durante estudio electrofisiológico. Se calculó sensibilidad, especificidad, VPP, VPN y curva ROC para determinar el mejor punto de corte de variables electrocardiográficas para predecir una respuesta a adenosina y requerimiento de marcapasos definitivo. Análisis estadístico: estadística descriptiva para todas las variables. Variables cuantitativas prueba de Kolmogórov-Smirnov. Para variables categóricas se utilizará la prueba de χ2. Variables cuantitativas con distribución normal se utilizará t de Student y para las variables con libre distribución u de Mann-Whitney. Resultados: se estudiaron 56 pacientes, la media de edad en el primer grupo fue de 68.2 ± 9.7 y el segundo grupo de 66.9 ± 8.5. La hipertensión arterial fue la comorbilidad más prevalente. Predominó el bloqueo completo de rama derecha de haz de His más hemibloqueo del fascículo anterior, el intervalo RR fue la variable estadísticamente significativa.
