Therefore, management and therapeutic mapping should be planned assiduously. Surgical treatment for SUI has been an optimal surgical intervention for over 50 years, beginning from Burch colposuspension and Kelly’s plication, to midurethral slings, tension free vaginal tapes, trans obturator tapes and autologous fascial slings. Even though synthetic midurethral slings represented the most common surgical approach since the mid 90’s, due to their minimally invasive character, nowadays, lots of concerns have been raised among medical community concerning their serious long-term complications such as, mesh erosion, vaginal extrusion, urethral erosion and injury, groin, thigh or pelvic pain, recurrent infections and dyspareunia. These complications forced FDA (Food and Drug Administration) to issue a warning against their use, leading the implementation concerning autologous facial slings come back in to the surface. Despite the fact that the use of autologous fascial slings depicts a more invasive and challenging approach to treat SUI, there are notably more benefits regarding postoperative outcome, as well as decreased rate of potential complications when compared to TVT (Tension-free Vaginal Tape) and TOT (Trans Obturator Tape). Although, more trials and data must be conducted in order to be considered as main approach. Aim of this analytic review consists efficacy’s depiction of the autologous fascial slings according to recent bibliography and the role they could play in the near future concerning optimal therapeutic strategy of SUI.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129099556","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is even more luckless that giving birth by elective CS based on the pregnant woman’s choice has become the first place among the justifications, notwithstanding its direct and long-term complications. Methods: This retrospective study reviewed data of all CS deliveries during the year 2022 at our hospital using electronic medical records in the hospital information system. Retrieved data include baseline demographic characteristics, mode of delivery, indications, and the type of CS, aiming to clarify the reason for CS to challenge the percentage of CS based on the yearning of the pregnant woman without a medical reason; to identify, analyze and try to solve the ethical problem raised up by the pregnant woman’s request for CS. Results: The results revealed two significant facts; a sturdy noteworthy increase in the percentage of pregnant women delivering by CS compared to vaginal delivery at 54% versus 45.6%, and a sharp increase in the CS deliveries on maternal requests at 22.78%. The main reason for this shifting practice is the previous one CS followed by a decision that was taken on personal and family convictions. These harvested results revealed a significant increase in the percentage of pregnant women not receiving proper antenatal counseling about the appropriate method of delivery, with improper justification to jump over nature. Conclusion: Cesarean section should be signposted when on earth there is any indication or menace of detriment to the maternal and fetal binomial. If in earlier times \"labor death\" was a fact of life, nowadays it is astonishing and disgraceful the death of a mother due to pregnancy-delivery-postpartum. The proclamation that vaginal delivery is better because it is \"natural\" cannot and should not be taken to the last consequences under the risk of bad luck. The best form of birth is the safe one. To provide every pregnant woman with the right to choose her child's mode of delivery is to arbitrate for her sovereignty, yielding her respect and pride, nonetheless, it should be minimalistic and not absolute under this banner. Minimizing the rate of primary CS carries the secret key to ideal obstetrical care.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"42 2","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133174049","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Fertility-sparing treatment options remain the mainstay of management where there is fertility desire. We present a case report of a 42-year-old Virgo intacta who had abnormal uterine bleeding due to multiple submucous fibroids. An open myomectomy was performed and she made good recovery. Combined oral contraceptive pills were commenced immediately after the operation and continued for 3 months. She had regular periodic bleeding of low volume, flow and duration. Thereafter, subsequent monthly follow-ups were uneventful as a normal regular menstrual cycle was restored. There were management challenges as she was Virgo intacta at 42 years of age. Measures to avoid intrauterine synechiae were limited and combined oral contraceptive was used with good outcome.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"29 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115720874","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Prognostic factors for the risk of relapse and overall survival such as tumor size, surgical margin, histological strain, and stromal growth are described. Materials and methods: Observational, analytical and cross-sectional study, which studied patients with a diagnosis of fusocellular tumor, treated in the Breast Pathology Service of the Marie Curie Municipal Oncology Hospital (HOMC) of Buenos Aires during the period 2003 to 2022. Quantitative variables were used and analyzed with measures of central tendency. For the comparison of these variables, the Chi 2/Fisher exact test was used. Results: A total of 58 patients with diagnosis of spindle-cell lesions were recruited, including Filode tumors, sarcomas, hamartomas, adenomyoepithelioma, aggressive fibromatosis, gigantocellular granuloma, fusocellular lesion. Conclusion: It was observed that early age (<40 years) is not a risk factor for relapse, with an increase in this index in patients with tumor size > 5 cm. Methods: An analysis was conducted on refractory OAB patients to determine which symptom (nocturia, daytime voids, urgency, leaks per day, pad use per day and time to reach the bathroom) most improved following a PNE. This was measured in two ways: 1) by patient responses to a questionnaire and 2) by determining changes in symptoms with use of a voiding diary. Patients completed a pre- and post-questionnaire by phone interview. Descriptive statistics were used to analyze demographic and baseline characteristics as well as responses to questionnaires. Changes in voiding patterns were analyzed using a paired-samples t-test. Results: Overall, 28 patients were included. Prior to their evaluation patients indicated they were most bothered by urgency (n=26, 92.8%), followed by number of leaks per 24 hours (n = 24, 85.7%). After their evaluation, symptom improvement was highest for number of voids at night (n = 20, 71.4%) followed by sense of urgency (n = 18, 64.3%). Paired-samples t-tests on pre- and post-PNE voiding diaries revealed significant improvements in number of daytime voids (p = 0.015), number of leaks per 24 hours (p = 0.001), and number of urges per 24 hours (p = 0.001). Conclusion: Voiding diaries alone cannot be used to accurately determine symptom improvement. Clinicians must take into account the degree to which their patients are bothered by their symptoms. In an elderly population, improvement in nocturia has important implications including decreased morbidity.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"29 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-02-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"117021537","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Their diagnosis is based on histology. Our work aims to determine the epidemiological, clinical, histological and therapeutic aspects of PTs. Materials and methods: This is a retrospective study conducted in the gyneco-obstetric II department at the hospital center Hassan II, between January 1st 2015, and December 31st 2020 of, on 22 patients histologically proved cases of phyllodes tumors. Results: The study was conducted on 22 women, for whom the average diagnosis age was 35.6years old from 19 to 63years. 63.6% of the patients were nulliparous. The main clinical manifestation was a unilateral breast nodule measuring between 1 and 15.5cm (5cm on average). Mammographic exploration in the majority of cases revealed hyperdense opacity, roughly oval, well limited, often with regular outlines. The histological type was benign in 68.18% of cases, borderline in 18.18% and phyllodes sarcoma in 13.63%. The treatment was conservative in 86.37% of cases (lumpectomy), and radical in 13.63% of cases (mastectomy). After surgical treatment, 2 patients had insufficient resection limits and subsequently benefited from tumor bed revision. After follow-up, two tumors locally recurred with a huge mass that took up the entire breast. The two patients who had undergone a mastectomy and whose development was marked by the appearance of a second recurrence on the mastectomy scar, one of which was operable and the second had pulmonary metastases hence the indication of chemotherapy. Conclusion: Out of this study, we noticed that Phyllodes tumors of the breast mainly affect young women. The diagnostic confirmation necessarily requires histological proof which is not often easy by only biopsy. The best management of this tumor remains adequate surgical resection with healthy margins, while emphasizing the need for monitoring because the risk of recurrence is always present. The prognosis is based on the histological characteristics of the tumor and the quality of tumor excision.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"26 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-02-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121916718","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This would mean improving primary care for women and their families, with a cost-effective activity practised by midwives. However, considering the experience of other countries where midwives undertake this activity, we believe it is essential that within the midwife's training in sexual and reproductive health, there is a high component of training in genetics in order to be able to carry out this task with the safety and effectiveness required by this change in care. The transition from hunting and gathering to the agricultural lifestyle might have triggered task of food fermentations, nowadays this task is carried on industrial level. Many parts of the human body; the skin, oral cavity, gut, and vaginal canal are populated by huge numbers of microbes. At birth, human gut is a sterile environment, however it will start to be colonized instantly after birth. Factors such as diet (formula or breast feeding) and type of delivery (either vaginal birth or abdominal) can both impact the colonization patterns. The pioneer microbes inhabiting the gut make permanent adaptations and thus determining the metabolic, physiological, behavioral, and immune development which will encourage vulnerability to diseases. Because Age and lifestyle are associated with alteration in microflora, therefore are of some causes of diseases. Latest research has shown that microbiota composition is remarkably different in diseases such as obesity and periodontal diseases with healthy individuals usually showing diverse, distinct, and temporary stable microbiota communities at these sites in comparison with individuals manifesting disease.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"20 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127811375","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

These treatments may cause premature ovarian failure in females and azoospermia in males. Non-oncological conditions may also require therapies that put women's and men´s reproductive potential at risk. Moreover, an increasing number of women today decide to postpone maternity beyond the age of 35 due to social reasons with a possible affectation of future fertility due to the effect of age. The development and evolution of assisted reproductive techniques offer these patients new alternatives to preserve their fertility. The objective of this review is to describe the different options of fertility preservation. Oocyte vitrification for female and sperm banking for male are the first line for fertility preservation at the present time. Other techniques such as ovarian and testis tissue preservation, in vitro maturation of gametes, pharmacological protection (GnRH agonists and antiapoptotic agents) and surgical treatments (oophoropexy, selective radical trachelectomy) will be described. One of the most important steps in optimizing the results of these fertility preservation procedures in cancer patients is the need for prompt and timely referral to reproductive medicine specialists. Fertility preservation should be integrated as part of the oncological healthcare. Optimal counselling from healthcare professionals should always be present.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"106 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131845830","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objectives: The main objectives of this study were: to evaluate the clinico-pathological characteristics of patients with breast cancer at the San Rafael Regional Public Hospital, Mendoza; and, additionally, to determine the knowledge of an independent cohort of women regarding breast controls in the peripheral Health Centers of this city. Material and method: For the epidemiologic analysis of breast cancer in San Rafael, the Mendoza Provincial Tumor Registry was consulted. To study the clinico-pathological variables, a decryptive, cross-sectional type study was conducted on 51 consecutive patients with breast cancer diagnosis, during the period January 2014 to July 2016, in the Gynecology Service of the Hospital “ . Teodoro Schestakow” of the City of San Rafael, Mendoza. To determine the level of knowledge regarding breast-river control, surveys were conducted in peripheral health centers in the neighborhoods of Pueblo Diamante, Constitution, El Molino, Villa Laredo and El Sosneado, San Rafael, with prior authorization from the Directorate of the Municipal Health Area. Results: Breast cancer in women corresponded to 33.9% of all cancers in the period 2008-2012, according to the Provincial Tumor Registry. The mean age at the time of diagnosis was 53 years (range: 24-81 years). Advanced stages (III and IV) were diagnosed in 41.2% of patients. The 96.1% of patients presented breast symptomatology at the time of diagnosis, being breast tumor (on self-examination) the most frequent. Only 22% of women surveyed perform annual breast control; and only 7% of patients refer to having a breast examination performed at the medical consultation. Conclusion: Breast cancer diagnosis in San Rafael is performed mainly in symptomatic patients, which reflects deficiencies in early diagnosis, breast controls and information on the female population. This represents an opportunity to conduct population-targeted prevention campaigns, as well as education of health effectors from peripheral centres.","PeriodicalId":272014,"journal":{"name":"Obstetrics & Gynecology International Journal","volume":"484 2","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"120897602","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
