{"title":"Analysis of Ultrasonic Pancreatic Histograms in Diabetes Mellitus","authors":"S. Enomoto","doi":"10.5361/JKMU1956.41.SUPPLEMENT_S98","DOIUrl":"https://doi.org/10.5361/JKMU1956.41.SUPPLEMENT_S98","url":null,"abstract":"Problems have been involved in the sonographical diagnosis of diffuse pancreatic and hepatic diseases in terms of its quantitativity and objectivity, because the diagnosis relies highly on the examiner's subjective judgement. This study was attempted for a easier and more objective assessment on the sonographical changes in the pancreas of the diabetics by analysing the histograms of echoic levels and pixel distribution. Fifty six patients with their age nearly equally distributed from 10 through 89 years with impaired glucose tolerance were subjected to the clinical study, along with 112 healthy controls of the similar age distribution. Sixteen other cases diagnosed as having diabetes mellitus both clinically and histologically and 13 controls having healthy pancreas and liver were chosen for the postmortem study. A 3. 5 MHz transducer was used for both studies. A region of interest (ROI) of a square centimeter was designated on sagittal scanning in the pancreas body and in the liver at the same depth from the body surface in the clinical study. Water immersion technique was employed for the postmortem study. To avoid multiple reflection, the ROI was placed inside the pancreas and the liver within the depth of 3cm below the water surface. A ROI consists of 3600 pixels and the luminance of pixels can be devided into 64 levels. The level of luminance having [the maximum number of pixels is given by \"L\" and the number of pixels is shown by \"M\" or the histogram at the L on the visual display. L : pancreas/liver (1) , L : pancreas-liver (2) , M : pancreas/liver (3) and M : pancreas-liver (4) were then calculated from the data displayed. Among the healthy controls, both (1) and (2) were significantly bigger in the group over 70 years of age than in those of the remaining age categories. The postmortem histology showed that the age-related change was due to degeneration and irregularity of acinar structure accompanied by fibrosis. Among the diabetics, (3) and (4) were significantly lower, compared with those of the healthy controls. The major factors possibly responsible include the influence of hepatic dysfunction on the acid and pepsin contents of gastric juice, insufficient hepatic inactivation of gastric juice secretion stimulating factors, reduced resistance of the gastric mucosa due to malnutrition, disturbed gastric mucosal circulation due to elevated portal pressure, and interactions between gastrointestinal hormones. The exact mechanism is still unclear. In a previous paper, we reported on the high incidence of gastric ulcer in patients with liver cirrhosis\" . This time the relationship between liver cirrhosis and gastric ulceration was investigated by determining the kinetics of gastric juice secretion, the levels of gastrointestinal hormones, the condition of the gastric mucosa, the site of gastric ulceration, and the presence of esophageal varices. {"title":"Drug Metabolizing Activity in Chronic Liver Disease: A Study Using Ultrasonically-Guided Needle Biopsy","authors":"Y. In 53 patients with chronic liver disease, 7-methoxycoumarin (7-MC) 0-demethylase and 7-ethoxycoumarin (7-EC) 0-deethylase activities in liver specimens obtained by ultrasonically-guided liver biopsy were measured and compared with the elimination half-life (T1/2) of theophylline and biochemical liver function tests. Both the 0-dealkylase activities toward 7-MC and 7-EC in the liver were gradually decreased with progressive histological impairment of the liver to 32. 8% and 39. 6% of the control level, respectively, in patients with liver cirrhosis. Both the 0-dealkylase activities toward 7-MC and 7-EC in the liver inversely and exponentially correlated with the T1/2 of theophylline. These two 0-dealkylase activities correlated well with serum albumin content (Alb) , cholinesterase (ChE) , prothrombin time (PT), hepaplastintest (HPT) , cholesterol ester ratio (E/T) , zinc sulfate turbidity test (ZTT) , the ratio of branched chain amino acids to aromatic amino acids (BCAA/AAA) or indocyanine green test (ICG) , but not with GPT, LAP or total cholesterol. The incidence of proximal gastric cancer and the incidence of stages I and I have been increasing. Investigation of survival rates indicates the necessity of curative resection of proximal gastric cancer. Proximal gastrectomy could achieve the aim of radical, i. e. , curative treatment, if it is performed in appropriate cases of proximal gastric cancer, that is : 1) carcinoma arising in the proximal third of the stomach without cancer expansion to the middle third of the stomach, 2) superficial cancer limited to submucosa invasion, and 3) no metastasis into lymph nodes along distal portion of stomach. These usually occur at the insertion or origin of biarticular muscles. In the present study, an attempt was made to analyze the mechanisms of these fractures using electromyography, electrogoniometry, and motion analysis of different sporting activities. Particular consideration was given to the stress imposed on biarticular muscles. Results showed that the avulsion site reflected the \"limiting factor joint\" of the resultant force output. The greatest stresses were concentrated at the hip joint (origin of the rectus femoris, tensor fascia latae and sartorius) during running, and at the knee joint (insertion of the rectus femoris) during jumping. EMG analysis of the knee extensors during kicking showed that only the rectus femoris exhibited marked discharge and little or no activity was observed in vastus medialis especially in unskilled subjects. Concentrations of alanine (Ala) , tryptophan (Trp) , histidine (His) and glucose in plasma in the fasted state were significantly lower, and the concentration of cystine (Cys) was significantly higher only in AHa than the control level. The glutamic acid (Glu) concentration was significantly higher than the control level in AHa (p<0. 02) , CAH (p<0. 001) and CPH (p<0. 001) patients. Tyrosine (Tyr) was higher than the control level in AHa (p<0. 02) , CPH (<0. 001) , CAH (p<0. 001) and LC (p<0. 001) patients, the lysine (Lys) level was significantly higher in the AHa (p<0. 02) and CPH (p<0. 05) patients, the IRG level was significantly higher in AHa (p<0. 001) , AHc (p<0. 01) and LC (p<0. 01) patients than the control level, and the valine (Val) level was lower in AHa (p<0. 01) and LC (p<0. 001) patients than the control level. The Trp concentration (sum of albumin bound and unbound type) was low in AHa patients whose aspartate transferase activity was on the increasing stage, but soon recovered to the normal level during the recovery stage. Significantly positive correlations were observed between plasma Ala and Trp (p<0. 01) , Ala and Val (p<0. 01) , Ala and FPG (p<0. 01) , Trp and His (p<0. 01) , Trp and Val (p<0. 01) , Trp and Glu (p<0. 01) , Trp and Tyr (p<0. 02) , His and Glu (p<0. 01) , Cys and Lys (p<0. 01) and Val and Glu (p<0. 01) in AHa, CAH and CPH. Significant negative correlations were found between Ala and Lys (p<0. 01). Trp and Lys (p<0. 01), Trp and IRG (p<0. 02) , Cys and Val (p<0. 02) , Cys and Glu (p<0. 01) , Val and Lys (p<0. 01), Lys and fasted plasma glucose (FPG) (p<0. 01) and between Glu and IRG (p<0. 05) in AHa, CAH and CPH. 6) Decrease ratio (DR) was defined as decrease at 3 hrs after glucose loading divided by it's initial value. The DRs which differed significantly from the control level in AHa were AlaDR and IRG DR (lower) , and leucine DR (Leu DR) and arginine DR (Arg DR) (higher) . The sum (total values of before, O. 5hr, lhr, 2hr and 3hr after glucose loading) of IRI (TIRO was higher than the control level in the LC, AHa, CAH, CPH and AHc patients in this order. The sum of CPR (XCRR) was higher than the control level in the AHa, LC, CAH, CPH and AHc patients. The concentration of Ala in plasma did not increase after glucose loading in any of the cases except in those with AHa, whose Ala gradually increased after glucose loading. Cirrhosis was apparent from increase in collagen content based on hydroxyproline determinationand from the histological evidence that fibrous tissue bands had completely encircled zones of hepatic parenchymal cells. In those rats, the amount of asialoglycoprotein receptor per gram liver weight decreased markedly, in inverse proportion to the collagen content, whereas the specific activity of the receptor did not change significantly. The decrease in the receptor content in cirrhosis appears to depend primarily on the decrease in the number of hepatocytes. In addition, the survival time of 99mTc-galactosylneoglycoalbumin which had been injected intravenously was prolonged in parallel with the advance of liver fibrosis. Hepatic clearance as expressed by the reciprocal of the half-life correlated well with the amount of asialoglycoprotein receptor. When fixed platelets were incubated with ITP plasama, the PBIgG value did not change with incubation time, whereas that of unfixed platelets increased with incubation time. On the other hand, with systemic lupus erythematosus (SLE) and leukemia plasma there was no significant difference for fixed or unfixed platelets. Furthermore, when 5 ,uM ADP was added to unfixed platelets prepared in ITP, SLE & Leukemia, or normal plasma, the detected PBIgG value increased slightly compared with that of unstimulated platelets. In ITP plasma, the time-dependent increase of PBIgG in ADP-stimulated platelets tended to higher than that in unstimulated platelets.
